This document summarizes a medical application with 6 windows forms: a login form, patient info form, appointment form, patient record form, doctor info form, and doctor record form. The login form allows users to enter a username and password, and will display an error message for incorrect credentials. The menu strip allows navigation between the different forms. The appointment form saves patient and appointment details like date and time. The patient and doctor forms allow adding, updating, and deleting patient and doctor information stored in the database, including images.
