Delivers accessible services with shorter waiting times for
urgent needs, enhanced in-person hours, around-the-clock
telephone or electronic access to a member of the care team.

AFTER HOURS ACCESS
The PCHM is expected to offer access to
care outside of regular business hours to
families to help avoid requiring them to
take time from work and school to address
their health care needs.
The practice may arrange for patients to be
seen by other (non-ER) facilities/Urgent
Care centers. This information must be
clearly conveyed to patients.
ELECTRONIC ACCESS/COORDINATION OF CARE
The PCHM is expected to provide
more than 50% of patients who
request an electronic copy of their
health information within three
business days.
NON-COMPETE
All patients are instructed to
follow-up with their PCP.
