per million populations per year [1]. Here we have discussed a case of Toxic Epidermal Necrolysis secondary
to Sulfasalazine managed with fluid replacement, analgesics, anti-infective therapy aggressive nutritional
support and intravenous high dose steroid therapy.

Abstract—This study was aimed to find out the maternal factors and fetal outcomes associated with anemia in 3rd Trimester pregnancy. A hospital based cross sectional descriptive type of observational study was carried out in 15 to 49 years who had undergone delivery at SP Medical College Bikaner. Information about the demographic profile, ANC factors and foetal outcome data were collected. To find out associating factors appropriate test of significances were used. The magnitude of anemia 91.3% (995/1090) was found high in third trimester of pregnancy. Caste, dietary – habit, Education, Occupation, Socio-economic status, ANC Visit, Iron and folic acid supplementation were associated (P<0.05)>0.05) with anemia. Although IUGR, premature births and still births were observed respectively 2.56 times (95% CI: 0.615 to 10.697 ), 1.3 times (95% CI: 0.723 to 2.351 ) and 0.651 times (95% CI: 0.286 to 1.481 ) in anemic mothers but it was not found significant with anemia status. But significantly more (35.5%) low-birth weight babies were born to anemic mothers as compared to ( 14.7% )among non- anemic mothers i.e. 3.181 times (Odds ratio) higher LBW babies in anemic mothers. Anemia in pregnancy may be reduce by proper Iron and folic acid supplementation which can be improved through IEC and providing proper ANC services.

The present study was carried out to evaluate the occurrence of association between homocysteine, and vitamin B12 in patients with Preeclampsia , Eclampsia and those with history of previous PIH. 30 such patients from obstetric ward were studied for estimation of serum homocysteine, and vitamin B12 over a period of Jan10 to Jun 2011. Serum homocysteine and vitamin B12 were determined by means of Immulite 1000 analyzer. The statistical analysis of study group of preeclampsia compared with normotensive control group, showed significant alterations in serum homocysteine, and vitamin B12 concentrations in preeclampsia and eclampsia group. Inverse association between serum homocysteine and vitamin B12 levels were observed in preeclampsia and eclampsia. The present study found hyperhomocysteinemia and deficiency of vitamin B12 along with increased blood pressure as a risk factor in preeclampsia. Final outcome of these patients after Inj Vitamin B12 therapy has improved at par with control group without any neonatal or maternal mortality in all four groups
Intrauterine insemination versus fallopian tube sperm perfusion in non tubal ...Internet Medical Journal
Background: Controlled ovarian hyper stimulation (COH) combined with intrauterine insemination (IUI), using a volume of 0.5 mail of inseminate is commonly offered to couples with non tubal infertility. Another method is Fallopian tube sperm perfusion (FSP) which is based on a pressure injection of 4 ml of sperm suspension while attempting to seal the cervix to prevent semen reflux. This technique ensures the presence of higher sperm density in the fallopian tubes at the time of ovulation than standard IUI. The aim of this study was to compare the efficiency of IUI and FSP in the treatment of infertility.
Methods: 200 consecutive patients with infertility in 404 stimulated cycles were included in the study. Those randomized to standard IUI included 100 patients in 184 cycles [158 Clomiphene citrate/human menopausal gonadotrophin cycles and 26 Letrozole/FSH cycles exclusively for polycystic ovarian disease patients] (group A). Patients subjected to FSP included 100 patients in 220 cycles (193 Clomiphene citrate/human menopausal gonadotrophin cycles and 27 Letrozole/FSH cycles exclusively for polycystic ovarian disease patients] (group B). Swim up semen preparation technique was used in all cases. Insemination was performed in both groups 34-37 hours after hCG administration. Standard IUI was performed using 0.5 ml of inseminate. In FSP 4ml inseminate was used.
Results: In group A (184 IUI cycles in 100 patients), 22 clinical pregnancies (presence of gestational sac with fetal cardiac activity) occurred (11.95% per cycle over four cycles). In group B, (220 cycles of FSP in 100 patients), 48 clinical pregnancies occurred (21.81%per cycle over four cycles) and this difference was statistically significant (p<0.05).
Conclusions: For non-tubal sub fertility, the results indicate clear benefit for FSP (Fallopian tube sperm perfusion) over IUI (Intrauterine insemination).

