This document discusses dual bronchodilation with tiotropium/olodaterol for the treatment of COPD. It begins with an overview of the prevalence and magnitude of COPD globally and in Spain. It then discusses the 2017 Spanish COPD treatment guidelines and algorithms. It aims to dispel myths around the use of inhaled corticosteroids (ICS) for COPD, looking at risks of pneumonia, exacerbations, withdrawal effects, and mitigating factors. Tiotropium is identified as the most studied bronchodilator. Results are presented showing that combining tiotropium with olodaterol provides superior bronchodilation to either agent alone. The document concludes by discussing the Respimat inhal
This document provides a summary of topics in pulmonology including smoking, chronic cough, COPD, asthma, bronchiectasis, pneumonia, tuberculosis, lung cancer, pulmonary fibrosis, and sleep apnea. It discusses the health effects of smoking and reviews several recent research studies on the links between smoking and mortality from various diseases. It also examines the impact of smoking on gender differences in life expectancy and the relationship between smoking and survival after a breast cancer diagnosis.
This study evaluated the coexistence of asthma and allergic rhinitis in adult patients attending allergy clinics. Over a 10-year follow-up period, 1,271 college freshmen were prospectively evaluated for medical conditions including asthma and allergic rhinitis. The results showed that the frequency of asthma and allergic rhinitis continued to increase as the individuals became older. Allergic rhinitis and positive allergy skin tests were significant risk factors for developing new asthma. Individuals with either of these diagnoses were about three times more likely to develop asthma over the 10-year period than those with negative skin tests.
This document contains a summary of topics in pulmonology, including asthma, COPD, bronchiectasis, lung cancer, smoking, cough, pneumonia, tuberculosis, sleep apnea, idiopathic pulmonary fibrosis, and new diseases. It discusses recent studies on these topics published between 2014 and 2015. The document is presented as part of an update in pulmonology given on May 9, 2015 in Barcelona.
This literature review covers three topics: pneumonia, lung cancer, and pulmonary fibrosis. For pneumonia, it summarizes that the declines in childhood pneumonia hospitalizations observed after the introduction of PCV7 vaccines were sustained over a decade. It also found substantial reductions in adult pneumonia hospitalizations. For lung cancer, it discusses findings from several large randomized controlled trials that low-dose CT screening detects more early-stage lung cancers compared to chest radiography and tends to result in a stage shift toward earlier diagnoses. For pulmonary fibrosis, it reviews that the natural history can vary significantly, from complete resolution to progressive fibrosis leading to respiratory failure.
This document provides an overview of COPD (chronic obstructive pulmonary disease) with perspectives on future directions and a look at the past. It discusses topics like demographics, the negative outlook for smoking, noncommunicable diseases, clinical phenotypes, new technologies, and conclusions. The summaries highlight that efforts must continue against smoking and early diagnosis. Future consensus is needed on COPD definitions. Regenerative medicine offers hope but its realities are unknown. The aging phenotype will dominate COPD's future, and obesity comorbidity warrants clinical interest.
