Week 9 Application: Case Study Case of Mrs. Hudson Mrs. Hudson is a 37-year-old Haitian American woman who lives with her husband of 10 years and two children, ages 8 (son) and 2 (daughter). She has come to therapy to discuss recent problems she has been having when leaving the house. She describes a pattern of behavior that began four months ago while shopping with her children. She was shopping for clothes for her daughter when she began to experience the following symptoms: heart racing, shortness of breath, sweating palm, and tightness in the chest. She worried that she might be having a heart attack and notified the store clerk who called the paramedics. The symptoms abated once the paramedics arrived and she was told to check with her primary care doctor. Mrs. Hudson described two subsequent incidents with similar symptoms—one at her son’s school during a parent-teacher conference, and one at the office. She noted that she has begun spending much more of her time at home, has called in sick from work, and arranged for her husband to do the necessary shopping for the house. She has missed church for the past month and has not visited her family in the past two months. This has placed a strain on her marriage and she has exhausted her sick leave. If she doesn’t return to work soon she will lose her job. She has not experienced any symptoms in the past month. In terms of history, Mrs. Hudson was born in Haiti in 1966 and lived with her paternal grandmother until the age of 7. Her mother and father emigrated to the United States shortly after Mrs. Hudson’s birth, where they married and had three more children (one girl and two boys). Mrs. Hudson’s mother sent for her when she was 7 and she lived with her mother, father, maternal grandmother, and siblings for the remainder of her childhood. Her mother had an elementary education and worked as a laborer in a sewing factory. Her father was a doctor in Haiti, but was unable to qualify to work as a doctor in the United States. According to Mrs. Hudson, he did not work during her childhood and her mother was the primary breadwinner. Mrs. Hudson described her relationship with her father as tense. She reported spending the majority or her life trying to gain his approval. Her father is now deceased (cancer). She reports being close to her mother and her siblings, particularly her sister. Mrs. Hudson’s development was within average limits and she reached all developmental milestones at the appropriate age. She is bilingual (Creole/French derivative is her native language) and she learned English once she emigrated to the United States. While Mrs. Hudson considers English to be her primary language she has never applied for citizenship. Andres is a retired child psychiatrist who completed medical school in Honduras and committed his career to working with Latino children and families in a major metropolitan area. Andres’ wife is a clinical psychologist who still maintains an active practice. Andres has a good relationship with his children, seeing them at least once a week for dinner, and his granddaughter is the light of his life.
Approximately 6 years ago, Andres was diagnosed with a rare brain tumor and Parkinson’s disease. Prior to his diagnosis, Andres was still on staff at a hospital, jogged daily, and had plans to travel with his wife. In a short time, Andres’ health deteriorated significantly. He now uses a cane and walker to ambulate. His speech is slow and soft. He requires assistance to get dressed and eat at times due to severe tremors and the loss of dexterity in his hands. Andres has fallen on multiple occasions and therefore cannot go out alone. He suffers from depression and anxiety and is currently on medication for these conditions. Andres spends a majority of time at home reading. He has lost contact with many of his friends and almost all of his professional colleagues.
Andres presented for treatment at an outpatient mental health setting. His daughter suggested it because she was concerned about her father’s worsening depression. Andres came into treatment stating his family thought he needed to talk to someone. He complied, but was unsure if treatment was really necessary. Andres agreed to weekly sessions and was escorted to each session by an aide who helped him at home.
While Andres had difficulty stating specific goals in the beginning, the focus of treatment became obvious to both of us early on, and we were able to agree to a treatment plan. Across multiple spheres of his life, Andres was struggling with accepting his illness and the resulting disabilities. In addition, he was extremely socially isolated despite the fact that he lived with his family and they were supportive of his medical needs. Finally, Andres’ role and identity had changed in his family and the world overall.

