This case report describes the use of intravenous and endocavitary contrast-enhanced ultrasound (CEUS) to guide drainage of a complex multiseptated pyogenic liver abscess in an 84-year-old woman. CEUS with Sonovue contrast agent allowed visualization of abscess cavities, septations, and surrounding structures to safely place drainage catheters. Endocavitary CEUS through the catheters confirmed correct positioning. CEUS provided real-time imaging guidance without radiation, avoiding surrounding vessels and confirming drainage between cavities. The successful drainage with CEUS guidance improved the patient's condition.

This document reviews interventional endoscopic ultrasound (EUS) procedures, including EUS-guided fine needle aspiration (FNA). EUS-FNA is a safe and accurate procedure used to diagnose lesions in the esophagus, pancreas, lymph nodes, liver, lungs and other organs. It has a diagnostic accuracy of 64-94% for pancreatic masses. EUS also guides drainage of pancreatic pseudocysts and bile ducts when conventional endoscopy fails. Emerging applications include EUS-guided celiac plexus neurolysis for pain relief, fiducial placement for tumor localization, and ablation techniques for treating pancreatic cysts and tumors. In summary, the document outlines the various diagnostic and therapeutic applications of

Successful Repeated CT-Guided Drainage Of Rectal Mucocele After LAleksandr Reznichenko
This document describes a case study of a rectal mucocele that was successfully treated with repeated CT-guided drainage after a patient underwent low anterior resection for rectal prolapse. A rectal mucocele developed as a fluid-filled cyst near the rectal stump that caused symptoms. It was drained multiple times under CT guidance, with catheters inserted each time. Analysis of the fluid indicated it was a rectal mucocele rather than an abscess. This case demonstrates that repeated CT-guided drainage can successfully treat a rectal mucocele in a patient who was not a candidate for surgical resection.
Prospective evaluation of single operator peroral cholangioscopy in liverDr. Zubin Sharma M.D.
This study evaluated the feasibility and clinical utility of single-operator cholangioscopy (SOC) using the SpyGlass system in liver transplant recipients with biliary complications. The study found that SOC allowed for direct visualization of biliary anastomotic strictures and identification of two patterns (patterns A and B) with different responses to endoscopic treatment. Pattern A strictures responded better to ERCP therapy and required shorter stenting durations than pattern B strictures. SOC also detected additional bile duct stones in some patients. The results suggest SOC can help predict outcomes and guide management of biliary complications after liver transplant.

