This document summarizes the Canadian Association of Paediatric Health Centres' (CAPHC) Temperature Check meeting on October 25, 2016. Consensus for a single patient safety advocacy priority area that
CAPHC is well-positioned to lead
 An overarching Patient Safety Culture throughout the continuum of child and
youth healthcare - Supporting an inter-professional collaborative approach
 Integrating Family-centred care
 Sharing of best practices between front-line teams and leadership to
dismantle hierarchical barriers
 Spanning all levels of care, urban, rural and remote services; patient/client
handover between facilities including transition from paediatric to adult
services; and
 Establishing safe disclosure practices to promote patient safety quality
improvement initiatives and optimize patient outcomes.

8.  Working in partnership with the Canadian Patient Safety Institute (CPSI) -
Bringing the voice of children and youth to CPSI’s National Patient Safety

will now become a part of CAPHC Knowledge Translation Platform, bringing a
greater focus on patient safety to a greater audience through CAPHC’s
national webinar program “CAPHC Presents”
 CAPHC’s October 23, 2016 - National Patient Safety Symposium: “Solutions
Without Borders: Bringing Solutions for Patient Safety to Canada”
