Objectives
The purpose of this National Patient Safety Goal program is to inform nurses about The Joint
Commission’s current NPSGs that apply to hospitals. After studying the information presented
here, you will be able to:

Consider this patient scenario. Amberly was scheduled for an arthroscopy on her right knee. Her
orthopedic surgeon wrote instructions to his administrative assistant that he would be operating
on Amberly’s left knee. She called the OR scheduling office and gave them Amberly’s name and
the physician’s request to schedule her for a left knee arthroscopy. Amberly was put on the OR
schedule as a left knee arthroscopy. Upon admission to the hospital, Amberly was prepared for
surgery. When consenting for surgery, she stated that her right knee was to have the operation.
In the OR, the nurse checked Amberly in to the preop holding area and verified her name and date
of birth with her ID wristband. The nurse asked Amberly what surgery she was scheduled for and
found that the OR schedule and consent form, along with the patient, did not agree on the site of
the surgery. The OR nurse stopped all activity associated with her surgery until the discrepancy
was resolved.
This stoppage by the nurse is part of the universal protocol, one of the original (and continuing)
National Patient Safety Goals, or NPSGs, that contains three requirements: verification of correct
procedure, patient and site against all relevant documentation; marking of the surgical site by the
proceduralist; and conducting a timeout just before the start of the procedure, when the

physician, nurse and anesthesia provider agree that they have the right patient and are doing the
right procedure at the right site. For Amberly, there would have been two other stops along the
way that would have prevented this error: site marking and the timeout before surg.

Description of procedure: The patient was brought to the operating room and placed in the
supine position. General endotracheal anesthesia was administered. The patient's gown was
removed for prepping, at which point clinicians noticed there were small, acne like lesions over
the anterior surface of her abdomen, her intriginous areas, and on her legs. Several of
the lesions fell on the incision line.
Additionally, there was a large midline abdominal wall defect, which was assumed to represent
an abdominal wall hernia, and most likely will require a mesh repair. For these two reasons the
case was canceled after general anesthesia was administered. The patient was awakened from
anesthesia and taken to the recovery room.
There were no immediate complications evident, with the exception of cancellation of the case
after general anesthesia.

Objectives
The purpose of this National Patient Safety Goal program is to inform nurses about The Joint
Commission’s current NPSGs that apply to hospitals. After studying the information presented
here, you will be able to:
Name the current goals and elements of performance (requirements)
Explain the new goal effective January 1, 2014
Describe the intent of the patient safety goals
Consider this patient scenario. Amberly was scheduled for an arthroscopy on her right knee. Her
orthopedic surgeon wrote instructions to his administrative assistant that he would be operating
on Amberly’s left knee. She called the OR scheduling office and gave them Amberly’s name and
the physician’s request to schedule her for a left knee arthroscopy. Amberly was put on the OR
schedule as a left knee arthroscopy. Upon admission to the hospital, Amberly was prepared for
surgery. When consenting for surgery, she stated that her right knee was to have the operation.
In the OR, the nurse checked Amberly in to the preop holding area and verified her name and date
of birth with her ID wristband. The nurse asked Amberly what surgery she was scheduled for and
found that the OR schedule and consent form, along with the patient, did not agree on the site of
the surgery. The OR nurse stopped all activity associated with her surgery until the discrepancy
was resolved.
This stoppage by the nurse is part of the universal protocol, one of the original (and continuing)
National Patient Safety Goals, or NPSGs, that contains three requirements: verification of correct
procedure, patient and site against all relevant documentation; marking of the surgical site by the
proceduralist; and conducting a timeout just before the start of the procedure, when the

physician, nurse and anesthesia provider agree that they have the right patient and are doing the
right procedure at the right site. For Amberly, there would have been two other stops along the
way that would have prevented this error: site marking and the timeout before surg.

(4) Received $23,000 in cash that must be spent for a particular type of office machine within the
next year or the money must be returned.
(5) Paid salaries of $90,000. Of that amount, $15,000 came from restricted funds. The payment
was made to individuals doing health care work.
(6) Spent the $23,000 in (4) for the appropriate office machine.
(7) Received a cash gift of $12,000 that Watson must convey to another specified charity.
However, Watson has the right to give this money to a different organization if officials so
choose.
(8) Bought a building for $500,000 by signing a long-term note for $450,000 and using restricted
funds for the remainder.
(9) Collected annual membership dues of $30,000. Individuals receive substantial benefits from
their memberships. By the end of the year, two-thirds of the time for the average membership
has passed.
(10) Received unrestricted income of $40,000 generated by net assets that must be held
permanently.
(11) The board of directors of the Watson Foundation vote to set aside $9,000 of its investments
for emergency purposes.
(12) Paid rent of $12,000 for the past month, advertising of $15,000, and utilities of $16,000.
These were half for the program service and one-fourth each for the two supporting services.
(13) Received an unrestricted pledge of $200,000. Watson will collect the money in five years
and does not expect any part to be uncollectible. Present value at inception is $149,000, but
interest for the year to date is $6,000.
(14) Computed depreciation of $40,000, 60 percent for health care, 30 percent for administrative,
and 10 percent for fundraising.
(15) Paid $15,000 in interest on the note signed in (8). All of this cost is assumed to be rel.

Indications: A 30-year-old female with a long history of morbid obesity, recently underwent
silastic gastric banding, and due to reflux disease, subsequently required a procedure to loosen
the band. Most recently she has experienced significant reflux disease, and presents for removal
of her band and open Roux-en -Y gastric bypass.
Description of procedure: The patient was brought to the operating room and placed in the
supine position. General endotracheal anesthesia was administered. The patient's gown was
removed for prepping, at which point clinicians noticed there were small, acne like lesions over
the anterior surface of her abdomen, her intriginous areas, and on her legs. Several of
the lesions fell on the incision line.
Additionally, there was a large midline abdominal wall defect, which was assumed to represent
an abdominal wall hernia, and most likely will require a mesh repair. For these two reasons the
case was canceled after general anesthesia was administered. The patient was awakened from
anesthesia and taken to the recovery room.
There were no immediate complications evident, with the exception of cancellation of the case
after general anesthesia.

Each NON_COVID patient has an IllnessType attribute. - COVID_TEST - is a laboratory test given only to COVID patients. Each COVID_TEST has a TestID (identifier), and a Description. Your team also noted the following information about relationships among entities: An EMPLOYEE supervises many employees, or may not supervise at all. But an EMPLOYEE must be supervised by exactly one employee. A DOCTOR must treat as a minimum one PATIENT; while, a PATIENT is treated by one or more DOCTORS. A COVID patient must undergo one or more COVID_TESTs. It is mandatory that COVID_TESTs are performed on one or more COVID PATIENTS. The attributes Date, and Result are recorded whenever a COVID patient undergoes a COVID_TEST. At a time, a NURSE is assigned to one and only one CARE_UNIT, but a CARE_UNIT must be assigned with one or more NURSEs. Moreover, one NURSE must manage a CARE_UNIT and each CARE_UNIT must be managed by exactly one NURSE. > A CARE_UNIT has one or more ROOMS assigned to it. And, a ROOM is assigned to exactly one CARE_UNIT. A ROOM must have one or more BEDS. It is mandatory that a BED is assigned to a single (one) ROOM. A PATIENT is assigned to one BED if he/she needs to be admitted, if not, a patient is not assigned a bed. A BED may or may not be assigned to any PATIENT.
