She told Robert a pregnancy would wreck her efforts to maintain her body. His desire to have a family was a goal he decided he needed to pursue with someone else. He left Kimi six months ago for a much younger woman and filed for divorce. Kimi began having issues with food during high school when she was on the dance team and felt self-conscious wearing the form-fitting uniform. During college, she sought treatment because her roommate became alarmed by her issues around eating. She never told her parents about this and felt it was behind her. Her parents are Danish and value privacy. They always expected Kimi to be independent. Her lack of communication about her private life did not concern them. They are troubled by Robert’s behavior and consider his conspicuous infidelity as a poor reflection upon their family. Kimi has moved in with her parents while she and Robert are selling the house, which has upended the balance in their relationship.

creates feelings of guilt as it contradicts her Catholic faith.
Symptoms appear most consistent with a generalized anxiety disorder(300.02). Criteria A, B,
E, F, are met as there is excessive worry and anxiety for more than a 6 month period over a
number of different events and activities (friends, family, sorority, religious faith, etc.), she finds
it difficult to control the worry, and symptoms do not appear to be related to the effects of a
substance or medication, and are not better explained by another disorder. More information
would be needed to establish whether she meets the physical symptoms of anxiety (criteria B),
as well as whether her feelings of fear and worry are causing a significant impact on her
activities of daily living.
She has also turned to alcohol and marijuana recently in order effort to cope with her sexual
identity struggles. It is unlikely that she meets the criteria for an alcohol use disorder (305.00),
or a cannabis use disorder (305.20), even a mild one. While she has increased her
consumption of alcohol and cannabis recently, the increase was intended as a means to cope.
She has not presented the indications of any cravings, unsuccessful efforts to quit or reduce,
skipping social events to consume, or symptoms of withdrawal. To be sure, more information
on these areas would be beneficial to rule out the diagnosis.

Abstract
As providers, it is vital to be open-minded, if you are not aware or do not understand a person’s
culture, show your patients respect. Be engaged, respectful, and ask questions. Learn from your
patients, this helps develop a rapport. As providers we are obligated to provide optimal care, if
we feel a patient's needs are out of our scope, the client should be referred to a provider that can
give them the help they need and deserve (Nichols & Davis, 2020).
The purpose of this paper is to provide documentation and a psychiatric comprehensive
assessment of a patient and family during a family therapy interview that highlights differences
in a multicultural family, respecting cultural norms and differences, and develop an
individualized treatment plan.

HPI: A 40-year-old female (Patti) and her five children (Sheela age 24, Sharleen aged 23
and present today, Shireen age 21 recently reunited with family, Armin Jr. age 18, and Sam age
15), of Iranian descent, has been in individual and family therapy for the last 18 months. Patti
came to the clinic related to chaos in the home after her daughter was finally reunited with the
family after ten years. The family initially had rejoiced and celebrated, and after a few weeks,
Shireen began to tell her mother and siblings the emotional, physical, and sexual abuse at the
hands of her father. She blamed and felt abandoned by her mother. Patti needed necessary
surgery to both her feet after an injury, that has now left her disabled and with chronic pain. The
additional burden of surgery and disability has increased tension and stress in the home. Patti
lives with her two sons; her daughters live on their own. Shireen recently moved out, marrying
someone the family does not know, with little contact. Mother speaks and sees Sheela and
Sharleen daily. Patti has increasingly felt alone, depressed, hopeless, and helpless wanting her
daughters to stay and help her at her home. A psychiatric provider has been referred for a
medication evaluation.
Past Psychiatric History:
General Statement: Patti and her family began therapy 18 months ago after daughter
Shireen reunited with the family after 12 years of separation. Shireen shared significant abuse