Living in cramped spaces while homeschooling her children contributed to her isolation and mental decline. Her religious views became increasingly extreme, influenced by a minister she followed fervently. Despite warnings from doctors, she had a fifth child and stopped taking her medication, which led to a complete mental breakdown and the drowning of her children in a psychotic state. You are the RN caring for Mrs. Faurah at the follow-up appointment. Her Blood Pressure is now
132/86, HR 115, HR 100, and O2 saturation is 96%. She states she is feeling much better and the
provider has determined her condition has improved. She is encouraged to follow-up again in

Carlos is an intelligent and caring young boy who presents with significant potential to excel academically.
CHIEF COMPLAINT/PRESENTING PROBLEM: Carlos was referred for an evaluation becausehis parents and teacher indicate that Carlos is restless, and often requires reminders to help him stay on task. He is described as "constantly running around" and presenting with difficulties listening and following instructions.
HISTORY OF PRESENT ILLNESS: Carlos enjoys spending time with his friends, and participating in physical activities such as swimming, running and skating. He also enjoys participating in social events, and is often invited to play dates and birthday parties. While Carlos interacts well with peers his own age, his parents believe he is easily led and influenced by others. Carlos does get upset when he does not receive recognition or feels that he has been ignored. His teacher notes that he sometimes acts 'socially immature', and that he often demonstrates attention-seeking behavior.
Carlos has difficulty focusing and sitting still in class. He is able to 'hyper focus' on some activities of interest however he often has difficulty sustaining his attention at school. daughter became a U.S. citizen following her marriage to her husband, who she met in college.
The Faurahs found life in the United States very different from their native country in Saudi
Arabia and continue to adjust. They are both overall healthy, Mr. Faurah speaks fluent English
and Mrs. Faurah continues to work on the language. Most of their time has been spent taking
care of the grandchildren in the house but the grandchildren have now grown older and their
daughter and her husband have advanced their careers and spend much of their time working.
Mrs. Faurah has been fighting a respiratory illness for around 4 weeks. She presents to the
Urgent Care.

Diagnosis from physician: Pneumonia- she is given a prescription, steroids, and is being sent to
her Primary Care Provider for follow-up in three days.
You are the RN caring for Mrs. Faurah at the follow-up appointment. Her Blood Pressure is now
132/86, HR 115, HR 100, and O2 saturation is 96%. She states she is feeling much better and the
provider has determined her condition has improved. She is encouraged to follow-up again in
two weeks to ensure the pneumonia has cleared up and provided information on where to go/who
to call is her condition worsens.
Upon additional conversation, Mrs. Faurah states that that she is feeling the loss of her
role/confusion about her identity as a mother and as a grandmother due to the independence of
her grandchildren and her daughter. Mrs. Faurah has been an occasional smoker her entire life;
however, within the last six months she has started to smoke regularly again and is up to pack

