General Structure of a Forensic Report (
lab – specimen
 Title of the Examination report
 Name and address of the laboratory with contact information like telephone,
mobile, fax, email
 Affiliation
 experience must be mentioned on the letter head of the report or at the end
of the report.
 Unique ID No. of the report with date
 Name of the customer
 Case Enquiry/ DD/ FIR no. ………date ………….u/s……….Police Station, under
which court
General Structure of a Forensic Report
 Mode of receipt of material (evidences/specimens) : Through messenger or by
post / by mail
 Sampling Method; How the sampling done by the IO/ Investigator/ Forensic
Expert
 Reference to the Test Method(s); Reference to lab procedure manual/
books/published standard method
 Description of Specimens/Parcels/Samples/Exhibits etc.
 Methodology of Examination
 Result of Examination & Opinion
 Signature or examining officer along with seal.
Examination of the forensic patient
 conducted in a thorough head-to-toe or toe-to-head manner
 using a body-map or wound chart
 The entire body surface should be palpated to identify areas of bruising that
may not yet be visible.
 Documentation and collection of evidence typically occurs at the same time
as the physical exam
 During the exam, vital signs should be taken and any urgent medical needs
should be addressed.