Cindy Hart is 23 years of age. She was born on 6/11/86, the fourth child born to Betty Hart (born 6/1/66) and Barney Hart (born 4/11/55). Her father, Barney Hart, was an alcoholic who drank daily and recently passed away from cirrhosis of the liver. Mr. Hart was extremely violent with his wife and his children. He repeatedly beat his wife throughout the marriage and was often violent with the children. Cindy and her three siblings were taken from the custody of their parents several times during their formative years because of their father's drinking, domestic violence, and their mother's inability to protect them. Cindy likely bore the brunt of her father's alcoholism and family violence because she was the youngest of her siblings, and her father's alcoholism was severe for the last 12 years before his death. Cindy's father could not hold a job because of his alcoholism, and her mother did not work, so the family had limited funds and finances were always a source of great strain and tension. Cindy's mother was ineffectual and inept as a parent. She allowed repeated beatings of the children by Mr. Hart. Her mental health is questionable.
Her older siblings are Noah, age 25; Colleen, age 26; and Laura, age 28. Noah is currently incarcerated after recently being found guilty of drug trafficking. He is to serve a 40-year sentence. Colleen has six children; three of whom have been removed by Child Protective Services (CPS) because of issues involving drugs, inadequate supervision, and her recent arrests for prostitution. Laura, who is the most stable of the siblings, is married, has one child, and is working full-time as a physician assistant. She has no ties with her siblings or her mother.
Cindy Hart has never been married and has two children: Benjamin, age 5 and Luke, age 3. The children have different fathers and neither father is involved on a regular basis with either child. Cindy is receiving Temporary Assistance to Needy Families (TANF) (i.e., welfare) and Medicaid as health insurance for the children. She lives in an apartment in a crime-ridden, rundown neighborhood. She did not complete high school and did not graduate with her class but instead she dropped out in the 11th grade. She had a series of drug binges during this period. She eventually did receive her GED and is very proud of that fact because only her sister Laura graduated.
Cindy works part-time in a day care center as an aide. She has a history of mental health treatment, which includes two psychiatric hospital stays (one at the age of 15 and the other at the age of 21). She currently takes antidepressant medication, has a dual diagnosis (DSM-IV-TR diagnosis plus a substance abuse diagnosis), and has very few social supports. She sees her psychiatrist monthly for medication consults but has missed her last two appointments. She also sees her social worker for ongoing mental health counseling twice per mon.

Much of the success of a group can be attributed to its composition. Group composition encompasses not only the group’s size and treatment focus but also characteristics of diversity such as gender, age, ability, and race and ethnicity. Imagine a group in which all members are of the same ethnicity except for the social worker. Or a group in which half are recent immigrants with limited language proficiency. Or one in which there is a truly diverse representation of cultures. How would each of these scenarios affect group dynamics and functioning? No matter the situation, a social worker must be poised to approach the group from a stance of cultural competence, humility, and sensitivity.

Manny and Donna Maldonado have been married for 42 years. Manny is age 65 and Donna is 63. Sarah,
Mike and Becky are the children of Manny and Donna Maldonado. Sarah is the eldest daughter,

Manny is Hispanic American and owns a 20,000-acre produce farm that has been in his family for
three generations. Although Manny speaks and understands English, he prefers to speak Spanish.
This creates a language barrier between Manny and other family members who do not speak
Spanish. Donna is fluent in Spanish, having learned the language from Manny and his family.
Donna works on the farm with her husband. She has long suffered from mood swings, which is
mostly frustrating to Manny. He says it is “brujeria,” meaning her moods are caused by witchcraft
and “mal d ojo” or “evil eye.” He believes someone put a spell on Donna. When this is believed to
be the case, the person will visit a Curandero (healer) who will perform a healing ritual.

She is unable to hold a job long-term. She now works on her grandparent’s produce

Josh, age 17, has been sneaking away with friends, smoking marijuana and skipping
school.
Evan, age 10, was recently diagnosed with leukemia; however, he has not yet started
treatments. Evan’s doctors have recommended chemotherapy, radiation, and a bone
marrow transplant. Sarah and Joe intend to follow this treatment plan.

How culture influences health beliefs
All cultures have systems of health beliefs to explain what causes illness, how it can be cured or treated, and who should be involved in the process. The extent to which patients perceive patient education as having cultural relevance for them can have a profound effect on their reception to information provided and their willingness to use it. Western industrialized societies such as the United States, which see disease as a result of natural scientific phenomena, advocate medical treatments that combat microorganisms or use sophisticated technology to diagnose and treat disease. Other societies believe that illness is the result of supernatural phenomena and promote prayer or other spiritual interventions that counter the presumed disfavor of powerful forces.Cultural issues play a major role in patient compliance. One study showed that a group of Cambodian adults with minimal formal education made considerable efforts to comply with therapy but did so in a manner consistent with their underlying understanding of how medicines and the body work.
Asians/Pacific Islanders are a large ethnic group in the United States. There are several important cultural beliefs among Asians and Pacific Islanders that nurses should be aware of. The extended family has significant influence, and the oldest male in the family is often the decision maker and spokesperson. The interests and honor of the family are more important than those of individual family members. Older family members are respected, and their authority is often unquestioned. Among Asian cultures, maintaining harmony is an important value; therefore, there is a strong emphasis on avoiding conflict and direct confrontation. Due to respect for authority, disagreement with the recommendations of health care professionals is avoided. However, lack of disagreement does not indicate that the patient and family agree with or will follow treatment recommendations. Among Chinese patients, because the behavior of the individual reflects on the family, mental illness or any behavior that indicates lack of self-control may produce shame and guilt. As a result, Chinese patients may be reluctant to discuss symptoms of mental illness or depression.
Some sub-populations of cultures, such as those from India and Pakistan, are reluctant to accept a diagnosis of severe emotional illness or mental retardation because it severely reduces the chances of other members of the family getting married. In Vietnamese culture, mystical beliefs explain physical and mental illness. Health is viewed as the result of a harmonious balance between the poles of hot and cold that govern bodily functions. Vietnamese don’t readily accept Western mental health counseling and interventions, particularly when self-disclosure is expected. However, it is possible to accept assistance if trust has been gained.