Managment Of Long Term Care In Era Covid-19komalicarol
COVID-19 gives the chance to address long-term care categories
that are sometimes disregarded and undervalued, such as nursing
and residential homes, as well as homecare. Each method of delivering long-term care must meet the highest possible standards
of ongoing care and quality of life. More study and evaluation are
needed to aid decision-making and policy-making, particularly on
the cost-effectiveness and cost-quality elements for each country,
region, or system.
Renal failure and Quality ofLlife Indicators in Kidney Transplantationkomalicarol
This article discusses quality of life indicators for patients undergoing kidney transplantation. It reviews several tools used to measure quality of life, including the SF-36, EQ-5D, and KDQOL questionnaires. Overall quality of life is generally improved after a successful kidney transplant compared to dialysis. However, factors like side effects of immunosuppressive drugs and the risk of rejection can still impact quality of life. The article concludes that while transplantation is associated with lower mortality and better quality of life than long-term dialysis, certain patient and treatment factors must be considered when evaluating individual quality of life outcomes.
A case of childhood Burkitt's lymphoma with gingival swelling as the first sy...komalicarol
This case report describes a 4-year-old child who presented with gingival swelling as the initial symptom of Burkitt's lymphoma. The child was eventually diagnosed with stage IV Burkitt's lymphoma/leukemia based on bone marrow and genetic testing. After initial chemotherapy, the gingival swelling and right cheek swelling recurred, indicating disease recurrence. The child received further chemotherapy but ultimately passed away half a year later. This case highlights that gingival swelling can be an early oral symptom of systemic disease like Burkitt's lymphoma. Dentists and oral physicians play an important role in identifying signs of systemic conditions through oral examinations.
Neuropsychiatric Profiles of Brivaracetam: A Literature Reviewkomalicarol
Anti-seizure medications (ASMs) can cause cognitive or behavioral adverse drug reactions, which is a significant consideration
when selecting an appropriate ASM. Brivaracetam (BRV) is a
newer synaptic vesicle protein 2A ligand, which is expected to
have less neuropsychiatric adverse effects due to its mechanism of
action. To understand the impact of BRV on cognition and behavior compared with other ASMs, we conducted literatures searching
from PubMed and MEDLINE databases. After the screening process, a total of two animal studies, one randomized controlled trial, one pooled-analysis of clinical trials, one controlled study and
nine observational studies were included. Animal studies showed
that BRV did not worsen cognition or behavior performance in rodents. Human studies showed that BRV had less cognitive adverse
events compared with other second or third generation ASMs. In
addition, currently available evidence suggests that behavioral disturbance is less common with BRV compared with levetiracetam.
This review revealed that BRV has a limited impact on cognition
and behavior. For patients who are intolerant to levetiracetam
and have levetiracetam-related behavioral side effects, switching
to BRV could be beneficial. However, the heterogeneity between
studies makes the quality of the evidence weak and further trials
are needed to confirm the findings.
Clinical and evolutionary features of SARS CoV-2 infection (COVID-19) in chil...komalicarol
Starting with December 2019 the medical world has faced a
new challenge as a consequence of a new type of coronavirus-2019-nCoV, similar to several familiar strains that determine
a comparable symptomatology (SARS- severe acute respiratory syndrome, MERS- Middle East severe acute respiratory syndrome), subsequently named SARS CoV-2, while the disease it

intermediate host (probably also a mammal) it suffered genetic
changes thus acquiring human cells receptors. In consequence,
SARS CoV-2 virus affects both children and even more frequently where it determines more severe clinical forms of disease. In
children, COVID-19 has various clinical forms, from asymptomatic ones to severe ones, complicated by multisystem inflammatory

Vonlay; A paradigm shift in post endodontic restoration: A case report.komalicarol
Porcelain veneers have long been a popular restorative option that
have evolved into a well- accepted treatment that can be fabricated
in various ways. Onlays are another common treatment modality
used in contemporary dentistry to restore large areas of decay and
to replace old restorations. With the availability of newer highstrength materials such as lithium disilicate and processing technologies like CAD/CAM and heat pressing, dental professionals
are now able to produce highly esthetic, high-strength restorations
that blend seamlessly with the natural dentition while also withstanding posterior occlusal forces. A tooth more complex restoration is required after endodontic treatment when compared to normal tooth restoration, because of factors such as extensive caries,
post-treatment root canal dentin and even the economics condition
of the patient. One such design proposed by Dr.Ronald E Goldstein
is “Veenerlay”or “Vonlay”. Vonlay is a blend of an onlay with an
extended buccal veneer surface for use in premolar region, where
there is sufficient enamel present to bond. This restorative option
requires a much less invasive preparation than a full coverage
crown but provides the same structural benefits. Thus, the aim of
this case report is to present a case of Vonlay following endodontic
treatement of lower mandibular premol

This document summarizes research on trends in height among children in Japan and South Korea from the mid-20th century onward. It finds that as the food supply and consumption of animal proteins increased in both countries following World War II and the Korean War, children grew taller. However, Japanese children stopped growing taller in the late 1980s while South Korean children continued increasing in height until the mid-2000s, eventually surpassing Japanese children. The author questions explanations that focus only on differences in ethnicity or animal protein consumption, noting changes in consumption of other nutrients may also play a role.

damage to young school children has been particularly great. One
may imagine a slowing trend in physical growth from the effects
of that disaster, but no clear findings have been reported.

