Duringthis time, I wasrequestedtoreturntoPhosphate Hill fora sevendayshut,at the endof thistime I was
requestedtostayonan extra4 days.Some of the workconsistedof changinglinersandscreensinthe Granulation
Plant.I wasalsoinvolvedwiththe removalof 2mspoolsat heights.EWPand craneswere bothinvolvedinthe
successful replacement.Once againmyabilitytoworksafelyamongstateamshone through.Iwas alsoinvolvedin
firstbreaksto replace valveswhichhave acidinthem, alwaysfollowingthe proceduresensuredasafe outcome. I
was alsoinvolvedinthe replacementof conveyorbeltrollers.

Withmy outstandingworkethicandrepourfrompreviousshut-downworkUGL requestedmypresence aspartof
the Demobilisationcrewforthe RioTintoYarwunshut-down.Once againIwas instrumentalinthe formulationof
JSA’sandgiventhe responsibility,withasmall teamto ensure the qualityassurance of partsandfixturestoensure a
successful shut.Asateam memberwe setuplaydownareasandworkfronts.Uponcommencementof the shutIwas
involvedinnumerousFirstBreaksalwaysensuringisolation/de-isolationprocedures were followedandconfirmed.I
was alsorequestedtofill inforthe leadinghandonhisfatigue day. CompletingnumerousTake 5’sbroughta great
awarenessof hazardsand insuredsafe workpracticeswere inplace enablingsafe productiveshut.

Duringthistime,I wasrequestedbyUGL to partake ina shortshut at Phosphate Hill.Here dealingwithdangerous
chemicalsSafetywasparamountinthe FirstBreakProcedure of numerousflangesandinspectionhatches.Thiswas
alsoa FIFOposition,Iapproachedthispositionwiththe understandingthatIwas a UGL representative bothatthe
worksite and incamp facilitiesalwaysconductingmyself inthe appropriate manner.

Duringthis time, I wasrequestedbyNRG(Gladstone powerstation) toreturnina more responsiblerole.Iwasgiven
the responsibilityof workinginateamto dismantle,inspectandrefurbishorreplace numerousvalvesandthe
alignmentof electricmotors.Workingina safe productive mannerIwasinstrumental indevelopingJSA andalways
completingpersonalandgrouptake 5’s ensuredasuccessful safe shut.

Duringthis period, Ihave beenemployedbyUGL as a Fitterfortheir shutdownatRioTintoYarwun. I have been
assessedandcertifiedonall relevantcompetencies.Workinginwithmycrew we have performedvarioustasks
includingmanyafirstbreaksfollowingthe Rioprocedure.Alwaysmindfulof everchangingconditionsourtake 5
checklistare a valuable tool tocomplete anytask. Tensioningandtorqueingmanyflangesusingcorrecttooling as
requiredbythe differentflangeapplications.Alwaysremainingmindfulof protectingnotonlythe gramophone
surface but the redzone (nofinger),ourteamhas successfullyboxedupnumerousflangesincidentfree andon
schedule.

Duringthis period, Iwas engageddirectlybyNRGtowork as a Fitterfor theirscheduledUnit3 shutdown.Workingin
withthe units’ teamI wasgivenvariousoverhaul tasks.MypreviousKnowledge of flangemanagementandvalve
refurbishmentwasagreat asset.Alwaysopeningflangesinadefensive mannerallowedourteamtoensure we had
a safe and successful shut.Ialsorefurbishedvalvesandpumpsduringthisshut.Aspart of a team, we installedand
alignedthe InducedDraftFanmotors,withthe size andpositionof these motorscranesandotherwork groupswere
necessaryalwaysindirectcommunication withall teammembersonall workfrontsallowedsuccessful liftsand
installation.All taskswere onlyattemptedafteraclearunderstandingof all workmethodstatementsandsigningof
all permits.Ikepta personal JSBup to date and informativeformy fellow workmatessafetyandmine.Iwasalso
trainedonsite specificconfinedspace andworkingatheights.

