The Washington Report does not specifically use the term
homelessness. But as you will see. It is relevant.
While the secondary consequences of PAE, as described in
the Washington Report and other subsequent papers, relate
homelessness etc to fasd, fasd and pae are rarely related to
homelessness. So, there are many papers etc about the
relationship of fasd to homelessness, but those who manage
and research homelessness generally ignore or downplay the
crucial role of pae and fasd.

Drug addiction is a problem for people in every community in the United States. From growing urban areas to beautiful suburbs to the friendly rural areas, people of all ages, genders and cultures struggle with addiction. Strong family backgrounds and socioeconomic status do not protect people from drug abuse. With more news reports about drug addiction and overdoses, it becomes clearer that drugs are a problem everywhere, even in places as idyllic as California. “San Joaquin County’s rate of drug-induced deaths is 56% higher than average rate across California (17.3 per 100,000 compared to 11.1 per 100,000)” ( San Joaquin County 2016 CHNA, 2016). 2 Description of the community San Joaquin County contains both rural and urban areas. Multi-cultural community Residents rate their health poorer than the state overall Notable health disparities in health status between county and state “San Joaquin County faces many of the same challenges seen throughout the state, but often to a greater degree” ( San Joaquin County 2016 CHNA, 2016). San Joaquin has one of the highest rates in California for diabetes mortality. Youth development tends to be undermined trauma and violence, unhealthy family functioning, and insufficient access to youth facilities. Poverty and unemployment are high compared to the state. Major concerns are often associated with family and community violence. “41.1% of community survey respondents report that drug abuse is among the most concerning health behaviors in their community” (San Joaquin County 2016 CHNA, 2016). There is a lack of safe and affordable housing. San Joaquin county ranks 9th highest in the nation for most polluted air. 3 Description of the boundaries The county is located in Northern California's Central Valley just east of the very highly populated nine-county San Francisco Bay Area region and is separated from the Bay Area by the Diablo Range of low mountains with its Altamont Pass. 4 The Stockton Metro area is divided by the U.S. Census Bureau into four neighborhood clusters. Ethical consideration is important in nursing practice, especially when providing care to patients from diverse sociocultural backgrounds. The population of the United States comprises various ethnic/racial groups with different cultural and social beliefs, practice, norms, and values. There is an increasing disparities on the incidence and prevalence of type 2 diabetes among different communities in the United States. According to the Center for Disease Control and Prevention (CDC), Hispanics and African Americans have the highest incidence and prevalence of type 2 diabetes in the country (Concha, Mayer, Mezuk, & Avula, 2016). Caring for patients from different ethnic/racial groups require consideration of ethical principles and concepts to prevent ethical issues that may arise during nurse-patient interaction. Recently, I cared for patient with type 2 diabetes mellitus who had been hospitalized for more than two weeks due to acute hypertension, partial loss of vision. Also, the patient had a chronic diabetic foot ulcer. The analysis of his medical history revealed that the diabetic foot ulcer had developed in the last two years and had never healed. The patient was so worried about his health status and kept asking when he was going to be discharged from the hospital. The patient came from the Hispanic community, which is one of minority groups with the highest incidence and prevalence of type 2 diabetes mellitus in the country. Being a culturally competent registered nurse, I had an obligation to take into account the specific ethnic background of the patient when providing care. Considering patient’s ethnic/racial background is important in providing quality, holistic, and patient-centered care based their health concerns, preferences, and values (Concha et al., 2016). When collecting subjective data for analysis and planning for the care. I asked the patient about his perceptions about the possible causes of type 2 diabetes that he was suffering from. Hispanics have different beliefs in the causation of diabetes mellitus (Frieden, 2016). First, the patient believed that diabetes is a temporary condition that is not fatal. Second, the patient narrated a story that attempts to identify the cause of diabetes and concluded that they believe that people with “good diabetes” do not experience a lot of complications. The Hispanics use the term “good diabetes” when referring to the type of diabetes that do not require insulin for therapeutic purposes; non-insulin-dependent diabetes mellitus (T2DM) (Frieden, 2016). Also, the patient had a fatalistic attitude and believed that his health condition is likely to be a punishment from God. The patient had a low health literacy level because caregivers had encouraged him to engage in some physical exercise and adopt a self-management approach as a way of controlling and preventing complications related to his condition, but he never implemented them. Analyses of Social Issues and Public Policy, Vol. 2, No. 1, 2002, pp. 223--238 Home Street Home: The Interpersonal Dimensions of Adolescent Homelessness Matthew Paradise∗ University of North Carolina Greensboro Ana Mari Cauce University of Washington This paper draws on the available literature, as well as unpublished data, to reconstruct the often chaotic early childhood experiences of homeless adolescents and to describe the problematic ways of relating that may result from these troubled life histories. We also examine the processes by which homeless youth become separated from their families, and we make connections between socio- emotional problems on the street and early neglect and abuse at home. The risk- amplification model is offered as a theoretically compelling explanation of the adolescent homeless experience, high rates of victimization on the streets, and mental health problems that arise out of these circumstances. Finally, we present a promising approach for intervening with homeless youth, one that explicitly targets this cycle of ineffective interpersonal relationships. It is one of life’s true injustices that the children who are most at risk for social, emotional, and behavioral problems so often come from equally vulnerable parents. Young people with the greatest needs frequently grow up in those families least able to meet them, and this scenario has been played out in the early years of many, if not most, youth who end up on the street. Health inequalities have been described by race, ethnic group, gender, and social class. A variety of theories have been proposed to account for health inequalities, including access to medical care and absolute material deprivation. Several theorists (including Michael Marmot and Richard Wilkinson) have argued that relative deprivation is the primary factor. By this they mean the inability of individuals to achieve the kind of lifestyle that is valued and considered normative in their social context. In this article, we show that the concept and measurement of cultural consonance can operationalize what Marmot and Wilkinson mean by relative deprivation. Cultural consonance is the degree to which individuals approximate, in their own beliefs and behaviors, the prototypes for belief and behavior encoded in shared cultural models. Widely shared cultural models in society describe what is regarded both as appropriate and desirable in many different domains. These cultural models are both directive and motivating: people try to achieve the goals defined in these models; however, as a result of both social and economic constraints, some individuals are unable to effectively incorporate these cultural goals into their own lives. The result is an enduring loss of coherence in life, because life is not unfolding in the way that it, culturally speaking, “should.” The resulting chronic stress is associated with psychobiological distress. We illustrate this process with data collected in urban Brazil. A theory of cultural consonance provides a uniquely biocultural contribution to the understanding of health inequalities.
