4. SOCIAL HISTORY
• Family moved to the United States from India six years ago.
• Practices the Hindu religion.
• In the fourth grade and is known to be a good student.
• Likes to write poems.
5. MEDICAL HISTORY
• Maya is under the care of a physician for her asthma.
• Has had several emergency room visits for difficulty breathing,
but she has not been hospitalized in the last 5 years.
Medications:
• Montelukast sodium (Singulair®)- leukotriene receptor
• Antagonist albuterol (Proventil HFA®)- bronchodilator inhaler
Medications cause xerostomia, sore throat, taste changes, and increased
anxiety.
6. DENTAL HISTORY & CHIEF COMPLAINT
• Maya keeps on track with her 6 month recall appointments for
oral prophylaxis, where she receives professionally applied
fluoride treatments
• Uses a fluoride toothpaste on a regular basis
• Impressions for study cast have been indicated to evaluate
occlusion
• Exaggerated gag reflex and and mouth breathing
• Chief Complaint: Nervous about dental treatment.
7. CURRENT ORAL HYGIENE STATUS
• Generalized marginal plaque with slight bleeding on probing
• BI is 10%
• Slight supragingival calculus lingual surfaces of the mandibular
anterior teeth
• PI is 15%
• Slight generalized brown stain
• Flosses occasionally
• Probing Depths: Range from 1-3 mm

• Gingiva is pointed, firm, stippled, and has pink/coral
appearance. There are pigmented areas due to Maya ethnicity.
• Slight tongue thrust
• Slight gingival sensitivity distal to the mandibular left
permanent canine
• Maxillary left primary first molar is mobile
• Restoration on tooth #I

16. PROGNOSIS AND GOALS
• Good with continuing home care and routine dental visits.
• Short Term: Lower BI and PI by half. BI down to 5% and PI down
to 7%. Reduce cariogenic beverages, increase water intake.
• Long Term: Reduce BI and PI to zero.
• Referral: Sealants for permanent first molars
17. PROCEDURE
• Preprocedural & postprocedural rinse (non-alcoholic)
• Scaling to remove hard/soft deposits
• Polishing with fine paste to remove brown stain
• Fine paste should also be applied because she is young
• Recommend sealants for first molars
