• A vessel was sailing in US waters
around 2013
• It was carrying cargo
• Cylinder Liner of main engine was
cracking. It finally failed. The vessel
manoeuvrability was effected.

• But this was not arranged.

15. FINDINGS
 Bunker (Fuel oil) was substandard
 Polluted fuel in turn damaged the liner.
 Scavenge Inspection was arranged. But result
not ascertained.
 The problem could have been detected if
Piston Topland check result was available.

16. CONSEQUENCES
HUGE PENALTY WAS
IMPOSED

 The ship had to be sold.
 8 to 9 other ships had also to be
sold by the owner.
 Office premises had to be
vacated and sold.
 Consequently the company went
into liquidation.

18. LESSONS LEARNT
 If linear was weak and leaking, ship
should not have sailed.
 It should have been first repaired.
 If Bunker was found to be of poor
quality, 2nd Analyses Report should
have been arranged.

Proper planning of inspections,
check and maintenance of the
ship’s engine room must cater for
correct provisioning and use of
stores, spares and tools.
