This memorandum provides a spot report on a re-stabbing incident that occurred in Butuan City, Philippines. According to the initial investigation, the victim, Jhon P. Rivera, was stabbed multiple times by a suspect while drunk at Rivera's Store in Brgy. Ongyiu. The victim received fourteen stab wounds and was rushed to the hospital but was declared dead on arrival due to blood loss. Evidence recovered from the scene includes a knife, blood stain, and saliva. The police are conducting a manhunt operation to apprehend the suspect.

SUBJECT : Spot Report on Re-Stabbing Incident

1. Reference : Stabbing Incident Transpired on October 01, 2016 at Rivera’s Store of Brgy.
Ongyiu, Butuan City
2. Initial investigation conducted by this office disclosed that OOA 6:00 o'clock in the evening
on October 2, 2016 the stabbing incident transpired. The victim was identified as Jhon P. Rivera,
single, 20 years of age, laborer, a resident on Brgy. Ongyiu, Butuan City. Jhon was drunk and he went
to Rivera’s Store to buy something to eat. While thereat, the suspect approached and asked the victim
to drink but the latter refused.
3. Initial investigation/interview with the witness reveals that the victim obtains fourteen(14)
stabbed wound to his differend part parts of the body. Based on follow-up investigation the suspect
found out that is under the influence of liquor of that said day. On the other hand the victim was
immediately rushed to the Butuan Medical Center to undergo medical treatment due to her multiple
injuries suffered but the doctor declared dead on arrival. Due to lossed of blood. Evidence recovered
at the scene to wit:
A. Knife
B. Blood Stain
C. Saliva
4. Cadaver of the victim is now laying at St. Peter Funeral Parlor. The team of PNP personnel
of this office conducted Manhunt Operation for the possible
apprehension of the suspect. Progress report follows.
