The document discusses the importance of using spotters when operating heavy machinery to direct movement when the operator's view is obstructed. A spotter stands to the side of the vehicle and uses clear hand signals to communicate with the operator to ensure safe movement and avoid injuries or property damage from collisions that blind spots could cause. Proper communication between the spotter and operator is essential for safety.
HSE Documentation kit is a set of documents that contain integrated ISO 14001:2015 and OHSAS 18001:2007 documents requirements for health, safety and environmental management systems certification
The document discusses the important safety responsibilities of supervisors in the workplace. It notes that supervisors are uniquely positioned to directly oversee employee safety by controlling work conditions and enforcing safety policies on a daily basis. The workshop aims to educate supervisors on key aspects of their safety role, including complying with laws, providing resources and support for safety, conducting training, overseeing work, and enforcing rules. Through exercises and discussion, supervisors will gain insight into managing safety programs and being safety leaders.
ICOP For Safe Working in A Confined Space 2010 by Mr Husdin Che MatMoon Girl
The document provides guidelines for safe working in confined spaces according to the Industry Code of Practice for Safe Working in a Confined Space 2010 (ICOP 2010). It defines a confined space and outlines general requirements including hazard identification, permit systems, and duties of responsible persons like employers, permit issuers, entry supervisors, and standby persons. The document also describes requirements for confined space entry programs, risk assessments, control measures, atmospheric testing, rescue plans, training, and record keeping.
The document provides a summary of HSE performance indicators for various sites and offices for the month of February 2023. It includes details on LTIF, SVR, HR rates as well as incident summaries from different manufacturing plants and projects. It also discusses analysis of unsafe acts, hazards, and sub-contractor HSE performance trends. National Safety Week 2023 events from different locations are highlighted. Insights from the SMS group corporate HSE survey are also provided.
This document provides an overview of job hazard analysis (JHA). It explains that a JHA identifies hazards associated with each step of a job in order to develop solutions to remove or control hazards. The document outlines benefits of a JHA such as improved safety, efficiency, planning and selection of qualified workers. It provides guidance on performing a JHA, including questions to consider for each job step and recommended procedures after completing a JHA.

The document discusses improving health and safety in the Bangladeshi construction industry. It outlines several key issues: construction accidents are common in Bangladesh due to lax safety regulations and enforcement; existing health and safety laws are not well implemented; and improving safety practices could reduce accidents and benefit contractors through lower costs. The document then provides definitions of safety terms and describes various hazards, accidents, protective equipment, and permit systems that could be implemented on construction sites to enhance worker well-being.
This document provides information on signal person training, including:
- Learning objectives around understanding crane operation hazards, OSHA signal person requirements, and gaining skills to safely direct crane movements.
- Signal person qualifications require documentation showing the person understands standard signals, equipment limitations, and can pass a written or practical test.
- Standard hand signals must be used and understood by the operator, or alternative agreed upon signals can be used if standard signals are not feasible.
- Requirements for maintaining communication between the operator and signal person, and procedures for stopping operations if communication is interrupted.
The close out report summarizes HSE performance for a sub sea manifold project from June to October 2013 at PT Gunanusa Utama Fabricators yard. Key highlights include:
- The project achieved its safety objectives of no fatalities, major accidents, fires or explosions. TRIR was 0.00 and LTIF was 0, meeting objectives.
- Medical treatment cases were 6 with no lost time injuries. Near miss incidents were 2.
- HSE programs like inductions, inspections, audits and management oversight were implemented to improve safety behavior.
- Occupational health objectives around emergency equipment, illness reduction and hygiene were also met.