Since LeavingBechtel Ipickedup a scheduled shutdownatCementAustralia.Here Iwas instrumental inthe removal
of a rotary feederandinstallationof a newfeeder,duringthisprocesswe had to addressdifferentobstacleslikethe
modificationof mountingbracketsandthe alignmentof new components.The feederwasalsoat heightso I worked
in withriggersand crane operatorsto ensure a smoothsafe process.Completingall JSA & HAZOBand interface with
othercrewdocuments.Iwasalsogiventhe tasktoensure differentworkpackswere completed,thisentailedthe task
of certainPMbeingfollowedduringthisprocessitwasevidentthatprevioustradesmanhadgone withthe attitudeof
tick and flickitwill be okay.I reportedall faultsthatwere evidenttosupervisorsandinconjunctionwiththemmade
decisions on what components and faults would need to be addressed during this shut and what could be reported
and completedinthe nearfuture.Iwasaskedtoworklongerhoursduring the course of the shutand replacedacrew
member on priority works. I was thanked and asked if I would be available for upcoming works.

I’ve beenworkingonthe APLNG Planton CurtisIslandas a Pipe fitter inaFIFO role workingonOutside Boundary
Limits(OSBL) andTrain 1. Duringthis time,Ihave workedona large varietyof pipe workrangingfrom50mm (2”) to
1200mm (47”) lightwall andheavywall thicknessand11”to 47” insulatedsch10 LNG transferpipe
installing/preppingforSurface TensionTransfer(STT) weldingIalsotookpart inorganisingandplanningoutworkfor
apprentice’sandtrade assistants.IcanefficientlyreadandunderstandpipingisometricsandP&IDdrawingsand
have no problemworkingwithotherdisciplineswhenworkisslow tomake constructionmore efficient. Keyrole
points, preppingpipeforSTTSettinguppurge dams for weldingstainless,Organisingworktouse welderseffectively,
ReadIsometricstosetout pipe workand pippingguides,Installationof pippingsupports,Springcansetc,Reading
P&IDs.

Here I workedinthe service departmentquotingtampingheadoverhauls,althoughmydutiesdidrequire me tobe

refineryHydrocrackerrefurbishment,refurbishmentof valves,FlangemanagementTensioningandtorqueing,
Commissioningof works,readingof isometricdrawings.Atthis time, Ialsogainedexperience withCMMS(computer
maintenance managementsystems). Alongwithall the safetypracticesthatare involvedwithhighriskworksIhave

I was employedbyTechnamillasa ProjectSupervisor/Mechanical Fittersourcrew consistedof 6 men.I wasgiven

neededtocomplete tasks.Iconducteddailyprestartmeetingsandweeklytoolboxes,all withoutanyincidentsor
delaysthe projectcame inearlyand underbudget.The Projectwasthe installationof arecyclingplant/waste

the project,the projectconsistedof co-ordinatingacrew of tradesman,equipmentoperators,riggersandtrade

