Diana was raped by a man she was interviewing for her thesis. She and her husband were trying to conceive their second child. Taking the morning after pill would prevent pregnancy from the rape but there is a chance it could also block implantation of an embryo from her husband. This presents Diana with a moral dilemma of whether to take the pill. She will use her own moral compass and consider her circumstances to make the choice that feels right to her.

Barbara is a 25-year-old, heterosexual, Caucasian female and is the mother of a 6-month-old baby boy. She is currently unemployed and has neither specific skills nor a college education. At the time we first met, she was living her with her son, Jonah, and her boyfriend, Scott (also Jonah’s father), in a home that her grandparents purchased for her. Scott, a 29-year-old, heterosexual, Caucasian male, is employed by a flooring company, although his work is not regular. Both Barbara and Scott have a long history of severe polysubstance abuse, including heroin addiction. They are both currently prescribed methadone.
Jonah was born with severe birth defects due in part to Barbara’s and Scott’s drug use. Jonah remained in the hospital for several weeks after his birth, and during that time he underwent multiple surgeries. Among other abnormalities, he was born with two stomachs, one of which formed on the exterior of his body. He will need additional surgeries in the future and his stomach will never be fully functioning. The full extent of his disabilities is not certain at this time.

Carol is a 23-year-old, heterosexual, Caucasian female and the
mother of a 1-year-old baby girl. She is currently unemployed,
having previously worked for a house cleaning company. The
baby is healthy and developmentally on target, and she and the
parents appear to be well bonded with one another. Carol lives in
a rented house with her husband, Joseph. Joseph is a 27-year-old,
heterosexual, Hispanic male. He was recently arrested at their
home for a drug deal, which he asserts was a setup. Both parents
were charged with child endangerment because weapons were
found in the child’s crib and drugs were found in the home. The
parents assert that the child never sleeps in the crib but in their
bed. As a result of the parents’ arrest, social services was notified,
and the child was temporarily placed in a kinship care arrange-
ment with the maternal grandmother, who resides nearby. As a
result of Joseph’s arrest, he was fired from the cleaning company
where he worked, and the family is now experiencing financial
difficulties.
After initial contact was made with the parents, a number of
concerns were noted and the family was recommended for addi-
tional case management. Carol’s mother indicated that she had
concerns about Carol’s drinking habits and stated that Carol’s

Raban King is a 45 male adult married with three children. He lives in Allston, Boston and his phone number is +1 845-892-3344. Raban is a substance abuse social worker working with the Avernus Rehabilitation Center which a not-for-profit organization that helps members of community suffering from any form of substance abuse to overcome the addiction. The Avernus Rehabilitation Center contact are +1 615-615-9090 and +1 615-655- 3939. Raban is scheduled to be interviewed on the 25th of May 2020 to determine whether he is a suitable candidate for participation in a summer mission’s trip in a very challenging environment.
Reason for Referral
Raban was referred to me for evaluation by the mission’s board for further consultation. The mission’s board found Raban eligible for the summer mission’s trip that is expected to take place in a very challenging environment and they believe his type of work and personality can help him push through with the mission.
Current Situation and Functioning
From the information obtained from Raban, he is a substance abuse social worker and has been in the career for twelve years. He claims he is passionate about substance abuse and his desire is to help as many people to overcome the challenge and make a positive transition. He further claims that his typical day is spent either in the field seeking and collecting substance abuse victims or in the facility offering assistance and treatment to those already admitted. He spends the day counselling them and also making sure they take their medication and feed properly. On some occasional, he has to travel far and wide after receiving information about substance abuse victims so as to carry them using facility vans to the center.
Raban claims that due to his great passion, he has the ability to accomplish his daily tasks within the stipulated time. From assessment made, he has high coping skills since he is calm, patient, and humble. Also, despite being put through uncomfortable situations, he appeared sensitive and emotionally balanced. Also, it is evident that he has a high problem-solving and conflict resolution abilities from the many situations and cases he has faced in his line of duty and compelled to handle them. He also appears empathic, cooperative, and good manager of stress. One of the main weaknesses noted is that he rarely speaks and he seemed to agree with a lot of things a personality that appears easy to manipulate. Nevertheless, he has a main strength in the sense that whenever he set out to do something, he pushes through with it no matter how hard.
Relevant Medical History
According to Raban, there is no major illness or injury that he has ever suffered from or he is suffering from and physically he appears strong. He is not on medication and does not have any form of disability. He only has one brother and two sisters who are all healthy and his pa.

