Ethanol (CH3CH2OH), or beverage alcohol, is a two-carbon alcohol
that is rapidly distributed in the body and brain. Ethanol alters many
neurochemical systems and has rewarding and addictive properties. It
is the oldest recreational drug and likely contributes to more morbidity,
mortality, and public health costs than all illicit drugs combined. The
5th edition of the Diagnostic and Statistical Manual of Mental Disorders
(DSM-5) integrates alcohol abuse and alcohol dependence into a single
disorder called alcohol use disorder (AUD), with mild, moderate,
and severe subclassifications (American Psychiatric Association, 2013).
In the DSM-5, all types of substance abuse and dependence have been
combined into a single substance use disorder (SUD) on a continuum
from mild to severe. A diagnosis of AUD requires that at least two of
the 11 DSM-5 behaviors be present within a 12-month period (mild
AUD: 2–3 criteria; moderate AUD: 4–5 criteria; severe AUD: 6–11 criteria).
The four main behavioral effects of AUD are impaired control over
drinking, negative social consequences, risky use, and altered physiological
effects (tolerance, withdrawal). This chapter presents an overview
of the prevalence and harmful consequences of AUD in the U.S.,
the systemic nature of the disease, neurocircuitry and stages of AUD,
comorbidities, fetal alcohol spectrum disorders, genetic risk factors, and
pharmacotherapies for AUD.

RESULTS: Overall life span (LS) was 2252.1±1742.5 days and cumulative 5-year survival (5YS) reached 73.2%, 10 years – 64.8%, 20 years – 42.5%. 513 LCP lived more than 5 years (LS=3124.6±1525.6 days), 148 LCP – more than 10 years (LS=5054.4±1504.1 days).199 LCP died because of LC (LS=562.7±374.5 days). 5YS of LCP after bi/lobectomies was significantly superior in comparison with LCP after pneumonectomies (78.1% vs.63.7%, P=0.00001 by log-rank test). AT significantly improved 5YS (66.3% vs. 34.8%) (P=0.00000 by log-rank test) only for LCP with N1-2. Cox modeling displayed that 5YS of LCP significantly depended on: phase transition (PT) early-invasive LC in terms of synergetics, PT N0—N12, cell ratio factors (ratio between cancer cells- CC and blood cells subpopulations), G1-3, histology, glucose, AT, blood cell circuit, prothrombin index, heparin tolerance, recalcification time (P=0.000-0.038). Neural networks, genetic algorithm selection and bootstrap simulation revealed relationships between 5YS and PT early-invasive LC (rank=1), PT N0—N12 (rank=2), thrombocytes/CC (3), erythrocytes/CC (4), eosinophils/CC (5), healthy cells/CC (6), lymphocytes/CC (7), segmented neutrophils/CC (8), stick neutrophils/CC (9), monocytes/CC (10); leucocytes/CC (11). Correct prediction of 5YS was 100% by neural networks computing (area under ROC curve=1.0; error=0.0).
CONCLUSIONS: 5YS of LCP after radical procedures significantly depended on: 1) PT early-invasive cancer; 2) PT N0--N12; 3) cell ratio factors; 4) blood cell circuit; 5) biochemical factors; 6) hemostasis system; 7) AT; 8) LC characteristics; 9) LC cell dynamics; 10) surgery type: lobectomy/pneumonectomy; 11) anthropometric data. Optimal diagnosis and treatment strategies for LC are: 1) screening and early detection of LC; 2) availability of experienced thoracic surgeons because of complexity of radical procedures; 3) aggressive en block surgery and adequate lymph node dissection for completeness; 4) precise prediction; 5) adjuvant chemoimmunoradiotherapy for LCP with unfavorable prognosis.

2. I want to see such a headline in 5 years’ time. Not in 20 years
or 50 years. I have been following research into this topic for
over 40 years. I have written a book Project Nirvana on the
subject. I have known some of the leading scientists in the field. I
mourn the decades lost after Nixon declared his “war on drugs”
to ward off his fear of losing a whole generation to the anti-
Vietnam war movement.

Decades lost: Now, billions of dollars and untold human
losses later (to imprisonment, violence, addiction, trauma and
damage to whole communities), the research is still being
hampered by government controls, resistance from the Big
Pharma and the medical research establishment. A researcher
may still waste months to get his/her research approval, and
tens of thousands of dollars to get medicines that were freely
available in the 50’s and 60’s at virtually no cost.

