disorders or developmental disorders that are present at birth. A congenital anomaly can be
caused by a single-gene disorder, which is a mutation in one gene in the ova or sperm that is
passed down to later generations. Mutations in body cells that are not reproductive cells can
cause a disorder or dysfunction but cannot be passed down the way mutations in reproductive

Chromosomal defects can also be the cause congenital anomalies. During meiosis, DNA
fragments can be displaced or lost. This kind of error is what usually causes chromosomal
anomalies and is more common when the mother is older than age 35. Some congenital
disorders happen at birth, but do not have a genetic component. These can occur from premature
birth, exposure to teratogenic agents, or a traumatic labor or delivery. Teratogenic agents are
those that can damage the embryo or fetus and its development. Some congenital anomalies are
caused by multiple genes, making them polygenic disorders (Vanmeter, 2014).
The article I chose to critique is entitled “Dietary glycemic index and glycemic load
during pregnancy and offspring risk of congenital heart defects: a prospective cohort study.” It
was authored by Amalie Schmidt, Marie Lund, Giulia Corn, Thorhallur Halldorsson, Nina Oyen,
Jan Wohfahrt, Sjurdur Olsen, and Mads Melbye, all of whom are affiliated with reputable
institutions, such as the University of Bergen Department of Global Public Health, Harvard TH
Chan School of Public Health, and Stanford University School of Medicine. The article was
published this year, 2020, in The American Journal of Clinical Nutrition. The purpose of this

article was research- to examine the relationship between mid-pregnancy dietary glycemic index,
glycemic load, and the risk of congenital heart defects in the baby. The article does not include a
formal literature review, but the introduction section provides information already published
about the topic from previous studies, mostly in the discipline of medicine. The journals cited
from are mostly medical journals on the topics of pediatrics, epidemiology, and diabetes. The
authors identify a research gap by stating that only one other study exists that assesses the risks
between glycemic index and heart defects. The first aim of the study was to investigate the
association between glycemic index and glycemic load during pregnancy and offspring risk of
congenital heart defects using a food-fre.

Clinical Problem
A report from the Center for Disease Control and Prevention in 2015 revealed that (9.4%) 30.3 million Americans are diabetic and 84.1 million have prediabetes. This is a total population of over 100 million is at risk of developing type 2 diabetes which is a growing health problem being the seventh leading cause of death in the U.S. An estimated 1.5 million new cases were among 18-year old bracket and the rates of diagnosed diabetes increased proportionally to age. Below 44 years accounted for 4%, below 64 years at 17 % and 25% for those above 65 years across both genders. One-third of adults in America has prediabetes but sadly, they are unaware despite reports released by The National Diabetes Statistics Report every year. These reports elaborate on prevalence and incidence, prediabetes, long-term complications, risk factors, mortality, and cost. Diabetes poses the risk of serious complications like death, blindness, stroke, kidney disorders, cardiac diseases and health problems that lead to amputation of legs. However, the risks can be mitigated through physical body activities, proper dieting and prescribed use of insulin and other related measures to control the blood sugar levels. Diabetes Prevention Program was funded by NIH to research a yearly evidence-based program to improve healthy weight loss through diet and physical activities. There also efforts to determine the effectiveness of public service campaigns in improving the real-life experience in the diagnosis and treatment of diabetes.
PICOT Question.
The population affected by diabetes cuts across all ages, gender, race, and ethnicity. The prevalence is significantly high from 18 years and it increases with age to about 25% above 65 years. In terms of gender, men are at higher risk accounting for 37% while women are at 30% across races and educational levels. On races, the rates were higher among Indians/Alaska natives at 15%, non-Hispanic blacks at 12.7% and Hispanics at 12%. Among Asians, the rates were lower at 8% and 7.4% for non-Hispanic whites.
Intervention indicator for diabetes shows that individuals who do not observe a healthy diet are more exposed to the disease. Some risk behaviors include lack of exercise and excessive intake of junk foods that lead to obesity and increased blood sugar levels. Diabetes prevalence varied according to education levels were those with less than high school education at 12.6% and 7.2% for those higher than high school education.
Comparison and use of a control group from the popularity of Complementary and Alternative Medicine and Traditional Chinese Medicine showed distinct knowledge of diabetes, blood sugar control, and self-care. The experimental group received education through interactive multimedia for three months while the control group received.
1) Childhood obesity has more than doubled in the past few decades and poses significant health risks.
2) Early identification of excessive weight gain is important through tracking BMI percentiles over time.
3) Pediatricians should discuss healthy eating and physical activity with families during routine visits to promote prevention and early recognition of obesity issues.

