The IFPAC Session: Controlling excipient impact during the product lifecycle.
Excipients enable the delivery of actives as a pharmaceutical product. Quality by Design requires that the impact of excipient variability on finished product quality be minimized, or, as paraphrased by Tobyn: - What matters doesn’t vary, and what varies doesn’t matter.
This parallels the current practice of categorizing excipients into critical vs non-critical, the assumption being that the latter do not impact the finished product Critical Quality Attributes. This binary classification of criticality has been criticized as too simple and it is not uncommon to observe excursions in finished product quality correlating with variability of a so-called non-critical excipient. The complexity of the excipients, and the products into which they are formulated, contributes to this uncertainty. For excipients, what varies may not have mattered prior to approval, but may come to matter later in the product lifecycle, especially for continuously manufactured products with real time release.
Excipients, even if fully compliant and manufactured under GMP, represent a reservoir of special cause variability in finished product quality. By definition this can only be addressed via the Control Strategy. Risk management requires continuous multivariate monitoring of finished product and raw materials to maintain quality and model fidelity.

Lies, Damned Lies and Cost-Effectiveness: Open-Source Models
The issue of open-source models in the cost-effectiveness and disease-level (collaborative) models has been brewing for many years. There has been a marked growth in open science, and funding bodies and publishers increasingly require that research data be made available. As mentioned in our previous Issue Panel, “cost-effectiveness models synthesise a wide range of evidence to facilitate extrapolation over time and from intermediate to final decision endpoints. These models are often statistically sophisticated and require assumptions that are not directly testable. This can lead to decision-makers “discounting” the results of cost-effectiveness analyses, particularly if the developer is seen as partial.” Open-source models, then, would encourage greater transparency in pharmacoeconomic modeling and the reuse and updating of the best/most useful models; they are essential if cost-effectiveness analyses are to be widely accepted to reduce bias, increase transparency, improve model access, and allow for faster access to critical knowledge. The ISPOR-SMDM guidelines and the EUnetHTA joint action projects, are supportive of these views on collaboration, transparency, confidentiality, processes and consistency offered by the availability of open-source models to improve decision-making around health care and reimbursement. With openness and sharing, however, come issues of copyright and access and a need to define how model sharing can be achieved in a fair and equitable manner. There is, therefore, a need to develop an ongoing dialog on openness, especially where the research may be considered precompetitive and not worthy of IP investment. The pros and cons of open source models and the proposed mission of the Open Source Model SIG to curate an ongoing dialog regarding issues around creating, disseminating, sharing, evaluating, and updating open source cost-effectiveness and comparative effectiveness models will be debated amongst SIG members.

The diabetes prevention program is great for individuals who are struggling with being overweight or obese and who are prediabetic. The program is designed to help prevent type 2 diabetes. They will work with a trained professional for one year to better their lifestyle. They will focus on learning to eat a healthier diet and incorporate exercise into their daily routine. An alternative strategy I suggest for this program, is to shorten it by 6 months. The purpose of the program is not only to prevent diabetes in overweight adults, but to change their lifestyle. I think 6 months is an adequate time to do that. I think one year may be too long and as a result, individuals will give up or not be completely devoted to the program. I also think that group sessions would increase the success rate in this program. I believe that exercising or eating healthy with another person, motivates that person even more. I think that group exercises and support groups could motivate people more. With any weight loss or dietary program, comes fear of losing participants. It will be difficult for people to understand that this program is not just a fad diet, but a new lifestyle. Not monitoring the success of the participants could lead to them not following the program. The participants should be checking in with their trained professional every week to talk about the progress they have made or to improve certain aspects of the program, such as diet. I think a possible problem that I would encounter when implementing my suggestion would be the group sessions and activities. It certainly helps people to be exercising and eating healthy with others, but for some it may be uncomfortable. Seeing that the program includes group classes may steer people away from it. For the people who feel uncomfortable with exercising or talking in front of others, one on one sessions can be added. This will ensure the participant that their information is confidential and will only be displayed to the trained professional helping them.

