Air was likely introduced into the soft tissues during the procedure through the use of a high-speed drill and air syringe. The emphysema resulted in swelling of the right side of the face. Such occurrences are usually self-limiting but can potentially lead to life-threatening complications if the air spreads. The report recommends using a rubber dam and clamps for isolation during Class V procedures to prevent air emphysema by deflecting air pressure away from the gingival sulcus. Care should also be taken when using dental instruments that use compressed air.
