 Meds2Home was created to assist Medicare Skilled
Nursing Facilities or Hospitals with Discharges to home,
independent living or assisted living facilities.
 We know Medications, their use, and/or failure to
continue usage as properly prescribed by their physician
contributes significantly to hospital re-admissions after
skilled nursing discharge.
 We know the role of nursing homes has changed.
Facilities are increasingly dedicated to transitioning older
adults from the hospital back to their own homes. Short
term stays has grown tremendously in the last 10 years.
 What We Know….
 20% of Medicare patients discharged from hospitals to
skilled nursing facilities are “re-hospitalized” for
conditions that could have potentially been avoided.
 40% of these patients being “re-hospitalized” are due to
medication errors.
 MEDPAC is currently tracking the “re-hospitalizations”
rates for nursing home residents, and are monitoring the
“rates for residents with potentially avoidable conditions”
 As a part of the Affordable Care Act, MEDPAC wants to
target and penalize skilled nursing providers with
aberrant patterns of “re-hospitalizations” Specifically duel
eligible's.
 What We Know….
 According to MEDPAC, Medicare payments to PAC providers
totaled $59 billion in 2013, more than doubling since 2001,
and the agency spends nearly as much on post-acute care
and readmissions in the first 30 days after discharge as it
does for the initial hospital admission.
 As a part of the Protecting Access to Medicare Act of 2014,
skilled nursing facilities are required to begin public
reporting of re-admission rates starting October 1, 2017.
 That CMS will begin withholding 2% of all SNF Medicare
payments starting October 1, 2018, and CMS will then re-
distribute 50-70% of the withheld back to SNF’s by way of
incentive payments to high performing facility providers with
reduced hospital re-admissions.
