The owner permits employees and equipment of Repertory Records to be on the premises for a production project. Repertory Records will not be held liable for any damage or injuries caused to the premises or people during the production. The owner confirms they have the legal right to grant permission and agrees that Repertory Records owns all footage filmed at the location and can use it for the project and promotion as they see fit. Both parties sign to agree to the terms.

1. 1. In considerationtoall of the above,the ownerherebyconsentstopermit RepertoryRecords
employees,agentsandall necessaryproductionequipmenttobe inand aroundthe premisesonthe
above date(s) forthe purposesof productioninconnectionwiththe above project.
2. In connectionwiththe project, RepertoryRecords will holdthe ownerharmlessfromanydamage
to the premisescausedbyitsagentsor employeesandforinjurytoanypersonsonthe site of the
premisescausedbysaidrepresentativesof RepertoryRecords.
3. The ownerwarrantsthat it hasthe legal rightandpowerto grant the rightsgrantedhere.The
owneralsoacknowledgesandagreesthatall right,title andinterestinall footage of the premises
filmedby RepertoryRecords duringthe above-mentionedday(s) will belongsolelyandcompletelyto
RepertoryRecords foritsuse in anymannerand mediaitseesfit.The owneragreesthat Repertory
Records may use and authorize otherstouse the likenessandinformationconcerningthe premises
for projectpublicityandinstitutional promotional purposesinconnectionwiththe project.