Gene therapy can be broadly defined as the transfer of genetic material to cure a disease or at least to improve the clinical status of a patient.
One of the basic concepts of gene therapy is to transform viruses into genetic shuttles, which will deliver the gene of interest into the target cells.
Safe methods have been devised to do this, using several viral and non-viral vectors.
In the future, this technique may allow doctors to treat a disorder by inserting a gene into a patient's cells instead of using drugs or surgery.
The biggest hurdle faced by medical research in gene therapy is the availability of effective gene-carrying vectors that meet all of the following criteria:
Protection of transgene or genetic cargo from degradative action of systemic and endonucleases,
Delivery of genetic material to the target site, i.e., either cell cytoplasm or nucleus,
Low potential of triggering unwanted immune responses or genotoxicity,
Economical and feasible availability for patients .
Viruses are naturally evolved vehicles that efficiently transfer their genes into host cells.
Choice of viral vector is dependent on gene transfer efficiency, capacity to carry foreign genes, toxicity, stability, immune responses towards viral antigens and potential viral recombination.
There are a wide variety of vectors used to deliver DNA or oligo nucleotides into mammalian cells, either in vitro or in vivo.
The most common vector system based on retroviruses, adenoviruses, herpes simplex viruses, adeno associated viruses.

GASTROINTESTINAL INFECTIONS AND GASTRITIS
Osvaldo Bernardo Muchanga
Gastrointestinal Infections
GASTROINTESTINAL INFECTIONS result from the ingestion of pathogens that cause infections at the level of this tract, generally being transmitted by food, water and hands contaminated by microorganisms such as E. coli, Salmonella, Shigella, Vibrio cholerae, Campylobacter, Staphylococcus, Rotavirus among others that are generally contained in feces, thus configuring a FECAL-ORAL type of transmission.
Among the factors that lead to the occurrence of gastrointestinal infections are the hygienic and sanitary deficiencies that characterize our markets and other places where raw or cooked food is sold, poor environmental sanitation in communities, deficiencies in water treatment (or in the process of its plumbing), risky hygienic-sanitary habits (not washing hands after major and/or minor needs), among others.
These are generally consequences (signs and symptoms) resulting from gastrointestinal infections: diarrhea, vomiting, fever and malaise, among others.
The treatment consists of replacing lost liquids and electrolytes (drinking drinking water and other recommended liquids, including consumption of juicy fruits such as papayas, apples, pears, among others that contain water in their composition).
To prevent this, it is necessary to promote health education, improve the hygienic-sanitary conditions of markets and communities in general as a way of promoting, preserving and prolonging PUBLIC HEALTH.
Gastritis and Gastric Health
Gastric Health is one of the most relevant concerns in human health, with gastrointestinal infections being among the main illnesses that affect humans.
Among gastric problems, we have GASTRITIS AND GASTRIC ULCERS as the main public health problems. Gastritis and gastric ulcers normally result from inflammation and corrosion of the walls of the stomach (gastric mucosa) and are generally associated (caused) by the bacterium Helicobacter pylor, which, according to the literature, this bacterium settles on these walls (of the stomach) and starts to release urease that ends up altering the normal pH of the stomach (acid), which leads to inflammation and corrosion of the mucous membranes and consequent gastritis or ulcers, respectively.
