During these intake assessments it is my job to record and take vital signs, educate families and patients on the disease, prognosis, treatments, and hospice care, and to ensure that families and patients understand and agree with the plan of care. On routine visits it is my responsibility to again assess patients, record vital signs, as well as monitor any changes from previous visits and report changes to our providers on staff.
Q: Does a correlation exist between your job description and your practice?
A: Although she has a very clear job description laid out for her, she explains that her duties go far beyond the typical hands on patient care the typical nurse sees in the office or hospital setting. She explains hospice is a special kind of job where you not only care for the patient but patient’s families as well. She explains sometimes the care for the patient includes caring for them family and helping them cope with the prognosis of death.
Q: Are you involved in policy formulation that affects client services?
A: Jennifer describes her work environment and boss as exceptional. All staff are seen as equals and their opinions valued to the fullest. Each day they have a meeting where all staff who reports to the Hospice House that day go through reports as well as ideas to make their jobs successful. She states having supportive staff members makes it easy to help policy implementation and change.
Q: Do you belong to a professional organization related to your current job position?
A: Currently Jennifer explains she is not involved in an organization related to hospice care. However there is an organization that several of her fellow employees are members of which is the National Hospice and Palliative Care Organization (NHPCO). When I think of nursing theory the first person that comes to mind is Florence Nightingale. She is well known for her environmental adaptation theory. In her theory she discusses ventilation and warming, noise, variety, diet, light, cleanliness, and chattering hopes and advices. I feel that her theory influences my values and goals. Her theory is also used every day in the hospital facility I work in (Cherry and Jacob, 2017).
Nightingales theory has influenced my nursing values by ensuring that I provide a safe environment for my patients. Patient rooms should be clean, well lit, and free of clutter to implement non-maleficence. Her theory has influenced my values such as altruism and dignity. I provide individualized care to my patients while respecting their morals. Every person is different and their care needs to be catered to their needs. As a nurse I am always concerned with the patient’s altruism. Safety is key and the driver in everything I do as a nurse. If safety is not up held then everything else fails.
Nurses apply Florence Nightingales theories in their practice to promote excellent nursing practices in many ways. There are restricted visitor’s hours to promote rest and reduce noise. Patients are put on specific diets depending on their health history to promote healing and further harm. For example, Nurses monitor the amount of fluids that patients take in that are admitted for Congestive Heart Failure to ensure they do not further complicate their disease process. In the morning nurses raise the blinds to let in the natural light so patients are aware that is a day time and time to be up and not sleeping. This helps to combat delirium in patients and helps them get the proper rest. Nurses also ensure the rooms are clean, linens are changed daily, patients are bathed daily and as needed, and housekeeping tends to the patient rooms every day. Not only does cleanliness apply to the patients but it also applies to the nurses. In current practice nurses must wash their hands with antimicrobial soap before entering and after leaving a room. To combat boredom of patients and have a variety of activities nurses can offer art therapy, music therapy, or pet therapy. There are many other things that nurses can do for patients in their nursing practice that are influenced by Florence Nightingales nursing theory.
Currently in my professional practice I also do many of the mentioned things above that align with Florence Nightingales nursing theory. When I bathe my patients I implement the use of CHG wipes that not only clean the patient but also provide cleanliness to help prevent infections. I also wash my hands prior to entering a room, assessing a patient, and upon exiting a room. While in a patient room I try to limit noise and distractions to help promote rest and healing. At the facility I work at we do not have strict visiting hours but we do have quiet time where we dim the lights and try to limit visitors to.

In the past three years, Indiana University Health (IUH) University Hospital Palliative Care Services has expanded its size and scope. Our mission remains to treat the suffering of patients with chronic, progressive illnesses, their families, and their providers through symptom optimization and the search for meaning. While continuing to work with patients near the end-of-life and transitioning to hospice we have increasingly been involved with complex patients whose deaths are not imminent or even expected.
Our Team
The team has transitioned from a part-time to a full-time physician, a new full-time nurse practitioner, a new position in a nurse clinical coordinator, increased time for out part-time social worker, and continued part-time chaplain.
New Opportunities for Palliative Care
IUH University Hospital sees some of the sickest of the sick including advanced liver failure, advanced pulmonary disease, and transplant patients. Our service has become involved in alleviating suffering in pancreatic, liver, renal, and multivisceral transplant patients. Our expertise in opiates has placed us in a unique position to assist with patients having pain due to opioid-hyperalgesia and narcotic bowel syndrome. In addition we have started seeing more hepatology, oncology, hematology, and pulmonary patients earlier in their disease.
Out-Patient Services
We have expanded our service to now include out-patient, currently by embedding within other clinics at University Hospital including seeing patient in the multidisciplinary oncology clinic, hematology, digestive and liver disease clinic, and surgical out-patient clinic. In the near future we hope to have dedicated clinic space within the geriatrics clinic.