Cindy Hart is 23 years of age. She was born on 6/11/86, the fourth child born to Betty Hart (born 6/1/66) and Barney Hart (born 4/11/55). Her father, Barney Hart, was an alcoholic who drank daily and recently passed away from cirrhosis of the liver. Mr. Hart was extremely violent with his wife and his children. He repeatedly beat his wife throughout the marriage and was often violent with the children. Cindy and her three siblings were taken from the custody of their parents several times during their formative years because of their father's drinking, domestic violence, and their mother's inability to protect them. Cindy likely bore the brunt of her father's alcoholism and family violence because she was the youngest of her siblings, and her father's alcoholism was severe for the last 12 years before his death. Cindy's father could not hold a job because of his alcoholism, and her mother did not work, so the family had limited funds and finances were always a source of great strain and tension. Cindy's mother was ineffectual and inept as a parent. She allowed repeated beatings of the children by Mr. Hart. Her mental health is questionable.
Her older siblings are Noah, age 25; Colleen, age 26; and Laura, age 28. Noah is currently incarcerated after recently being found guilty of drug trafficking. He is to serve a 40-year sentence. Colleen has six children; three of whom have been removed by Child Protective Services (CPS) because of issues involving drugs, inadequate supervision, and her recent arrests for prostitution. Laura, who is the most stable of the siblings, is married, has one child, and is working full-time as a physician assistant. She has no ties with her siblings or her mother.

As future nurse practitioners, we will be providing care to patients with diverse backgrounds. Diversity means every one is unique and has individual differences in relation to race, ethnicity, gender, sexual orientation, socio-economic status, age, physical abilities, religious beliefs, political beliefs, or other ideologies (Queensborough Community College, 2018, para. 1). It is important for clinicians to understand that when caring for diverse patients, we must provide acceptance and respect so that a good ‘relationship’ can form, and optimal health care can be provided to that patient.

Social HX: Pt attended graduate school and is currently is an attorney for a large local firm practicing family law. Pt states she has “good health insurance” and is “very financially secure” at this time. Pt states she has a partner, DR, who is currently living separately from the patient. TJ lives in a large single-family home 10 minutes outside of the main city of Boise. The patient has one dog and no other pets including cats or exotic animals. The patient states they are planning on living together in her home after the baby arrives. TJ states she is in a monogamous relationship with DR and believes that her partner is monogamous as well. Hamilton Howard Albert Fish is a renowned name in serial killing, child rapes, and cannibalism. Case of Mrs. Hudson
Mrs. Hudson is a 37-year-old Haitian American woman who lives with her husband of 10 years and two children, ages 8 (son) and 2 (daughter). She has come to therapy to discuss recent problems she has been having when leaving the house. She describes a pattern of behavior that began four months ago while shopping with her children. She was shopping for clothes for her daughter when she began to experience the following symptoms: heart racing, shortness of breath, sweating palm, and tightness in the chest. She worried that she might be having a heart attack and notified the store clerk who called the paramedics. The symptoms abated once the paramedics arrived and she was told to check with her primary care doctor. Mrs. Hudson described two subsequent incidents with similar symptoms—one at her son’s school during a parent-teacher conference, and one at the office. She noted that she has begun spending much more of her time at home, has called in sick from work, and arranged for her husband to do the necessary shopping for the house. She has missed church for the past month and has not visited her family in the past two months. This has placed a strain on her marriage and she has exhausted her sick leave. If she doesn’t return to work soon she will lose her job. She has not experienced any symptoms in the past month.
In terms of history, Mrs. Hudson was born in Haiti in 1966 and lived with her paternal grandmother until the age of 7. Her mother and father emigrated to the United States shortly after Mrs. Hudson’s birth, where they married and had three more children (one girl and two boys). Mrs. Hudson’s mother sent for her when she was 7 and she lived with her mother, father, maternal grandmother, and siblings for the remainder of her childhood. Her mother had an elementary education and worked as a laborer in a sewing factory. Her father was a doctor in Haiti, but was unable to qualify to work as a doctor in the United States. According to Mrs. Hudson, he did not work during her childhood and her mother was the primary breadwinner. Mrs. Hudson described her relationship with her father as tense. She reported spending the majority or her life trying to gain his approval. Her father is now deceased (cancer). She reports being close to her mother and her siblings, particularly her sister. Mrs. Hudson’s development was within average limits and she reached all developmental milestones at the appropriate age. She is bilingual (Creole/French derivative is her native language) and she learned English once she emigrated to the United States. While Mrs. Hudson considers English to be her primary language she has never applied for citizenship. Mrs. Hudson completed college and medical school and worked part time as a primary care doctor in order to be home with her children. Mr. Hudson is a firefighter. Mrs. Hudson was raised Catholic and attended mass regularly as a child. She has since rejected Cath.

