Infraworks is reinstating its policy that protection officers must report all safety incidents, injuries, and accidents to their safety managers without delay. This includes any safety breaches or near misses on the Sydney Trains Network. A recent incident where a ladder fell onto live tracks from a station platform was not immediately reported as an emergency, putting lives at risk. The safety alert serves as a reminder that all incidents must be reported to safety managers so they can determine if escalation to Sydney Trains is needed.

This Safety Alert reinstates
Infrawork’s policy with respect to
reporting Safeworking Incidents as
an absolute priority.
Our Protection Officer’s must report
all Safeworking breaches injuries
and accidents to their Safeworking
Manager.
The occurrence may amount to
nothing, but it will be up to
Infraworks Safeworking Manager to
determine the extent of the matter
and whether Infraworks will need to
escalate to Sydney Trains.
This alert serves as a reminder of
your reporting obligations.
An incident that occurred in the
Sydney Trains Network recently
where by a Protection was on the
platform of a station working
closely with a contractor using a
ladder near the edge of the
platform. A severe gust of wind
blew up and pushed the ladder of
the platform onto the live tracks.
The Protection Officer proceeded to
call the Signal Box to alert them of
the CAN, however according to the
PO, the Station Master proceeded
climbed down onto the live track to
retrieve the ladder.
In the first instance that first
conversation to the signal box
should have been “emergency,
emergency, emergency” due to
something as large as a ladder on
the track. The potential of a train
approaching unaware of the large
object on track was very real.
This incident did not come to light
until several days after the event.
