Tri-City Health Center (TCHC) is a
Federally Qualified Health Center (FQHC)
that offers primary medical, dental and
behavioral health services for the entire
family through nine sites, including six
Fremont-based primary care sites, 2
dental sites, and 4 mobile clinics.
TCHC is committed to delivering excellent
health services in a caring, nurturing, and
respectful atmosphere and improving the
quality of life for every individual and
family in our community.
The organization provides a wide
spectrum of health education, healthcare,
and social services to everyone, without
regard to financial position, insurance
coverage, ethnicity, language, culture,
sexual orientation, citizenship, or any
other criteria.
2. Tri-City Health Center is a community health
clinic that serves approximately 26,000
patients, who predominantly reside in the
Fremont, Union City, and Newark.
The population served by TCHC is 37 percent
Asian, 34 percent Hispanic, 14 percent White, 6
percent black, 6 percent multiple, and 2 percent
indigenous.
The TCHC service population is incredibly
diverse, with people of many backgrounds
seeking out our services. 91 percent of our
patients are below the federal poverty level, and
37 percent come to us without any form of
private or public insurance coverage and need
enrollment assistance.

department will work with you to identify
your eligibility for free or low cost medical
services and/or insurance programs.
TCHC serves all patients, regardless of
age, race, or ability to pay, and a Sliding
Scale Program is available for those
patients who qualify, based on household
income and family size.

In addition to these clinics, we also have 4 mobile
health clinics, which provide services to at-risk
groups in the community, including teens and the
homeless.