How culture influences health beliefs
All cultures have systems of health beliefs to explain what causes illness, how it can be cured or treated, and who should be involved in the process. The extent to which patients perceive patient education as having cultural relevance for them can have a profound effect on their reception to information provided and their willingness to use it. Western industrialized societies such as the United States, which see disease as a result of natural scientific phenomena, advocate medical treatments that combat microorganisms or use sophisticated technology to diagnose and treat disease. Other societies believe that illness is the result of supernatural phenomena and promote prayer or other spiritual interventions that counter the presumed disfavor of powerful forces.Cultural issues play a major role in patient compliance. One study showed that a group of Cambodian adults with minimal formal education made considerable efforts to comply with therapy but did so in a manner consistent with their underlying understanding of how medicines and the body work.
Asians/Pacific Islanders are a large ethnic group in the United States. There are several important cultural beliefs among Asians and Pacific Islanders that nurses should be aware of. The extended family has significant influence, and the oldest male in the family is often the decision maker and spokesperson. The interests and honor of the family are more important than those of individual family members. Older family members are respected, and their authority is often unquestioned. Among Asian cultures, maintaining harmony is an important value; therefore, there is a strong emphasis on avoiding conflict and direct confrontation. Due to respect for authority, disagreement with the recommendations of health care professionals is avoided. However, lack of disagreement does not indicate that the patient and family agree with or will follow treatment recommendations. Among Chinese patients, because the behavior of the individual reflects on the family, mental illness or any behavior that indicates lack of self-control may produce shame and guilt. As a result, Chinese patients may be reluctant to discuss symptoms of mental illness or depression.
Some sub-populations of cultures, such as those from India and Pakistan, are reluctant to accept a diagnosis of severe emotional illness or mental retardation because it severely reduces the chances of other members of the family getting married. In Vietnamese culture, mystical beliefs explain physical and mental illness. Health is viewed as the result of a harmonious balance between the poles of hot and cold that govern bodily functions. Vietnamese don’t readily accept Western mental health counseling and interventions, particularly when self-disclosure is expected. However, it is possible to accept assistance if trust has been gained.

Cindy Hart is 23 years of age. She was born on 6/11/86, the fourth child born to Betty Hart (born 6/1/66) and Barney Hart (born 4/11/55). Her father, Barney Hart, was an alcoholic who drank daily and recently passed away from cirrhosis of the liver. Mr. Hart was extremely violent with his wife and his children. He repeatedly beat his wife throughout the marriage and was often violent with the children. Cindy and her three siblings were taken from the custody of their parents several times during their formative years because of their father's drinking, domestic violence, and their mother's inability to protect them. Cindy likely bore the brunt of her father's alcoholism and family violence because she was the youngest of her siblings, and her father's alcoholism was severe for the last 12 years before his death. Cindy's father could not hold a job because of his alcoholism, and her mother did not work, so the family had limited funds and finances were always a source of great strain and tension. Cindy's mother was ineffectual and inept as a parent. She allowed repeated beatings of the children by Mr. Hart. Her mental health is questionable.
Her older siblings are Noah, age 25; Colleen, age 26; and Laura, age 28. Noah is currently incarcerated after recently being found guilty of drug trafficking. He is to serve a 40-year sentence. Colleen has six children; three of whom have been removed by Child Protective Services (CPS) because of issues involving drugs, inadequate supervision, and her recent arrests for prostitution. Laura, who is the most stable of the siblings, is married, has one child, and is working full-time as a physician assistant. She has no ties with her siblings or her mother.
Cindy Hart has never been married and has two children: Benjamin, age 5 and Luke, age 3. The children have different fathers and neither father is involved on a regular basis with either child. Cindy is receiving Temporary Assistance to Needy Families (TANF) (i.e., welfare) and Medicaid as health insurance for the children. She lives in an apartment in a crime-ridden, rundown neighborhood. She did not complete high school and did not graduate with her class but instead she dropped out in the 11th grade. She had a series of drug binges during this period. She eventually did receive her GED and is very proud of that fact because only her sister Laura graduated.
Cindy works part-time in a day care center as an aide. She has a history of mental health treatment, which includes two psychiatric hospital stays (one at the age of 15 and the other at the age of 21). She currently takes antidepressant medication, has a dual diagnosis (DSM-IV-TR diagnosis plus a substance abuse diagnosis), and has very few social supports. She sees her psychiatrist monthly for medication consults but has missed her last two appointments. She also sees her social worker for ongoing mental health counseling twice per month but has missed her last two appointments.