In Australia, getting approval is even more difficult
than in most other countries. Instead of building on
foreign experience, Australia seems to be playing
catch up while losing annually thousands of people to
prescription and illicit drug abuse, tobacco and
alcohol.

It’s the economy, stupid. We need to be bolder. We do
not need to prove again and again that psilocybin is safer
and more effective for treatment of anxiety and PTSD
than any known prescription drug, or that MDMA is
useful for family therapy. What we do need to show is
that psychedelic medicines can improve general well-
being and health, enhance productivity and enjoyment of
life, and replace damaging, expensive and ineffective
drugs. It seems that what may finally sway our
governments will be economics. We simply will not be
able to afford to lock up our young in prisons for mere
possession of marijuana. And we will need our old folks
to work productively well into their pension years.
6. New paradigm: save money, reach more people. We need
to be both bold AND pragmatic, cheap and effective. In
addition to testing new medicines on some morbidly sick or
dying people, or, at the other extreme, on “advanced”
meditators to see the effects that a particular plant extract will
have on them, we need to test healthy people with PCM to
see the effect on their general health, well-being and
productivity. Earlier studies proved the existence of “the
relaxation response” (thus making meditation respectable),
and confirmed morphological changes which meditation and
relaxation produced in patients’ cardiovascular systems.

Right now we can use simple and inexpensive
tools to show what kind of difference PCM can
make. For example, we can use heart rate
variability (HRV), which is being increasingly
used by our elite athletes, to monitor the
improvement of a subject’s general health and
fitness. HRV is perhaps the most sensitive measure
of fitness as manifested by the subtle balance
between alternating periods of rest and activity in
the autonomous nervous system, often damaged
by stress. There are a number of simple and
inexpensive gadgets available to anyone right now
to monitor HRV in real time.
8. Simple test for brain integration: At the other end of the scale, we
could use tests developed by the late Dr Herbert Spiegel, a Columbia
University psychiatrist, who introduced the term “neural trance[1]”.
These tests (known as HIP or “hypnotic induction profile”) require
no expensive equipment. They measure the level of intactness of the
CNS and the coherence and “flow” of neural traffic between different
brain areas, indicating the degree of stress-free functioning. In the
same way in which HRV measures physical health, HIP could be
used as a powerful indicator of psychological health.

Later on: When such inexpensive and simple tests prove the
desirable health effects of PCM for the general population,
we can apply more advanced medical technologies, such as
non-invasive deep brain imaging and stimulation, fMRI, etc.,
to further substantiate the results.
Researchers self-experiment: Such research will no doubt
bring improvements in protocols and tools. We can gradually
reduce doses of psychedelics; we can combine such reduced
doses with new memory and brain enhancement substances
until the psychedelic catalyst can be withdrawn completely
or spaced as widely as desired. We can provide set-and-
settings improvements that will only come when researchers
themselves are able to use their own experimentation with
PCM to increase their perception of age-old truths and to
interrogate traditional/shamanistic guides more fruitfully.

With humanity increasingly confronting its most testing
challenges, we will be able to relate such research more
closely to advances in genetics, biology, and evolutionary
psychology. We can then define and measure the
individual’s Transcendence Quotient or TQ – the ability
to experience altered states of consciousness and integrate
them, its genetic and personality basis, and adjust our
regimens accordingly. It may be too early to dream of
integrating bionics and genetic enhancements into this
quest but such may be our horizons sooner than we think.

progress. In Australia, our underfunded and
overstretched traditional medicine is losing its
ground to the Four Horsemen of the modern
epidemiological Apocalypse: Stress, Addiction,
Obesity, and Depression. Medicine is

flattened by the horses’ hooves. With our ageing
population, the burden on the health system will
soon become simply unbearable.

Summary: Two markers of a person’s general health
are paramount: the autonomic balance (i.e. the smooth
transition between rest and activity as measured by
HRV and breath frequency) and the intactness and
integrity of the CNS (as measured by HIP). They
indicate the effectiveness of the organism’s interaction
with the environment.
Meditation practice, similarly, rests on three pillars: a
profound relaxation response, an ability to maintain
steady flow of awareness, and spiritual insight. The
effect that the mass adoption of research-based PCM
techniques can have on the general population and the
sick will be truly profound.

PCM techniques will prosper and advance more
than others.