I was alsoinvolvedinthe replacementof conveyorbeltrollers. SHUTDOWN WORK; RIOTINTO YARWIN NOVEMBER /DECEMBER Withmy outstandingworkethicandrepourfrompreviousshut-downworkUGL requestedmypresence aspartof the Demobilisationcrewforthe RioTintoYarwunshut-down.Once againIwas instrumentalinthe formulationof JSA’sandgiventhe responsibility,withasmall teamto ensure the qualityassurance of partsandfixturestoensure a successful shut.Asateam memberwe setuplaydownareasandworkfronts.Uponcommencementof the shutIwas involvedinnumerousFirstBreaksalwaysensuringisolation/de-isolationprocedures were followedandconfirmed.I was alsorequestedtofill inforthe leadinghandonhisfatigue day. CompletingnumerousTake 5’sbroughta great awarenessof hazardsand insuredsafe workpracticeswere inplace enablingsafe productiveshut. SHUTDOWN WORK; PHOSPHATEHILL, NOVEMBER Duringthistime,I wasrequestedbyUGL to partake ina shortshut at Phosphate Hill.Here dealingwithdangerous chemicalsSafetywasparamountinthe FirstBreakProcedure of numerousflangesandinspectionhatches.Thiswas alsoa FIFOposition,Iapproachedthispositionwiththe understandingthatIwas a UGL representative bothatthe worksite and incamp facilitiesalwaysconductingmyself inthe appropriate manner. SHUTDOWN WORK; NRG SEPTEMBER / NOVEMBER Duringthis time, I wasrequestedbyNRG(Gladstone powerstation) toreturnina more responsiblerole.Iwasgiven the responsibilityof workinginateamto dismantle,inspectandrefurbishorreplace numerousvalvesandthe alignmentof electricmotors.Workingina safe productive mannerIwasinstrumental indevelopingJSA andalways completingpersonalandgrouptake 5’s ensuredasuccessful safe shut. SHUTDOWNWORK; RIO TINTO YARWIN JULY / AUGUST Duringthis period, Ihave beenemployedbyUGL as a Fitterfortheir shutdownatRioTintoYarwun. I have been assessedandcertifiedonall relevantcompetencies.Workinginwithmycrew we have performedvarioustasks includingmanyafirstbreaksfollowingthe Rioprocedure.Alwaysmindfulof everchangingconditionsourtake 5 checklistare a valuable tool tocomplete anytask. Tensioningandtorqueingmanyflangesusingcorrecttooling as requiredbythe differentflangeapplications.Alwaysremainingmindfulof protectingnotonlythe gramophone surface but the redzone (nofinger),ourteamhas successfullyboxedupnumerousflangesincidentfree andon schedule. SHUTDOWNWORK; NRG MAY/ JULY 2015 Duringthis period, Iwas engageddirectlybyNRGtowork as a Fitterfor theirscheduledUnit3 shutdown.Workingin withthe units’ teamI wasgivenvariousoverhaul tasks.MypreviousKnowledge of flangemanagementandvalve refurbishmentwasagreat asset.Alwaysopeningflangesinadefensive mannerallowedourteamtoensure we had a safe and successful shut.Ialsorefurbishedvalvesandpumpsduringthisshut.Aspart of a team, we installedand alignedthe InducedDraftFanmotors,withthe size andpositionof these motorscranesandotherwork groupswere necessaryalwaysindirectcommunication withall teammembersonall workfrontsallowedsuccessful liftsand installation.All taskswere onlyattemptedafteraclearunderstandingof all workmethodstatementsandsigningof all permits.Ikepta personal JSBup to date and informativeformy fellow workmatessafetyandmine.Iwasalso trainedonsite specificconfinedspace andworkingatheights.

SHUTDOWN WORK; Cement Australia, APRIL 2015 Since LeavingBechtel Ipickedup a scheduled shutdownatCementAustralia.Here Iwas instrumental inthe removal of a rotary feederandinstallationof a newfeeder,duringthisprocesswe had to addressdifferentobstacleslikethe modificationof mountingbracketsandthe alignmentof new components.The feederwasalsoat heightso I worked in withriggersand crane operatorsto ensure a smoothsafe process.Completingall JSA & HAZOBand interface with othercrewdocuments.Iwasalsogiventhe tasktoensure differentworkpackswere completed,thisentailedthe task of certainPMbeingfollowedduringthisprocessitwasevidentthatprevioustradesmanhadgone withthe attitudeof tick and flickitwill be okay.I reportedall faultsthatwere evidenttosupervisorsandinconjunctionwiththemmade decisions on what components and faults would need to be addressed during this shut and what could be reported and completedinthe nearfuture.Iwasaskedtoworklongerhoursduring the course of the shutand replacedacrew member on priority works. I was thanked and asked if I would be available for upcoming works.
