My trip to Chicago in June was for business but I arrived early to sightsee. I stayed at the historic Palmer House Hotel, built in 1875, which had great room service. For work, I helped at a booth at a convention and attended industry parties. The trip combined business with enjoying Chicago's architecture, food, and views from skyscrapers.

Emphysema is an abnormal permanent enlargement of the air spaces distal to terminal bronchioles, accompanied by destruction of their walls and without obvious fibrosis.
Emphysema of lung is defined as hyper inflation of the lung ais spaces due to obstruction of non respiratory bronchioles as due to loss of elasticity of alveoli.

- 4. 1. Comprehensive Care Meeting the large majority of each patient’s physical and mental health care needs, including prevention and wellness, acute care, and chronic care. 5. Quality and Safety A commitment to safe, high-quality care through engagement in quality improvement activities and safety monitoring. 2. Patient-Centered Provides primary health care that is relationship-based with an orientation toward the patient and families unique needs. 3. Coordinated Care Coordinates care across all elements of the broader health care system. 4. Accessible Services Delivers accessible services with shorter waiting times for urgent needs, enhanced in-person hours, around-the-clock telephone or electronic access to a member of the care team.
- 5. Applicable only for participating PCMH Insurance companies- Check with your local provider rep for participatiion.
- 7. AFTER HOURS ACCESS The PCHM is expected to offer access to care outside of regular business hours to families to help avoid requiring them to take time from work and school to address their health care needs. The practice may arrange for patients to be seen by other (non-ER) facilities/Urgent Care centers. This information must be clearly conveyed to patients.
- 8. ELECTRONIC ACCESS/COORDINATION OF CARE The PCHM is expected to provide more than 50% of patients who request an electronic copy of their health information within three business days.
- 9. NON-COMPETE All patients are instructed to follow-up with their PCP.