As future nurse practitioners, we will be providing care to patients with diverse backgrounds. Diversity means every one is unique and has individual differences in relation to race, ethnicity, gender, sexual orientation, socio-economic status, age, physical abilities, religious beliefs, political beliefs, or other ideologies (Queensborough Community College, 2018, para. 1). It is important for clinicians to understand that when caring for diverse patients, we must provide acceptance and respect so that a good ‘relationship’ can form, and optimal health care can be provided to that patient.

Hamilton Howard Albert Fish is a renowned name in serial killing, child rapes, and cannibalism. Case of Mrs. Hudson
Mrs. Hudson is a 37-year-old Haitian American woman who lives with her husband of 10 years and two children, ages 8 (son) and 2 (daughter). She has come to therapy to discuss recent problems she has been having when leaving the house. She describes a pattern of behavior that began four months ago while shopping with her children. She was shopping for clothes for her daughter when she began to experience the following symptoms: heart racing, shortness of breath, sweating palm, and tightness in the chest. She worried that she might be having a heart attack and notified the store clerk who called the paramedics. The symptoms abated once the paramedics arrived and she was told to check with her primary care doctor. Mrs. Hudson described two subsequent incidents with similar symptoms—one at her son’s school during a parent-teacher conference, and one at the office. She noted that she has begun spending much more of her time at home, has called in sick from work, and arranged for her husband to do the necessary shopping for the house. She has missed church for the past month and has not visited her family in the past two months. This has placed a strain on her marriage and she has exhausted her sick leave. If she doesn’t return to work soon she will lose her job. She has not experienced any symptoms in the past month.
In terms of history, Mrs. Hudson was born in Haiti in 1966 and lived with her paternal grandmother until the age of 7. Her mother and father emigrated to the United States shortly after Mrs. Hudson’s birth, where they married and had three more children (one girl and two boys). Mrs. Hudson’s mother sent for her when she was 7 and she lived with her mother, father, maternal grandmother, and siblings for the remainder of her childhood. Her mother had an elementary education and worked as a laborer in a sewing factory. Her father was a doctor in Haiti, but was unable to qualify to work as a doctor in the United States. According to Mrs. Hudson, he did not work during her childhood and her mother was the primary breadwinner. Mrs. Hudson described her relationship with her father as tense. She reported spending the majority or her life trying to gain his approval. Her father is now deceased (cancer). She reports being close to her mother and her siblings, particularly her sister. Mrs. Hudson’s development was within average limits and she reached all developmental milestones at the appropriate age. She is bilingual (Creole/French derivative is her native language) and she learned English once she emigrated to the United States. While Mrs. Hudson considers English to be her primary language she has never applied for citizenship. Mrs. Hudson completed college and medical school and worked part time as a primary care doctor in order to be home with her children. Mr. Hudson is a firefighter. Mrs. Hudson was raised Catholic and attended mass regularly as a child. She has since rejected Cath.

They moved to the United States because of political issues in Haiti. Ronald, the youngest son of this family, is 27 years old and lives at home with his mother and father. Ronald was in shock when the doctor informed him that he was HIV positive. He confessed to the doctor that he was gay, but he could not tell his family. He said that he did not want to bring shame to the family. Because he couldn’t be in a formal relationship disowning to his family and the Haitian community’s view of homosexuality, he has been very promiscuous over the years. What are Haitians’ views of homosexuality? If Ronald’s parents were to learn of his positive HIV status, how might they react if they are religious and traditional? Identify three major culturally congruent strategies a healthcare provider can implement to address HIV prevention practices in the Haitian community?
