The experience I wish to use to illustrate an ethical decision has to do with a doctor friend of mine, who
told me about the event several years ago.
My friend was a young medical doctor, actually just graduated from medical school. His grandfather at that
time was a very old man and had a number of ailments he was battling with. My friend was one of the
doctors who was taking care of the old man, and he was well acquainted with his situation.
One night, in fact in the middle of the night, around 2 am, my friend’s grandfather became very ill, and
emergency medical facilities like ambulances were not readily available at that time of the night. The family
was at a loss as to what to do. Then somebody remembered that one of the neighbours was working in a
hospital, either as a pharmacy technician or assistant, or some similar job description-he was not a medical
doctor, in any case. Due to the desperate situation at hand, it was suggested that this hospital staff should
be contacted and pleaded with to come and examine the old man and see whether he could help in any
way.
The hospital staff was contacted and begged to come and see the old man, and he reluctantly agreed to do
so, despite the interruption to his sleep. Upon examining the patient, the hospital staff concluded that the
man was a having a severe attack of asthma. He then went ahead and administered a drug for asthma
patients to the old man by intravenous injection. A few hours later, the old man died.
My friend the young doctor was informed the following morning on what happened to his grandfather, and
he was utterly shocked. His grandfather had never had any history of asthma-he had never in his life been
treated for asthma. No medical doctor would have prescribed that drug for that patient.
It is at this point that the ethical question arose, and there was need to make an ethical decision on what to
about this person who was not a medical doctor but went ahead to not only prescribe, but administer a
drug to this patient.
In the argument that followed this event, there were two groups with views that varied with each other.
One group argued straight away that the person in question should be prosecuted, because what he did
amounted more or less to pre-meditated murder. He should not have administered any drug to the patient,
since he was not qualified to do so. He should have concentrated on finding ways to get the patient to a
hospital.
The other group argued that this person was just acting as good Samaritan, trying to help out the ailing old
man. They saw it as an act of ingratitude to even consider prosecuting this man.
In the end, the ethical decision made was based on the Duty-based Ethical theory, since it is a universal rule
that only qualified medical personnel are to diagnose and prescribe drugs to patients, as well as administer
those drugs. The medical staff was neither of these-he was merely a te.

In the mid 1970s, a nursing educator in Idaho had contact, through a student, with a female client who had chronic myelogenous leukemia. This form of leukemia can often be managed for years with little or no chemotherapy. The woman had done well for about twelve years and ascribed her good condition to health foods and a strict nutritional regime. However, her condition had turned worse several weeks before and her physician had advised her that she needed chemotherapy if she were to have any chance at survival. The physician had also advised her of the potential side effects of the therapy including hair loss, nausea, fever, and immune system suppression.
The woman consented to the therapy and signed the appropriate forms, but later, she began to have second thoughts. The nursing educator and student had given the patient one dose of the therapy when the woman began to cry and express her reservations about the therapy. She questioned the nurse about alternative treatments to the use of chemotherapy. The patient related that she had accepted the therapy because her son had advised her that this was the best treatment. She related that she had not asked about alternate forms of treatment as the physician had indicated that chemotherapy was the only treatment indicated. The nurse did not discuss the patient's concerns with the physician, and later that evening, she talked to the patient about alternate therapies. In the discussion, rather nontraditional and controversial therapies were covered including reflexology and the use of laetrile. During the talk, the nurse made it very clear that the treatments under discussion were not sanctioned by the medical community.
The patient's feelings toward alternate therapies were strengthened by the evening's conversation; however, she continued with chemotherapy. The treatments, however, did not bring remission to her crisis and she died two weeks later. Upon hearing about the conversation between the off duty nurse educator and his patient, the physician brought charges against the nurse for unprofessional conduct and interfering with the patient-physician relationship. (In re Tuma, 1977).

The experience I wish to use to illustrate an ethical decision has to do with a doctor friend of mine, who
told me about the event several years ago.
My friend was a young medical doctor, actually just graduated from medical school. His grandfather at that
time was a very old man and had a number of ailments he was battling with. My friend was one of the
doctors who was taking care of the old man, and he was well acquainted with his situation.
One night, in fact in the middle of the night, around 2 am, my friend’s grandfather became very ill, and
emergency medical facilities like ambulances were not readily available at that time of the night. The family
was at a loss as to what to do. Then somebody remembered that one of the neighbours was working in a
hospital, either as a pharmacy technician or assistant, or some similar job description-he was not a medical
doctor, in any case. Due to the desperate situation at hand, it was suggested that this hospital staff should
be contacted and pleaded with to come and examine the old man and see whether he could help in any
way.
The hospital staff was contacted and begged to come and see the old man, and he reluctantly agreed to do
so, despite the interruption to his sleep. Upon examining the patient, the hospital staff concluded that the
man was a having a severe attack of asthma. He then went ahead and administered a drug for asthma
patients to the old man by intravenous injection. A few hours later, the old man died.
My friend the young doctor was informed the following morning on what happened to his grandfather, and
he was utterly shocked. His grandfather had never had any history of asthma-he had never in his life been
treated for asthma. No medical doctor would have prescribed that drug for that patient.
It is at this point that the ethical question arose, and there was need to make an ethical decision on what to
about this person who was not a medical doctor but went ahead to not only prescribe, but administer a
drug to this patient.
In the argument that followed this event, there were two groups with views that varied with each other.
One group argued straight away that the person in question should be prosecuted, because what he did
amounted more or less to pre-meditated murder. He should not have administered any drug to the patient,
since he was not qualified to do so. He should have concentrated on finding ways to get the patient to a
hospital.
The other group argued that this person was just acting as good Samaritan, trying to help out the ailing old
man. They saw it as an act of ingratitude to even consider prosecuting this man.
In the end, the ethical decision made was based on the Duty-based Ethical theory, since it is a universal rule
that only qualified medical personnel are to diagnose and prescribe drugs to patients, as well as administer
those drugs. The medical staff was neither of these-he was merely a te.

