Life is compared to a train journey with ups and downs. We board the train at birth with our parents but they cannot stay for the whole ride. We meet other passengers like siblings and friends who become important. Some enjoy the ride while others face difficulties. Passengers get on and off, some leaving a lasting impression while others depart quickly. Our companions may sit elsewhere but we can seek them out. Our final stop is unknown, as is when we will lose our traveling partners. The goal is to make the journey as pleasant as possible and contribute positively to others before we each reach our end.

People with mental illness have long experienced prejudice and discrimination. Researchers have been able to study this phenomenon as stigma and have begun to examine ways of reducing this stigma. Public stigma is the most prominent form observed and studied, as it represents the prejudice and discrimination directed at a group by the larger population. Self-stigma occurs when people internalize these public attitudes and suffer numerous negative consequences as a result. In our article, we more fully define the concept of self-stigma and describe the negative consequences of self-stigma for people with mental illness. We also examine the advantages and disadvantages of disclosure in reducing the impact of stigma. In addition, we argue that a key to challenging self-stigma is to promote personal empowerment. Lastly, we discuss individual- and societal-level methods for reducing self-stigma, programs led by peers as well as those led by social service providers.

Les stigmates du public sont la forme prédominante qui a été observée et étudiée, car elle représente les préjugés et la discrimination dirigés vers un groupe par l’ensemble de la population. L’auto-stigmatisation se produit lorsque les gens internalisent ces attitudes du public et par la suite, souffrent de nombreuses conséquences négatives. - 1. I’m First – This first level of motivation is exclusively self-centered; decision makers at this level do not consider other people’s feelings. For example, a person acting on this level of motivation might cry or yell or throw a temper tantrum to get their way.
- 2. I’m Afraid – This second level of motivation involves an awareness of others. However, but they are perceived as able to harm or stifle or encourage the decision maker. His concern focuses on how others affect him. A good example would be someone refusing to smoke, not because he believes it is bad for his health, but because fear that he could be caught or punished. Another way of looking at it is for someone to smoke not because he likes it, but he feels pressured by his friends to do so.
- 3. I’m Responsible – This third level of motivation involves the awareness that one’s decisions affect the well being of others as well as his. For example, one would be motivated not to smoke because he knows that it would be jeopardise their own as well as other’s safety and health.