This study assessed cardiovascular risk factors and knowledge gained among 28 ninth grade participants in the Jackson Heart Study Science, Language Arts and Mathematics program. Baseline assessments found the majority were overweight/obese (61%) and pre-hypertensive/hypertensive (40%). Participants completed pre- and post-surveys on heart health knowledge. Scores improved from 52% correct at baseline to 70% post-education, demonstrating increased knowledge. However, post-scores were still below the desired 75% level. While education appeared to increase knowledge, longitudinal evaluation is still needed to determine effective methods for addressing cardiovascular risk in this vulnerable population.

However, according to (Healthy People 2020), most Americans don't check their menu and are lazy in taking apart in physical activities to the required levels as a way of maintaining proper health.
According to CDCP (2013), fruit consumption among adults is 1.1 times in a day, and the use of vegetables on a daily scale is 1.6, with adolescents recording the lowest use of both fruits and vegetables. The statistic shows that the average daily consumption of both fruits and vegetables among Americans doesn't meet the recommended intake of fruits and vegetables. About 81.6% and 81.8% of American adults and adolescents respectively don't take part in physical activities on a recommended daily scale. These behaviors are among the leading factors that contribute to the rising cases of Obesity. Approximately 1 out of 3 US adults, which represents 34 % and 1 out of 6 adolescents and children, which is 16.2 %, are obese.
Obesity-related complications include stroke, heart disease, and type two diabetes. The current in death cases is as a result of the obesity-related complications. Besides the death cases, obesity-related diseases cots this country millions of money annually, making it one of the most significant burdens that this country is struggling with regards to the health care system.

Estas diretrizes visam melhorar a qualidade dos cuidados pós-natais essenciais e de rotina prestados às mulheres e aos recém-nascidos, com o objetivo final de melhorar a saúde e o bem-estar materno e neonatal.

Overall life span (LS) was 1671.7±1721.6 days and cumulative 5YS reached 62.4%, 10 years – 50.4%, 20 years – 44.6%. 94 LCP lived more than 5 years without cancer (LS=2958.6±1723.6 days), 22 – more than 10 years (LS=5571±1841.8 days). 67 LCP died because of LC (LS=471.9±344 days). AT significantly improved 5YS (68% vs. 53.7%) (P=0.028 by log-rank test). Cox modeling displayed that 5YS of LCP significantly depended on: N0-N12, T3-4, blood cell circuit, cell ratio factors (ratio between cancer cells-CC and blood cells subpopulations), LC cell dynamics, recalcification time, heparin tolerance, prothrombin index, protein, AT, procedure type (P=0.000-0.031). Neural networks, genetic algorithm selection and bootstrap simulation revealed relationships between 5YS and N0-12 (rank=1), thrombocytes/CC (rank=2), segmented neutrophils/CC (3), eosinophils/CC (4), erythrocytes/CC (5), healthy cells/CC (6), lymphocytes/CC (7), stick neutrophils/CC (8), leucocytes/CC (9), monocytes/CC (10). 1. A Link Between Obesity and MS?
Health risks associated with obesity have been known for years, particularly theincreased risks of
developing heart disease and cancer along with other long-term diseases. New research now shows that
obesity can also increase your risk of developingneurological diseases such as Multiple Sclerosis.
Multiple Sclerosis (MS) is a neurological disease in which the protective coating around nerve fibers
breaks down slowing signals and raveling between the brain and body. It affects 400,000 Americans
every year.
While this research, presented in the journal Neurology, does not prove that carrying extra weight when
you’re younger will cause MS, the findings suggest that rising levels of obesity in children could lead to
an increase in MS diagnoses when compared to the number of cases reportedin the past, according to
Annette Langer-Gould from Kaiser Permanete of Southern California.
Langer-Gould and her colleagues compared heights and weights of 75 young people with pediatric MS
and clinically isolated syndrome (CIS), which is a possible MS precursor, and over 900,000 children
without either disease.
“Our findings suggest that childhood obesity epidemic is likely to lead to increased morbidity from
MS/CIS, particularly in adolescent girls,” Langer-Gould and her colleagues wrote.
A little over half of the young people with MS were overweight or obese compared to 37 percent of the
other children.
Kassandra Munger, who studies MS at the Harvard School of Public Health in Boston (not involved in this
study), suggests the possibility of deficient vitamin levels and a greater amount of chemicals secreted by
fat cells as to why overweight children and adolescents could have an increased risk of MS.
Langer-Gould and her colleagues are continuing to track the children in their study and they are also
working on researching whether adulthood weight affects their chance of developing MS.