In addition to bacterial infections, gastritis and gastric ulcers are associated with several factors, with emphasis on prolonged fasting, chemical substances including drugs, alcohol, foods with strong seasonings including chilli, which ends up causing inflammation of the stomach walls and/or corrosion. of the same, resulting in the appearance of wounds and consequent gastritis or ulcers, respectively.
Among patients with gastritis and/or ulcers, one of the dilemmas is associated with the foods to consume in order to minimize the sensation of pain and discomfort.
Breast cancer: Post menopausal endocrine therapyDr. Sumit KUMAR
Breast cancer in postmenopausal women with hormone receptor-positive (HR+) status is a common and complex condition that necessitates a multifaceted approach to management. HR+ breast cancer means that the cancer cells grow in response to hormones such as estrogen and progesterone. This subtype is prevalent among postmenopausal women and typically exhibits a more indolent course compared to other forms of breast cancer, which allows for a variety of treatment options.
Diagnosis and Staging
The diagnosis of HR+ breast cancer begins with clinical evaluation, imaging, and biopsy. Imaging modalities such as mammography, ultrasound, and MRI help in assessing the extent of the disease. Histopathological examination and immunohistochemical staining of the biopsy sample confirm the diagnosis and hormone receptor status by identifying the presence of estrogen receptors (ER) and progesterone receptors (PR) on the tumor cells.
Staging involves determining the size of the tumor (T), the involvement of regional lymph nodes (N), and the presence of distant metastasis (M). The American Joint Committee on Cancer (AJCC) staging system is commonly used. Accurate staging is critical as it guides treatment decisions.
Treatment Options
Endocrine Therapy
Endocrine therapy is the cornerstone of treatment for HR+ breast cancer in postmenopausal women. The primary goal is to reduce the levels of estrogen or block its effects on cancer cells. Commonly used agents include:
Selective Estrogen Receptor Modulators (SERMs): Tamoxifen is a SERM that binds to estrogen receptors, blocking estrogen from stimulating breast cancer cells. It is effective but may have side effects such as increased risk of endometrial cancer and thromboembolic events.
Aromatase Inhibitors (AIs): These drugs, including anastrozole, letrozole, and exemestane, lower estrogen levels by inhibiting the aromatase enzyme, which converts androgens to estrogen in peripheral tissues. AIs are generally preferred in postmenopausal women due to their efficacy and safety profile compared to tamoxifen.
Selective Estrogen Receptor Downregulators (SERDs): Fulvestrant is a SERD that degrades estrogen receptors and is used in cases where resistance to other endocrine therapies develops.
Combination Therapies
Combining endocrine therapy with other treatments enhances efficacy. Examples include:
Endocrine Therapy with CDK4/6 Inhibitors: Palbociclib, ribociclib, and abemaciclib are CDK4/6 inhibitors that, when combined with endocrine therapy, significantly improve progression-free survival in advanced HR+ breast cancer.
Endocrine Therapy with mTOR Inhibitors: Everolimus, an mTOR inhibitor, can be added to endocrine therapy for patients who have developed resistance to aromatase inhibitors.
Chemotherapy
Chemotherapy is generally reserved for patients with high-risk features, such as large tumor size, high-grade histology, or extensive lymph node involvement. Regimens often include anthracyclines and taxanes.
This presentation gives information on the pharmacology of Prostaglandins, Thromboxanes and Leukotrienes i.e. Eicosanoids. Eicosanoids are signaling molecules derived from polyunsaturated fatty acids like arachidonic acid. They are involved in complex control over inflammation, immunity, and the central nervous system. Eicosanoids are synthesized through the enzymatic oxidation of fatty acids by cyclooxygenase and lipoxygenase enzymes. They have short half-lives and act locally through autocrine and paracrine signaling.

