This observation form is used to record notes from classroom observations of teachers as part of the Classroom Observation Tool–RPMS evaluation system. The form collects information such as the observer, teacher, subject, and time of the observation. It was developed by the Philippine National Research Center for Teacher Quality with support from the Australian Government.