In the mid 1970s, a nursing educator in Idaho had contact, through a student, with a female client who had chronic myelogenous leukemia. This form of leukemia can often be managed for years with little or no chemotherapy. The woman had done well for about twelve years and ascribed her good condition to health foods and a strict nutritional regime. However, her condition had turned worse several weeks before and her physician had advised her that she needed chemotherapy if she were to have any chance at survival. The physician had also advised her of the potential side effects of the therapy including hair loss, nausea, fever, and immune system suppression.
The woman consented to the therapy and signed the appropriate forms, but later, she began to have second thoughts. The nursing educator and student had given the patient one dose of the therapy when the woman began to cry and express her reservations about the therapy. She questioned the nurse about alternative treatments to the use of chemotherapy. The patient related that she had accepted the therapy because her son had advised her that this was the best treatment. She related that she had not asked about alternate forms of treatment as the physician had indicated that chemotherapy was the only treatment indicated. The nurse did not discuss the patient's concerns with the physician, and later that evening, she talked to the patient about alternate therapies. In the discussion, rather nontraditional and controversial therapies were covered including reflexology and the use of laetrile. During the talk, the nurse made it very clear that the treatments under discussion were not sanctioned by the medical community.
The patient's feelings toward alternate therapies were strengthened by the evening's conversation; however, she continued with chemotherapy. The treatments, however, did not bring remission to her crisis and she died two weeks later. Upon hearing about the conversation between the off duty nurse educator and his patient, the physician brought charges against the nurse for unprofessional conduct and interfering with the patient-physician relationship. The client was diagnosed with HIV approximately 25 years ago but indicated that his wife was not “positive.” The client indicated having multiple concerns with his ability to eat, sleep, function from day to day, and that he is oftentimes afraid of what he might do to himself. Client was asked and also assessed for suicidal ideations, and was administered a PHQ-9 to assess if client should be further evaluated for depression or to determine if current symptoms are a result of “normal” grief. The client also expressed that he has a known opioid addiction to prescription pain pills. While in therapy, the client repeatedly expressed how much he was currently in pain.
Utilizing information from the course readings, describe the approach you used when working with these presenting issues, and how do you determine which approach would be most effective?
The intern and supervisor let the client express himself and his reasons for coming into the facility, as he presented himself to be in a crisis. One particular approach that the intern attempted to use with the client was motivational interviewing by expressing empathy, offering reflective listening, attempting to help the client develop self-efficacy, and attempting to understand where the client is and where he would like to be. The intern wanted to determine and help to strengthen the client’s motivation overcome his addiction in order to link him to other services, such as that could help provide pharmacological treatment, address physical health needs, and locate other social support systems that can be beneficial to helping his current presenting issues.
However, the client came to therapy and dropped out of therapy after the first session and did not keep his follow-up appointment for his HIV care, per the client’s primary physician. Thus, it is hard to decipher if the patient came to therapy because he wanted help dealing with his mental incapacities and his physical health or whether this was an outcry for an attempt to retrieve pain medications. Depression, Shields (2006) writes that she had always dreamed
of being a mother. Although she and her husband, Chris Henchy,
initially had trouble conceiving, Shields eventually became
pregnant through in vitro fertilization and gave birth to a daugh-
ter in 2003. Just as her attempt at conceiving wasn’t without
effort, easing into life as a mother wasn’t effortless, either.
Almost immediately after returning home from the hospital,
Shields began to experience symptoms of postpartum depres-
sion. Once referred to as the “baby blues,” postpartum depres-
sion has recently come to be considered a very legitimate type
of depression. Symptoms range from anxiety and tearfulness to
feelings of extreme detachment and even being suicidal.
Shields notes that her “baby blues” rapidly gave way to full-
blown depression, including thoughts of self-harm and frighten-
ing visions of harm coming to her baby. In addition to the birth of

This patient has gone through treatment day and night for more than years. Such situation worsens the patient pain and suffering but who else can feel what the patient is feeling? Parent should decide on what they would do to their child regardless. In my opinion, the child will depend on the parent to survive living healthy. It would be very painful if the parent income -wise is poor. Which will lead the child to go through painful life. Therefore, parent knows their situation at hand and they need to make their own decision regardless.

- 1. Moral Dilemmas Moral dilemmas are thought experiments which ask you to imagine a difficult situation and decide what you think the morally correct course of action would be. There are no truly ‘right’ answers to these questions, as they often ask you to compare two different moral imperatives and choose which one you feel is most important. For example, if we accept that it is morally correct to never torture a living creature, and that it is morally correct to save a human’s life if you have the ability to do so, how do you decide what to do if you can only save a human’s life by torturing someone else? Read through this moral dilemma, and have a think about what you might do this situation. 1.- The Deliberate Infection Ken was a doctor. One of his patients, whom he had diagnosed as HIV positive, was about to receive a blood transfusion prior to being released from the hospital. He had told Ken, in the confidence of their doctor-patient relationship, that after he got his transfusion, and his medicine from Ken, he intended to infect as many people as possible with HIV starting that evening. It occurred to Ken that he could contaminate his medication by putting an untraceable poison in it that would kill him before he got a chance to infect others. Should Ken have poisoned this man in order to prevent him from spreading HIV?
