Myocardial revascularization is a cardiovascular surgery used to resolve coronary pathologies by bypassing blocked coronary arteries. Grafts such as the left mammary artery and saphenous vein are used to reroute blood flow around obstructions in the coronary arteries to areas of the heart experiencing ischemia. The ideal candidates for this procedure are patients with obstruction of the left main coronary artery trunk, proximal obstruction of two or three major coronary arteries, or proximal obstruction of the anterior descending artery where angioplasty is not possible, especially if they also have diabetes or heart failure.

Myocardial revascularization parrafo critico
1. MYOCARDIAL REVASCULARIZATION
Myocardial revascularization is a cardiovascular specialty surgery used to resolve coronary
pathologies, where there are twomaintypesof action, whichare; withExtra Corporeal Circulation
(CEC) and without Extra Corporeal Circulation (CEC).
Grafts are used to perform the necessary anastomoses during the procedure where the coronary
arteries are obstructed to bring blood to the areas of the heart where there is a flow deficit
(ischemia)
Differenttypesof graftsare usedwhichare a) the leftmammary arteryto the anteriordescending
artery and b) saphenous vein to the right coronary artery.
But the right mammary artery and the radial artery can also be used as grafts, and very rarely the
gastroepiploic artery, and the inferior epigastric artery.
Indications for the ideal patient for this surgical procedure. They are those with pathological
characteristics such as
• Obstruction of the left coronary artery trunk.
• Proximal obstructionof twoorthree maincoronaryarteries (especiallyif the anteriordescending
artery is involved).
• Proximal obstruction of the anterior descending artery in which coronary angioplasty is
impossible.
Patients with these obstructions, and who also suffer from diabetes or failure of the heart to
contract, are often suitable candidates for this type of surgery.
A surgery that, in my case, I observed without CPB, and, therefore, several jets of blood were
presented during the process, but that ultimately generates a lot of intrigue because of how it is
going to be developed and how the surgical team is so well complemented by all, generating an
environment of trust and constant support among them.