CHIEF COMPLAINT/PRESENTING PROBLEM: Carlos was referred for an evaluation becausehis parents and teacher indicate that Carlos is restless, and often requires reminders to help him stay on task. He is described as "constantly running around" and presenting with difficulties listening and following instructions.
HISTORY OF PRESENT ILLNESS: Carlos enjoys spending time with his friends, and participating in physical activities such as swimming, running and skating. He also enjoys participating in social events, and is often invited to play dates and birthday parties. While Carlos interacts well with peers his own age, his parents believe he is easily led and influenced by others. Carlos does get upset when he does not receive recognition or feels that he has been ignored. His teacher notes that he sometimes acts 'socially immature', and that he often demonstrates attention-seeking behavior.
Carlos has difficulty focusing and sitting still in class. He is able to 'hyper focus' on some activities of interest however he often has difficulty sustaining his attention at school. Andy, a 16-year-old girl just returned from a 2-month stay at a residential facility in Utah. Her counselor referred her to YOU. During your first session, Andy and her mother, Jane, were present. Arnold, her father was to attend but was busy at work and didn't make it. During the session the mother and daughter sat next to one another and were even arm in arm at times. They reviewed the situation prior to her going to the facility. Basically, Andy was out of control and belligerent. She was critical of both parents and they realized they couldn't manage her and needed to get some relief from the conflict.
Andy presents as a normal teenager, but she was obviously quite sensitive, reactive to her mother, and frequently felt controlled by her mother. Andy was adopted. She is not Jane and Arnold's natural daughter. She is actually a niece. Her father, Arnold had a sister who was a drug abusing stripper, and gave birth to Andy when she was 20. Jane and Arnold raised and finally adopted Andy. Andy has never had
contact
with her birth mother. She is extremely angry about this.
Both Jane and Arnold are the adult children of alcoholics (ACOAs). Jane's father was a WWII vet and was disabled most of his life. His problems mainly consisted of mental issues and alcoholism. Prior to the commencement of this evaluation and psychological testing, Ms. Jones was advised of the nature and purpose of the evaluation. after a long and debilitating illness during which Lucille was his primary caregiver. During their
marriage, he worked at the sanitation department, and she was a homemaker. She continues to live
in the house where she and her husband raised their three children. Lucille receives a limited
income of approximately $2,100/month from her husband’s retirement pension
and Social Security; she owns her home and has no major outstanding debts. She receives
Medicare to cover her major medical expenses and a small supplemental health plan to cover any
outstanding medical costs. Her physical health is good, and she has not had any major illnesses or
surgeries, although she has not had a complete physical in over two years. Her favorite hobbies
are gardening and cooking. Lucille has two sons and one daughter, each living away from home
with their own families. Lucille’s daughter and one son reside in the local area; her other son lives
in another state.
Lucille’s major concern is about her daughter, Alice (33), who has battled substance abuse and
alcoholism since adolescence. At present, Alice is not employed and has had several encounters
with law enforcement for drug possession and intent to sell illegal substances. Alice has admitted
that she has used cocaine as well as other substances in the past. She has made several attempts to
go into drug rehabilitation, but she has never completed a program. Her siblings have essentially
disowned her. Alice has three children, Michael (6), Rachael (4), and Randy (18 months), who
was recently diagnosed with fetal alcohol effects (FAE). Lucille is not certain who is the father of
her grandchildren; it is a subject Alice refuses to discuss. Alice has repeatedly left her children
alone for several hours in their tiny apartment, and once she was gone for several days. Child
Welfare has interceded, but Alice continues to have custody of her children. Whenever Lucille
visits her daughter and grandchildren, the living conditions are filthy, there is little food in the
house, and there is talk of constant “visitors” to the house well into the night. Because of Alice’s
instability, Lucille has taken physical custody of her grandchildren without any redress from Alice.
Lucille’s family members are not aware of the stress Lucille is feeling about possibly having to
spend the rest of her life raising her grandchildren, including one with a disability. This causes
Lucille to often feel “down in the dumps,” resulting in overeating because, as she stated, “comfort
food makes me feel better.” Within 2 months, she gained 15 pounds.
Lucille heard about a counseling program at the local community center for grandparents
raising grandchildren. The program provides support, group meetings, parenting classes,
individual counselin.

Dear Judge Kirk:
Sue Jones is a 52 year old Caucasian female who was referred by the Court for a guardianship evaluation.
Dr. Betty Rubble interviewed Ms. Jones at Anytown Nursing Home on January 1, 2016 for approximately 105 minutes. She was administered the Independent Living Scales on that date.
Prior to the commencement of this evaluation and psychological testing, Ms. Jones was advised of the nature and purpose of the evaluation. Ms. Jones was informed that the resulting report was not confidential, and that information obtained could be included in the report that would be submitted to the Court. She was aware this information was not related to treatment, but rather for her current case. Ms. Jones was provided this information both orally and in a written format. She stated that she understood the information provided to her, including the limits of confidentiality and her rights concerning the evaluation.

SOCIAL HISTORY: Ms. Jones reported that she was born on January 1, 1963 and reared in Kentucky. She said her father worked as a security guard and died eight years ago, while her mother worked as a waitress and died five years ago. She identified having a “good” relationship with her parents. Ms. Jones said she has two brothers with whom she has an “all right” relationship, as well as one older maternal half sister that she “[doesn’t ] get along with at all.” Ms. Jones stated that one of her brother has been diagnosed with bipolar disorder, and noted her brothers and her father had difficulties with alcoholism. She denied any childhood history of abuse and reported that she ran away from home at 17 years of age when she became pregnant.
Ms. Jones reported that she lived independently until two years ago when she was placed in a nursing home. She said she remains in a nursing home against her will because the court has appointed her a legal guardian due to her alcoholism. She would like to return to her home of Nowhere, California, where her cousin lives. She said that she talks with her cousin regularly on the phone, but acknowledged that she has not seen her in many years. She does not want a guardian and would like to make her own decisions.
According to her legal guardian, Ms. Flintstone, prior to her nursing-home placement, Ms. Jones was in sober housing. That home had staff present on site, but Ms. Jones continued to drink alcohol and visit hospital emergency rooms to obtain opiates.
EDUCATION HISTORY: Ms. Jones stated that she last completed the 9th grade and had “all right” grades. She said that she was not diagnosed with any learning.

