Federally Qualified Health Center
(FQHC) that offers primary medical,
dental and behavioral health
services for the entire family through
nine sites, including six Fremont-
based primary care sites, 2 dental
sites, and 4 mobile clinics.

Tri-City Health Center is a community health
clinic that serves approximately 26,000
patients, who predominantly reside in the
Fremont, Union City, and Newark.
The population served by TCHC is 37 percent
Asian, 34 percent Hispanic, 14 percent White, 6
percent black, 6 percent multiple, and 2 percent
indigenous.
The TCHC service population is incredibly
diverse, with people of many backgrounds
seeking out our services. 91 percent of our
patients are below the federal poverty level, and
37 percent come to us without any form of
private or public insurance coverage and need
enrollment assistance.
