General observation
•Diagnosis and reason of referral are mixed up
•Majority of referrals ended up with Vaginal deliveries
•Referrals are made by Midwives, followed by no cadre indicated
•Majority of referrals could be prevented if portogram were used, and
referrals made by most senior cadre