where he worked, and the family is now experiencing financial
difficulties.
After initial contact was made with the parents, a number of
concerns were noted and the family was recommended for additional
case management. Carol’s mother indicated that she had
concerns about Carol’s drinking habits and stated that Carol’s
father and grandfather were alcoholics. She and the father separated
when Carol was a baby, and Carol has had only limited
contact with him. There appears to be significant tension between
the grandmother and Carol and Joseph. I addressed the alcohol
issue with both parents, who denied there was a problem, but
shortly after the discussion, Carol was involved in a serious car
accident with the baby in the car. She was determined to have been
under the influence of alcohol. I advised Carol that she could not
have any unsupervised contact with her child until she completed
intensive inpatient substance abuse treatment. I made arrangements
for her placement, but after a week, she was discharged
for noncompliance with the rules. She was then referred to an
intensive outpatient program and began therapy there. Initially
her attendance was erratic because she had lost her license as a
result of the DUI. Eventually, however, she became engaged in the
program and began to address her issues. She acknowledged that
she had started using drugs at a very young age but said that she
had only begun drinking in the previous year or so. We discussed
the genetics of her family, and she said that she realized that she
had deteriorated rapidly since beginning to drink and knew that
she simply could not drink alcohol.
Joseph’s mother is deceased, and his father travels extensively
in his job and is not available as a support. Joseph was
very devoted to his mother and was devastated by her premature
death. We discussed the strengths that he and Carol demonstrated

Ms. Torrez is a 17-year-old Caucasian woman without prior psychiatric history who is brought to the Emergency room by ambulance after her parents called 911 when they found her having a seizure in their living room. She was admitted to the medical intensive care units in status epilepticus and was quickly stabilized with intramuscular lorazepam and fosphenytoin loading. Her heigh is 5 feet 6 inches, she is of medium build, and her weight is 101 lbs. (BMI16.3kg/m2). She does not suffer any medical conditions, and this is her first seizure. Laboratory workup shows an electrolyte imbalance as the most likely cause of the seizures. Although initially reluctant, she admits to purging with the use of ipecac several times this week. She reports that although she normally restricts her daily caloric intake to 500 calories, she regularly induces vomiting if her weight is above 100 lbs. Her last menstrual cycle was 1 year ago. Identifying Information
Raban King is a 45 male adult married with three children. He lives in Allston, Boston and his phone number is +1 845-892-3344. Raban is a substance abuse social worker working with the Avernus Rehabilitation Center which a not-for-profit organization that helps members of community suffering from any form of substance abuse to overcome the addiction. The Avernus Rehabilitation Center contact are +1 615-615-9090 and +1 615-655- 3939. Raban is scheduled to be interviewed on the 25th of May 2020 to determine whether he is a suitable candidate for participation in a summer mission’s trip in a very challenging environment.
Reason for Referral
Raban was referred to me for evaluation by the mission’s board for further consultation. The mission’s board found Raban eligible for the summer mission’s trip that is expected to take place in a very challenging environment and they believe his type of work and personality can help him push through with the mission.
Current Situation and Functioning
From the information obtained from Raban, he is a substance abuse social worker and has been in the career for twelve years. He claims he is passionate about substance abuse and his desire is to help as many people to overcome the challenge and make a positive transition. He further claims that his typical day is spent either in the field seeking and collecting substance abuse victims or in the facility offering assistance and treatment to those already admitted. He spends the day counselling them and also making sure they take their medication and feed properly. On some occasional, he has to travel far and wide after receiving information about substance abuse victims so as to carry them using facility vans to the center.
Raban claims that due to his great passion, he has the ability to accomplish his daily tasks within the stipulated time. From assessment made, he has high coping skills since he is calm, patient, and humble. Also, despite being put through uncomfortable situations, he appeared sensitive and emotionally balanced. Also, it is evident that he has a high problem-solving and conflict resolution abilities from the many situations and cases he has faced in his line of duty and compelled to handle them. He also appears empathic, cooperative, and good manager of stress. One of the main weaknesses noted is that he rarely speaks and he seemed to agree with a lot of things a personality that appears easy to manipulate. Nevertheless, he has a main strength in the sense that whenever he set out to do something, he pushes through with it no matter how hard.

Overall life span (LS) was 1671.7±1721.6 days and cumulative 5YS reached 62.4%, 10 years – 50.4%, 20 years – 44.6%. 94 LCP lived more than 5 years without cancer (LS=2958.6±1723.6 days), 22 – more than 10 years (LS=5571±1841.8 days). 67 LCP died because of LC (LS=471.9±344 days). AT significantly improved 5YS (68% vs. 53.7%) (P=0.028 by log-rank test). Cox modeling displayed that 5YS of LCP significantly depended on: N0-N12, T3-4, blood cell circuit, cell ratio factors (ratio between cancer cells-CC and blood cells subpopulations), LC cell dynamics, recalcification time, heparin tolerance, prothrombin index, protein, AT, procedure type (P=0.000-0.031). Neural networks, genetic algorithm selection and bootstrap simulation revealed relationships between 5YS and N0-12 (rank=1), thrombocytes/CC (rank=2), segmented neutrophils/CC (3), eosinophils/CC (4), erythrocytes/CC (5), healthy cells/CC (6), lymphocytes/CC (7), stick neutrophils/CC (8), leucocytes/CC (9), monocytes/CC (10). Correct prediction of 5YS was 100% by neural networks computing (error=0.000; area under ROC curve=1.0).