RESULTS: Overall life span (LS) was 2252.1±1742.5 days and cumulative 5-year survival (5YS) reached 73.2%, 10 years – 64.8%, 20 years – 42.5%. 513 LCP lived more than 5 years (LS=3124.6±1525.6 days), 148 LCP – more than 10 years (LS=5054.4±1504.1 days).199 LCP died because of LC (LS=562.7±374.5 days). 5YS of LCP after bi/lobectomies was significantly superior in comparison with LCP after pneumonectomies (78.1% vs.63.7%, P=0.00001 by log-rank test). AT significantly improved 5YS (66.3% vs. 34.8%) (P=0.00000 by log-rank test) only for LCP with N1-2. Cox modeling displayed that 5YS of LCP significantly depended on: phase transition (PT) early-invasive LC in terms of synergetics, PT N0—N12, cell ratio factors (ratio between cancer cells- CC and blood cells subpopulations), G1-3, histology, glucose, AT, blood cell circuit, prothrombin index, heparin tolerance, recalcification time (P=0.000-0.038). Neural networks, genetic algorithm selection and bootstrap simulation revealed relationships between 5YS and PT early-invasive LC (rank=1), PT N0—N12 (rank=2), thrombocytes/CC (3), erythrocytes/CC (4), eosinophils/CC (5), healthy cells/CC (6), lymphocytes/CC (7), segmented neutrophils/CC (8), stick neutrophils/CC (9), monocytes/CC (10); leucocytes/CC (11). Correct prediction of 5YS was 100% by neural networks computing (area under ROC curve=1.0; error=0.0).
CONCLUSIONS: 5YS of LCP after radical procedures significantly depended on: 1) PT early-invasive cancer; 2) PT N0--N12; 3) cell ratio factors; 4) blood cell circuit; 5) biochemical factors; 6) hemostasis system; 7) AT; 8) LC characteristics; 9) LC cell dynamics; 10) surgery type: lobectomy/pneumonectomy; 11) anthropometric data. Optimal diagnosis and treatment strategies for LC are: 1) screening and early detection of LC; 2) availability of experienced thoracic surgeons because of complexity of radical procedures; 3) aggressive en block surgery and adequate lymph node dissection for completeness; 4) precise prediction; 5) adjuvant chemoimmunoradiotherapy for LCP with unfavorable prognosis.

The Ohio State University Comprehensive Cancer Center, will provide insight into new directions in targeted therapeutic approaches for older adults with mantle cell lymphoma.
STATEMENT OF NEED
Mantle cell lymphoma (MCL) is a rare, aggressive B-cell non-Hodgkin lymphoma (NHL) accounting for 5% to 7% of all lymphomas. Its prognosis ranges from indolent disease that does not require treatment for years to very aggressive disease, which is associated with poor survival (Silkenstedt et al, 2021). Typically, MCL is diagnosed at advanced stage and in older patients who cannot tolerate intensive therapy (NCCN, 2022). Although recent advances have slightly increased remission rates, recurrence and relapse remain very common, leading to a median overall survival between 3 and 6 years (LLS, 2021). Though there are several effective options, progress is still needed towards establishing an accepted frontline approach for MCL (Castellino et al, 2022). Treatment selection and management of MCL are complicated by the heterogeneity of prognosis, advanced age and comorbidities of patients, and lack of an established standard approach for treatment, making it vital that clinicians be familiar with the latest research and advances in this area. In this activity chaired by Michael Wang, MD, Professor in the Department of Lymphoma & Myeloma at MD Anderson Cancer Center, expert faculty will discuss prognostic factors informing treatment, the promising results of recent trials in new therapeutic approaches, and the implications of treatment resistance in therapeutic selection for MCL.

Em consonância com os ODS – Objetivos do Desenvolvimento Sustentável e a Estratégia Global para a Saúde das Mulheres, Crianças e Adolescentes, e aplicando uma abordagem baseada nos direitos humanos, os esforços de cuidados pós-natais devem expandir-se para além da cobertura e da simples sobrevivência, de modo a incluir cuidados de qualidade.
Estas diretrizes visam melhorar a qualidade dos cuidados pós-natais essenciais e de rotina prestados às mulheres e aos recém-nascidos, com o objetivo final de melhorar a saúde e o bem-estar materno e neonatal.
Uma “experiência pós-natal positiva” é um resultado importante para todas as mulheres que dão à luz e para os seus recém-nascidos, estabelecendo as bases para a melhoria da saúde e do bem-estar a curto e longo prazo. Uma experiência pós-natal positiva é definida como aquela em que as mulheres, pessoas que gestam, os recém-nascidos, os casais, os pais, os cuidadores e as famílias recebem informação consistente, garantia e apoio de profissionais de saúde motivados; e onde um sistema de saúde flexível e com recursos reconheça as necessidades das mulheres e dos bebês e respeite o seu contexto cultural.
Estas diretrizes consolidadas apresentam algumas recomendações novas e já bem fundamentadas sobre cuidados pós-natais de rotina para mulheres e neonatos que recebem cuidados no pós-parto em unidades de saúde ou na comunidade, independentemente dos recursos disponíveis.
É fornecido um conjunto abrangente de recomendações para cuidados durante o período puerperal, com ênfase nos cuidados essenciais que todas as mulheres e recém-nascidos devem receber, e com a devida atenção à qualidade dos cuidados; isto é, a entrega e a experiência do cuidado recebido. Ethics hit the forefront of medicine with the announcement of Richard Seed, PhD, a physicist from
Chicago, that he planned to clone human beings ( http://www.bmj.com/archive/7126/7126n1.htm). The
recent skiing deaths of Sonny Bono and Michael Kennedy renewed calls for the mandatory use of ski
helmets ( http://www.bmj.com/archive/7126/7126n12.htm). For inquiring minds (and bodies), a recent
survey found that Americans have sex on average once a week

