This is a building assignment that you will be completing over Week 1 through Week 9. This is a stepwise project proposal assignment in which you will complete one (1) section each week for your MSN Capstone Project Proposal. By the end of the Week 9, this form will have been filled in completely and your MSN Capstone Project Proposal will be completed. It is important that you keep up with the Weekly Assigned Section. Each
section is graded separately on a weekly basis while the final completed form will be graded with an overall grade. Each week the student will receive feedback from the instructor and the student is expected to incorporate the instructor feedback to edit and improve the weekly sections. My name is Raul Rodriguez, I am a Cuban immigrant who came to the states when I was eleven (11) years old in 1995. I grew up in the midst of Miami and graduated from Felix Varela Senior High; to further my education, I completed my AS Degree here at FNU and have moved myself forward throughout the years. I am currently a STEMI nurse for Mount Sinai Medical Center where I’ve been for almost two years, but have almost 4 years of ICU experience working for the Bellevue Hospital in New York City.
I’d like to continue pursuing my goal of becoming an ARNP to take care of those that need us most. I would like to focus on Cardiology since that is the specialty I love the most and where most of my experience is in, feel that with my given background/knowledge I can provide the best practice care possible.
As a nurse practitioner and provider various interrelated factors have contributed to changes in how nurses deliver care today. The unobtainability of adequate health care services in rural or even underserved areas, a reducing number of medical providers, and even economic circumstances have all led to the introduction of nurse practitioners (NP) as part of the solution to the issues that plague care delivery. The implementation of the NPs, by and large, ensures that there is improved access to treatment, provision of cost-effective care and outreach services, and that clinical expertise to other health professionals and mentorship is made possible. Most importantly, however, NPs have the authority to prescribe medications that are related to their scope of practice (Fong, Buckley, & Cashin, 2015).
As prescribers, NPs must gather information, make clinical decisions, communicate and evaluate before they initiate, continue, or cease the prescription of drugs. NPs in the United States are vital as they reduce physician workloads and address the needs of patients in remote areas. As part of their mandate, they have prescriptive privileges that have continually superseded the original arrangement that required they collaborate with other medical practitioners, and they are now able to independently prescribe medication that can also include controlled drugs in some 21 states (AANP, 2017). For NPs to obtain licensure, they must possess a master’s degree, a post-master’s certificate, or a doctoral degree in nursing from a recognized or endorsed program. Indeed, there is even a move that NPs be required to hold a doctorate as an entry level qualification. In general terms, however, the Board of Nursing for the state in which NPs practice is charged with licensing nurse practitioners, and as is the case in such circumstances, each state has its own regulations (Fong, Buckley, & Cashin, 2015).

Musculoskeletal conditions comprise one of the leading causes of severe long-term pain in patients. I grew up in the midst of Miami and graduated from Felix Varela Senior High; to further my education, I completed my AS Degree here at FNU and have moved myself forward throughout the years. I am currently a STEMI nurse for Mount Sinai Medical Center where I’ve been for almost two years, but have almost 4 years of ICU experience working for the Bellevue Hospital in New York City.

As a nurse practitioner and provider various interrelated factors have contributed to changes in how nurses deliver care today. The unobtainability of adequate health care services in rural or even underserved areas, a reducing number of medical providers, and even economic circumstances have all led to the introduction of nurse practitioners (NP) as part of the solution to the issues that plague care delivery. The implementation of the NPs, by and large, ensures that there is improved access to treatment, provision of cost-effective care and outreach services, and that clinical expertise to other health professionals and mentorship is made possible. Most importantly, however, NPs have the authority to prescribe medications that are related to their scope of practice (Fong, Buckley, & Cashin, 2015).
As prescribers, NPs must gather information, make clinical decisions, communicate and evaluate before they initiate, continue, or cease the prescription of drugs. NPs in the United States are vital as they reduce physician workloads and address the needs of patients in remote areas. As part of their mandate, they have prescriptive privileges that have continually superseded the original arrangement that required they collaborate with other medical practitioners, and they are now able to independently prescribe medication that can also include controlled drugs in some 21 states (AANP, 2017). For NPs to obtain licensure, they must possess a master’s degree, a post-master’s certificate, or a doctoral degree in nursing from a recognized or endorsed program. Indeed, there is even a move that NPs be required to hold a doctorate as an entry level qualification. In general terms, however, the Board of Nursing for the state in which NPs practice is charged with licensing nurse practitioners, and as is the case in such circumstances, each state has its own regulations (Fong, Buckley, & Cashin, 2015).

Ethical Relativism is the claim that moral views are relative to the culture in which one lives or to the individual (also called Subjectivism). Many people declare themselves to be ethical relativists, but very few actually believe it to be true in practice. Often people are simply trying to avoid getting into an argument when they say that their ethical positions are just opinions. If it was true that you should avoid arguments about ethical issues, you would have to believe that there are good moral or possibly prudential reasons for not getting into arguments with others, that it was good for everyone to avoid conflict about controversial issues, which means that it is simply correct to be tolerant, making you opposed to relativism. depression is on the rise. Recent surveys indicate that as many

worldwide is a big concern and it is important to know how to
detect it. Depression can be difficult to diagnose in adolescents
because adults may expect them to act moody. Also, adolescents

They may not be aware of the symptoms of depression and may
not seek help. This proposal addresses an opportunity to
improve providers’ skills to properly identify potential risk
factors and effective treatment tools we can use to better the

Abstract
Even though persistent pain is highest among
older adults, they have been insufficiently
represented in clinical trials and studies on the
management of persistent pain. Agism has had
a negative impact on the adequate assessment
and management of pain in older adults.
Evidence has emerged indicating serious risk for
cardiovascular, gastrointestinal and renal
dysfunction with consistent use of NSAIDs and
COX-2 analgesics. Other evidence supports the
efficacious use of opioid analgesic in the
treatment of moderate-to-severe persistent pain
in older adults. Updated guidelines recommend
more reliance on opioid analgesia.. This
proposal aim is to implement an evidence-based
practice protocol on the pharmacological
management of persistent pain in older adults.

least some of the time. Undertreated persistent
pain in older adults has been linked to
depression, anxiety, cognitive impairment,
delirium, sleep disturbances, functional decline
and increased healthcare utilization and cost.
This project will implement a best practice so
that each older adult in a clinic will be assessed
and treated more effectively.

Untreated or Inadequate Pain Management
• Persistent pain is under-recognized and under-treated
in older adults in many settings.
• Those with dementia have a greater risk for
inappropriate and inadequate pain treatment.
Pain Assessment with Older Adults
• Many older adults under-report their pain so as not to
be a burden.
• Numeric Rating Scale (1-10) has been shown
ineffective with older adults.
•IPT has high validity and reliability.
Opioid Use with Older Adults
• Guidelines support for persistent pain.
• Prescribing practice of opioids in older adults is mixed.
