First Reconstruction - Discussion Points
To what extent do you feel part of a team? Do you think about the bigger picture or
just your own role?
In the scenario we saw do you think the signallers and incident controller could have
resolved the situation more quickly?
What can be done to improve the speed and value of information for staff and
passengers?
Why do some people go the extra mile, like the driver who walked through his train to
reassure passengers, while others do the bare minimum?
How long would you swelter on a crowded train, until you forced the doors open and
walked away?
In this Olympic year, our railways will be in the spotlight. So, how can we make sure
visitors are impressed?

Second Reconstruction - Discussion Points
How should the COSS and the signaller have established a complete understanding
about the COSS’s exact location?
Do you think the signaller should have checked the information given by the COSS on a
map of the area to establish that the COSS was in the designated location, or was he
entitled to expect that the COSS would be in the right place?
Is there local jargon in use in your area that could be potentially confusing?
What do you think about the way in which the signaller and the COSS confirmed that
the train had passed? Should there have been better communication about exactly
which train was being discussed?
Would you know, from the mileage given, whether someone was where they thought
they were? Do you think you should know?

In the scenario we saw do you think the signallers and incident controller could have resolved the situation more quickly? What can be done to improve the speed and value of information for staff and passengers? Why do some people go the extra mile, like the driver who walked through his train to reassure passengers, while others do the bare minimum? How long would you swelter on a crowded train, until you forced the doors open and walked away? In this Olympic year, our railways will be in the spotlight.   • 5. Second Reconstruction - Discussion Points How should the COSS and the signaller have established a complete understanding about the COSS’s exact location? Do you think the signaller should have checked the information given by the COSS on a map of the area to establish that the COSS was in the designated location, or was he entitled to expect that the COSS would be in the right place? Is there local jargon in use in your area that could be potentially confusing? What do you think about the way in which the signaller and the COSS confirmed that the train had passed? Should there have been better communication about exactly which train was being discussed? Would you know, from the mileage given, whether someone was where they thought they were?