Now for the pearls of the week...
Polyunsaturated fats seem to increase the risk of breast cancer, whereas monounsaturated fats
actually decrease the risk. Monounsaturated fats are found in olive and canola oils. Arch Intern Med

Breast feeding should be delayed for at least one hour after vigorous exercise; immediately after
exercise the antibodies in breast milk fall to low levels, but return to normal within an hour. Med
Sci Sports Exercise 1997 029:1596.
Dextromethorphan was found to cause birth defects in an animal study; it is recommended that
pregnant women not take this popular over-the-counter cough medicine. Ped Research 1998 043:0001.
The use of quilts as bedding material for infants under age 1 yr old was associated with an
increase in the rate of Sudden Infant Death (SIDS). BMJ 1998 316:0125.
In a study of 5,601 patients scheduled for elective surgery, of whom all received a second opinion,
it was determined that 9% of the patients did not need the surgery. Implication- a second opinion
may help you avoid unnecessary surgery. J Am Coll Surgeons 1997 185:0451.
Physicians supporting the use of calcium channel blockers were more likely to have financial
relationships with the pharmaceutical companies making calcium channel blockers than those
physicians who supported a neutral or negative view. NEJM 1998 338:0101.
The regular (at least weekly) use of acetaminophen was associated with a decreased risk for
ovarian cancer in this study of 1086 women over a 5 year period. Lancet 1998 351:0104.
Medical information on the internet is often faulty. In this study, 41 sites were found discussing
the management of fever in children, and of those, only 4 sites closely adhered to standard

Supplementation with 27 grams of rhubarb fiber a day lowered total cholesterol by 8% and LDL
cholesterol by 9% in this study of 10 men. J Am Coll Nutr 1997 016:0600.
The Chinese herbal treatment for menopausal symptoms, Dong Quai, is no better than placebo.
It reduced symptoms by 25 - 30 %, comparable to placebo. In contrast, treatment with estrogen

Green tea contains a powerful antioxidant that kills human cancer cells in-vitro. The ingredient is

2. Drinking alcohol outside of mealtimes increased the risk of liver cirrhosis by 5 times; those
drinking multiple types of alcohol (corrected for total intake of EtOH) had 23 times the risk for
cirrhosis. Gut 1997 041:0845.
Children who own and wear cigarette promotional items such as T-shirts, hats, or jackets were 4
times more likely to smoke than those who don't own such items. Youngsters with friends and/or

Amiodarone, taken for one week before heart surgery, can decrease the post operative rate of

The symptoms of fever, joint pain, and night sweats help identify people with recent HIV

Postmenopausal hormone replacement therapy protected against age related memory loss in this

The use of an electric blanket early in pregnancy increased the risk of miscarriage in this study of

Among male smokers, those who drank chlorinated water versus those who drank
nonchlorinated water, for 40 or more years, where more likely to develop bladder and rectal

Physician behaviors likely to result in fewer lawsuits: longer visits, asking pts' opinions, checking
understanding, using connectors ("go on, tell me more"), humor, and orientation (e.g. "here's

People taking long half-life benzodiazepines had an approximately 45% increased risk of having
a motor vehicle accident; those taking short half-life benzodiazepines did not have an elevated

A low-fat, high carbohydrate diet was more effective than a fixed energy maintenance diet in
keeping the weight off after dieting; energy intake from fat was 20% to 25% and from

Vitamin E at 2,000 IU per day given to people with Alzheimer's disease delayed progression of the

Inhaled ipratropium bromide (ATROVENT) has been shown to help resolve persistent postviral

Heparin therapy along with aspirin is more effective in the treatment of unstable angina if it is
discontinued prior to 48 hours as compared with a similar group of patients treated for longer

In this study of 3,187 women aged 25 to 44, the risk of a delayed conception (waiting 9 1/2 months