Overview
Your Final Project will be a medium-length essay in which you address the cultural viewpoints toward death and dying in Western society and compare these to the perspectives toward death and dying in other parts the world. As the course content will point out, much about the Western response to death and dying amplifies the process of grief and bereavement, perhaps unnecessarily. In the West, death is something that is denied and, in many ways, is something that people seem ill-equipped to deal with once the event occurs or is imminent. Your job in this paper will be to put on your social scientist cap and offer “cultural solutions” to the way that death and dying is dealt with in Western society.
Assignment
Based on your own independent research in which you compare the cultural beliefs toward death and dying in Western societies to those in Eastern societies such as China, Vietnam, Japan, and so forth, write an essay that addresses the following points:
Describe in detail the major differences between the Western viewpoint toward death and dying and those in these Eastern societies (you may focus on one country, such as China, or talk about Eastern societies more generally). Be sure to describe the rituals involved with the process of death and dying, the various technologies, or anything that helps explain these differences.
Give one specific example of a famous case of death and dying in Western society that demonstrates some of the problems or dysfunctions of the Western viewpoint (examining cases in the media, for example, will be a great place to start!).
Finally, conclude your final paper with a detailed commentary about what we can do to change the cultural beliefs toward death and dying in Western societies in a way that makes us less afraid of the dying process. Be very specific in your commentary.

- 1. Week 9 Application: Case Study Case of Mrs. Hudson Mrs. Hudson is a 37-year-old Haitian American woman who lives with her husband of 10 years and two children, ages 8 (son) and 2 (daughter). She has come to therapy to discuss recent problems she has been having when leaving the house. She describes a pattern of behavior that began four months ago while shopping with her children. She was shopping for clothes for her daughter when she began to experience the following symptoms: heart racing, shortness of breath, sweating palm, and tightness in the chest. She worried that she might be having a heart attack and notified the store clerk who called the paramedics. The symptoms abated once the paramedics arrived and she was told to check with her primary care doctor. Mrs. Hudson described two subsequent incidents with similar symptoms—one at her son’s school during a parent-teacher conference, and one at the office. She noted that she has begun spending much more of her time at home, has called in sick from work, and arranged for her husband to do the necessary shopping for the house. She has missed church for the past month and has not visited her family in the past two months. This has placed a strain on her marriage and she has exhausted her sick leave. If she doesn’t return to work soon she will lose her job. She has not experienced any symptoms in the past month. In terms of history, Mrs. Hudson was born in Haiti in 1966 and lived with her paternal grandmother until the age of 7. Her mother and father emigrated to the United States shortly after Mrs. Hudson’s birth, where they married and had three more children (one girl and two boys). Mrs. Hudson’s mother sent for her when she was 7 and she lived with her mother, father, maternal grandmother, and siblings for the remainder of her
- 2. childhood. Her mother had an elementary education and worked as a laborer in a sewing factory. Her father was a doctor in Haiti, but was unable to qualify to work as a doctor in the United States. According to Mrs. Hudson, he did not work during her childhood and her mother was the primary breadwinner. Mrs. Hudson described her relationship with her father as tense. She reported spending the majority or her life trying to gain his approval. Her father is now deceased (cancer). She reports being close to her mother and her siblings, particularly her sister. Mrs. Hudson’s development was within average limits and she reached all developmental milestones at the appropriate age. She is bilingual (Creole/French derivative is her native language) and she learned English once she emigrated to the United States. While Mrs. Hudson considers English to be her primary language she has never applied for citizenship. Mrs. Hudson completed college and medical school and worked part time as a primary care doctor in order to be home with her children. Mr. Hudson is a firefighter. Mrs. Hudson was raised Catholic and attended mass regularly as a child. She has since rejected Catholicism and now attends a nondenominational Christian church. She and her husband are physically healthy as is her older son. Her daughter was diagnosed with a severe heart condition at 14 months which required several surgeries to correct. Her daughter is still monitored regularly by her doctor. This is Mrs. Hudson’s first visit to a mental health counselor. She was referred by her primary care doctor.
