Alisha Sarr, a 4-year-old girl of Angolan origin, presented with headaches, fatigue, joint pain, and swelling of the hands and feet. Her physical exam showed jaundice, swelling of the left hand, and a weak response. Initial testing showed elevated white blood cells and decreased red blood cells, hemoglobin, and hematocrit. Further sickle cell blood testing was ordered to analyze her hemoglobin fractions due to these abnormal blood results. A blood sample was taken and labeled for this sickle cell analysis.

Vitals: BP- 100/70 Resting heart rate – 120 Weight- 28lbs Height 36inches
Physical Exam – Taken by GP: Patient presented pale with visible swelling of the left hand, cold
to the touch. Lungs open and clear, no chest pain. Abdominal abnormalities included swelling of
upper left region and limited digestive sounds.
Testing – By GP: Urine – raised leukocytes
Testing – Nurse: Complete Blood Count (CBC), Fasting Blood Glucose (BG)
Results: ˄WBC, ˅RBC inc Hemoglobin (Hg) and Hematocrit (HCT)
Further Testing: Sickle Cell Bloods for Hemoglobin fractionation by HPLC
Hemoglobin fractionation by HPLC: Venus blood taken, deposited and labeled into EDTA Whole
blood (purple) tube.
