Rhinoplasty is a plastic surgery procedure used to correct and reconstruct the form of the nose, restore its functions, and enhance its aesthetics. There are two types of rhinoplasty procedures - closed rhinoplasty involves separating the skin and tissues from the nasal framework without making incisions. Open rhinoplasty requires incisions to separate the skin and tissues from the underlying framework. The goal is to create a functional, proportionate nose by correcting any issues with the nasal structures.

Nasal bone fractures comprise up to 50% of all facial fractures. Nasal fractures can be classified in two broad categories based on impact force: lateral-type versus frontal-type injuries. Lateral-type injuries tend to be more common, have fewer residual anatomic and functional defects compared with frontal injuries, and are more amenable to closed reduction. Frontal injuries classically produce a posteriorly displaced fracture where the nasal septum is always involved. They have a higher risk of residual post-surgical deformity, and as the impact force increases, nasal, orbital, and ethmoidal fractures occur in combination. The extent of the septal injury determines the appropriate technique for septal correction. Closed reduction of fractured nasal bone can be performed by elevation of depressed bones or depression of elevated bones to restore the symmetry of the nasal aperture. Septal injuries that cannot be realigned with a closed reduction should be addressed with open techniques. Symmetrical fixation of the bones, restoration of orbital volume, globe position, frontonasal angle, and nasal projection are essential for a satisfactory cosmetic outcome.

Background
The columellar strut graft is one of the most commonly used invisible grafts in tip plasty techniques for nasal tip projection. However, the columellar strut graft induces cephalic rotation of the dome with nasal tip projection. This is an effective change in Western people with a long nose; however, this change should be avoided in Asians who have a relatively short nose and visible nostrils. We designed a more convenient and effective technique using a rein-shaped columellar strut graft that can prevent cephalic rotation of the dome.
Methods
A total of 32 patients underwent surgery with a rein-shaped columellar strut graft with a septal cartilage. The projection and location of the nasal tip, nasal length, and nasolabial angle were measured after taking a photograph of the lateral view, and the preoperative and postoperative results were compared.
Results
There were statistically significant differences between the preoperative and postoperative values of the nasal tip projection ratio and nasal tip location ratio. There were no revision surgeries and no direct complications associated with the use of the columellar strut graft.
Conclusion
We performed tip plasty with a modified columellar strut graft—the rein-shaped columellar strut graft. In most cases of using this method, the tip projection was increased and the cephalic rotation of the tip was prevented. This surgical procedure can also be used for lengthening (rotating caudally) of the nose in some cases, as well as for the purpose of preventing the cephalic rotation of the tip.

Some disadvantages are disruption of minor tip support mechanisms and potential for tip ptosis. Precise grafting techniques can then be used to correct dorsal abnormalities, strengthen the nasal valve, alter tip projection and rotation, and address septal deviations.

The Chimeric technique for Asian rhinoplasty aims to control final radix position and preserve the nasal profile. It involves using a composite silicone-PTFE implant with a glabellar component made of cartilage or PTFE. 49 patients underwent rhinoplasty with or without the Chimeric technique. Those who received the Chimeric technique had a more favorable horizontal vector of radix position change compared to the traditional technique, which resulted in a more vertical vector and potential for an elevated radix deformity. There is increasing confidence that cell therapies will soon play a role in the treatment of autoimmune disorders, but the extent of this impact remains to be seen. Early readouts on autologous CAR-Ts in lupus are encouraging, but manufacturing and cost limitations are likely to restrict access to highly refractory patients. Allogeneic CAR-Ts have the potential to broaden access to earlier lines of treatment due to their inherent cost benefits, however they will need to demonstrate comparable or improved efficacy to established modalities.
In addition to infrastructure and capacity constraints, CAR-Ts face a very different risk-benefit dynamic in autoimmune compared to oncology, highlighting the need for tolerable therapies with low adverse event risk. CAR-NK and Treg-based therapies are also being developed in certain autoimmune disorders and may demonstrate favorable safety profiles. Several novel non-cell therapies such as bispecific antibodies, nanobodies, and RNAi drugs, may also offer future alternative competitive solutions with variable value propositions.
Widespread adoption of cell therapies will not only require strong efficacy and safety data, but also adapted pricing and access strategies. At oncology-based price points, CAR-Ts are unlikely to achieve broad market access in autoimmune disorders, with eligible patient populations that are potentially orders of magnitude greater than the number of currently addressable cancer patients. Developers have made strides towards reducing cell therapy COGS while improving manufacturing efficiency, but payors will inevitably restrict access until more sustainable pricing is achieved.
Despite these headwinds, industry leaders and investors remain confident that cell therapies are poised to address significant unmet need in patients suffering from autoimmune disorders. However, the extent of this impact on the treatment landscape remains to be seen, as the industry rapidly approaches an inflection point.

In the surgeries — closed rhinoplasty and open rhinoplasty — an otolaryngologist (ear, nose, and throat specialist), a maxillofacial surgeon (jaw, face, and neck specialist), or a plastic surgeon, creates a functional, aesthetic, and facially proportionate nose by separating the nasal skin and the soft tissues from the osseo-cartilaginous nasal framework, correcting them as required for form and function, suturing the incisions, and applying either a package or a stent, or both, to immobilize the corrected nose to ensure the proper healing of the surgical incision.[1]