PGx Analysis in VarSeq: A User’s PerspectiveGolden Helix
Since our release of the PGx capabilities in VarSeq, we’ve had a few months to gather some insights from various use cases. Some users approach PGx workflows by means of array genotyping or what seems to be a growing trend of adding the star allele calling to the existing NGS pipeline for whole genome data. Luckily, both approaches are supported with the VarSeq software platform. The genotyping method being used will also dictate what the scope of the tertiary analysis will be. For example, are your PGx reports a standalone pipeline or would your lab’s goal be to handle a dual-purpose workflow and report on PGx + Diagnostic findings.
The purpose of this webcast is to:
Discuss and demonstrate the approaches with array and NGS genotyping methods for star allele calling to prep for downstream analysis.
Following genotyping, explore alternative tertiary workflow concepts in VarSeq to handle PGx reporting.
Moreover, we will include insights users will need to consider when validating their PGx workflow for all possible star alleles and options you have for automating your PGx analysis for large number of samples. Please join us for a session dedicated to the application of star allele genotyping and subsequent PGx workflows in our VarSeq software.

Pyogenic liver abscess is a potentially life-threatening pathology, while image-guided drainage is
highly indicative as first-line treatment approach. We report the case of an 84-year-old woman, diag-
nosed with large multiseptated pyogenic liver abscess, aiming to stress out the immense contribution

4. Introduction
Throughout the last decades, an imaging revolution with the ac-
cretive use of Ultrasound Contrast agents (UCAs) and a gradual
establishment of Contrast Enhanced Ultrasound (CEUS) as anim-
aging technique, are being witnessed. Although the value of CEUS
in diagnostic practice have been demonstrated by numerous stud-
ies, the utilization of UCAs in interventional procedures has been
emerging the last few years, either with intravenous or endocavi-
tary administration [1].
On the other hand, pyogenic liver abscess (PLA) is a high mortality
associated pathology with a rate ranging from 10-40% [2]. Now-
adays, the vast majority of etiologies of PLAs concern pathologic
conditions of the biliary tract, when a certain risk factor is present
[3]. Combined antibiotic therapy and image-guided percutaneous
drainage have become the first-line treatment in most cases, greatly
improving patients’ prognosis, hence the decrease of the mortality
rate to 6.31% [3-5].
In this study, the feasibility of intravenous and intracatheterad-
ministration of Sonovue™
as an UCA in management of a complex
PLA is demonstrated, aiming to stress out Contrast Enhanced Ul-
trasound contribution and possibilities in ultrasound guided inter-
ventional procedures, as well as the benefits arising from the use
of UCAs contrary to B-mode imaging, for daily clinical practice,
in complex septic liver fluid collections needing percutaneous in-
tervention.
5. Case Report
An 84-year-old female with no prior relevant medical history
presented to the emergency department with symptoms of acute
cholangitis and sepsis. Initial laboratory results demonstrated in-
creased white blood cell (WBC) count (~14X103
) and deranged
values of hepatic enzymes and C-Reactive Protein (CRP). Imag-
ing investigation with unenhanced abdominal US, revealed a large
(~15X13 cm) hypoechoic micro cystic lesion in the right liver
lobe, dilated common bile duct and choledocholithiasis, indicative
of liver abscess due to ascending cholangitis. Abdominal CT and
MRI scans demonstrated a large multiloculated, peripherally en-
hancing lesion, with low density (on CT)/hyperintense (on DWI)
numerous central regions, involving multiple liver segments (IV, V,
VI), confirming the diagnosis of a large multiseptated liver abscess
(Figure 1).

Figure 4: Endocavitary CEUS; injection via the misplaced catheter. Note
the microbubble jet over the liver capsule to the peritoneal cavity (arrow).
Following a slight retraction, intracatheter Sonovue™
injection demonstrates
correct catheter sitting.
On the contrary to other contrast-enhanced modalities, CEUS is a
cost-effective, radiation-free and real-time examination and may
be performed bedside, proving portability as an important char-
acteristic for image-guided interventional procedures. Moreover,
modern second generation UCAs (such as Sonovue™
) have a non
nephrotoxic profile and their administration is rarely complicated
with allergic or severe adverse reactions [1, 11, 14, 16].
CEUS is a well proven imaging modality that improves the de-
tection and characterization of a liver abscess, with the presence
of rim enhancement (in arterial phase), septa enhancement (hon-
eycomb appearance), no enhancement in the cystic areas and ve-
nous hypoenhancement being the most common imaging features
of this certain pathology [10]. While hepatic abscesses require the
careful assessment of several aspects (number of lesions, size, con-
tent, extent of the abscess and presence of fistulas) CEUS improves
imaging quality, thus being very useful for interventional proce-
dure planning and process[9-11].
7. Conclusion
Pre-interventional intravenous CEUS is a useful tool, providing
substantial details in PLA imaging. Abscess cavities, as long as the
surrounding structures can be distinctly depicted, in order to plan
the ideal track towards the target sites [12, 14, 16, 17].
In concordance with current literature, endocavitary injection of
Sonovue™
in PLA has been an invaluable process in our case, delin-
eating the abscess cavities, confirming inter septal communication
and allowing the identification of correct positioning of drainage
catheters [15, 16, 18-21].
The utilization of both intravenous and endocavitary CEUS could
further assist interventional radiologists in management of com-
plex liver abscesses, increasing the success rate in drainage, whilst
lessening the complication rates and the patient-doctor radiation
induced.