The public health issue I have chosen for my project is diabetes and the program I have selected is the National Diabetes Educational Program (NDEP). This program is very detailed and covers a lot of grounds. Most importantly, it offers a lot of resources such as personnel, booklets, CDs, workshops, etc. However, an important component to the treatment and prevention of diabetes is diet. Research shows that by increasing the intake of vegetables and fruits, an individual decreases the risk of developing type 2 diabetes and it can help with the treatment of type 2 diabetes (Bryce et al., 2017). In order to bring the program down to the local level, the alternative strategy I suggested is a partnership with local farmer’s markets so that health care providers can “prescribe” fruits and vegetables. Abstract: Although a vast number of mobile health interventions has been discussed in the scientific literature, the scientific evidence for engineering gamified health interventions is too meager. Depending on the context, this leads to immature investments on one side of the spectrum and delayed implementations on the other side. GameBus is a web platform for increasing the rigor of gamified health experiments in living lab settings. This article clarifies the scientific design rationale of this platform and provides an eight-year evaluation of its performance. We demonstrate that a large variety of studies has been performed, leading to a promising data set for analyzing the impact of gamification techniques in the context of health promotion. While various articles on such studies have been published, along with data sets, this article shares novel information to enable other scholars to scale up such efforts. Lies, Damned Lies and Cost-Effectiveness: Open-Source Models
The issue of open-source models in the cost-effectiveness and disease-level (collaborative) models has been brewing for many years. There has been a marked growth in open science, and funding bodies and publishers increasingly require that research data be made available. As mentioned in our previous Issue Panel, “cost-effectiveness models synthesise a wide range of evidence to facilitate extrapolation over time and from intermediate to final decision endpoints. These models are often statistically sophisticated and require assumptions that are not directly testable. This can lead to decision-makers “discounting” the results of cost-effectiveness analyses, particularly if the developer is seen as partial.” Open-source models, then, would encourage greater transparency in pharmacoeconomic modeling and the reuse and updating of the best/most useful models; they are essential if cost-effectiveness analyses are to be widely accepted to reduce bias, increase transparency, improve model access, and allow for faster access to critical knowledge. The ISPOR-SMDM guidelines and the EUnetHTA joint action projects, are supportive of these views on collaboration, transparency, confidentiality, processes and consistency offered by the availability of open-source models to improve decision-making around health care and reimbursement. With openness and sharing, however, come issues of copyright and access and a need to define how model sharing can be achieved in a fair and equitable manner. There is, therefore, a need to develop an ongoing dialog on openness, especially where the research may be considered precompetitive and not worthy of IP investment. The pros and cons of open source models and the proposed mission of the Open Source Model SIG to curate an ongoing dialog regarding issues around creating, disseminating, sharing, evaluating, and updating open source cost-effectiveness and comparative effectiveness models will be debated amongst SIG members.

There are various issues and opportunities which have been identified in the SWOT analysis which was carried out on the new product to be developed by the company. One of the issues which will be facing the coca cola's new product is the fact that there is usually weak coordination between government agencies. The company will have to follow the government directives, and it will have to obtain the necessary licensing. The diabetes prevention program is great for individuals who are struggling with being overweight or obese and who are prediabetic. The program is designed to help prevent type 2 diabetes. They will work with a trained professional for one year to better their lifestyle. They will focus on learning to eat a healthier diet and incorporate exercise into their daily routine. An alternative strategy I suggest for this program, is to shorten it by 6 months. The purpose of the program is not only to prevent diabetes in overweight adults, but to change their lifestyle. I think 6 months is an adequate time to do that. I think one year may be too long and as a result, individuals will give up or not be completely devoted to the program. I also think that group sessions would increase the success rate in this program. I believe that exercising or eating healthy with another person, motivates that person even more. I think that group exercises and support groups could motivate people more. With any weight loss or dietary program, comes fear of losing participants. It will be difficult for people to understand that this program is not just a fad diet, but a new lifestyle. Not monitoring the success of the participants could lead to them not following the program. The participants should be checking in with their trained professional every week to talk about the progress they have made or to improve certain aspects of the program, such as diet. I think a possible problem that I would encounter when implementing my suggestion would be the group sessions and activities. It certainly helps people to be exercising and eating healthy with others, but for some it may be uncomfortable. Seeing that the program includes group classes may steer people away from it. For the people who feel uncomfortable with exercising or talking in front of others, one on one sessions can be added. This will ensure the participant that their information is confidential and will only be displayed to the trained professional helping them.

The public health issue I have chosen for my project is diabetes and the program I have selected is the National Diabetes Educational Program (NDEP). This program is very detailed and covers a lot of grounds. Most importantly, it offers a lot of resources such as personnel, booklets, CDs, workshops, etc. However, an important component to the treatment and prevention of diabetes is diet. Research shows that by increasing the intake of vegetables and fruits, an individual decreases the risk of developing type 2 diabetes and it can help with the treatment of type 2 diabetes (Bryce et al., 2017). In order to bring the program down to the local level, the alternative strategy I suggested is a partnership with local farmer’s markets so that health care providers can “prescribe” fruits and vegetables. Abstract: Although a vast number of mobile health interventions has been discussed in the scientific literature, the scientific evidence for engineering gamified health interventions is too meager. Depending on the context, this leads to immature investments on one side of the spectrum and delayed implementations on the other side. GameBus is a web platform for increasing the rigor of gamified health experiments in living lab settings. This article clarifies the scientific design rationale of this platform and provides an eight-year evaluation of its performance. We demonstrate that a large variety of studies has been performed, leading to a promising data set for analyzing the impact of gamification techniques in the context of health promotion. While various articles on such studies have been published, along with data sets, this article shares novel information to enable other scholars to scale up such efforts. We also highlight critical gaps to guide future work on the platform.

We know we want to create products which our customers find to be valuable. Whether we label it as customer-centric or product-led depends on how long we've been doing product management. There are three challenges we face when doing this. The obvious challenge is figuring out what our users need; the non-obvious challenges are in creating a shared understanding of those needs and in sensing if what we're doing is meeting those needs.
In this webinar, we won't focus on the research methods for discovering user-needs. We will focus on synthesis of the needs we discover, communication and alignment tools, and how we operationalize addressing those needs.
