7. Begin your paperwork by entering
your Full Name exactly as it
appears on your birth certificate

8. Full name includes your full
middle name and any suffixes
such as Jr. or Sr.

mistakes happen.

names on your birth
certificate you must
include all 5 names

10. Fill in your official Company
Name as registered with MSHA

11. Complete your Mine or Contractor I.D.

13. Contractors are not
required by law to have
an I.D. If your company
doesn’t have an MSHA

14. The training time on
your certificate must
add up to at least 24
hours.
