The purpose of this form is to explain to you a summary of services/set proper expectations . My knowledge and
experience with Energy Healing has shown that each of us has the potential to become his or her own Healer.
HolisticDNA Energy Healing occurs by making a connection between the subconscious mind and the source of all
Energy – Infinite Intelligence. I assist you in your Healing by helping you make the needed connection between
these 2 super powers, request the Energy needed, and this will “trigger” the Energy Healing “Event”. The techniques
that are used to achieve HolisticDNA Energy Healing can be executed in person / private session, on the phone, or
by Skype. I will thoroughly assess your needs, plan a course of action, pinpoint where energy is needed to be
channeled for Healing, and help you initiate the process from start to finish.. There are no miracles, but with that
said, results can be nothing short of “miraculous”. In some situations, Energy Healing can even help when modern
medicine fails. (as in my case) Should a Healing Event not occur during your session, you will have the knowledge to
pursue this option on your own, with no limit on how often you use this procedure to gain successful mind/body
Healing results, and with no additional cost to you. You will experience HolisticDNA Energy Healing - either with
my help during a live session, or on your own with the knowledge that you gain. We may discuss things like the
major stresses in your life, your belief systems, health history, your childhood experiences, and other issues that
have impacted your emotional and physical well-being, and prompted your seeking of Holistic options.

I am not a physician and therefore do not diagnose disease or prescribe drugs. I am a Healing Science Practitioner
utilizing Energy Healing thru activation of the sixth sense, which we all have the ability to use, once instructed properly.
At all times your healing is your responsibility. I am available to be your partner in this process, your guide,
committed listener, and your mentor. I do not advise you to discontinue any medical treatment you may be receiving.
My work is intended to be in harmony with any other healing work that you undertake, including traditional
medicine. Please feel free to discuss our work with your doctor or any other trusted Health Professional.
Cancellation Policy:
I prefer to set up a regular schedule to work with you but there is never an obligation to continue treatment. Our first
session is 1 hour and follow-up treatments are generally one hour in duration as well. Because of the nature of my work
sometimes sessions are longer. If this occurs the treatment fee is limited to the 1 hour session cost. If you cancel an
appointment please give as much notice as possible. I ask for full payment for the session if you cancel within 24 hours
from the session time. If you need to cancel over a weekend, I ask for 48 hours notice of cancellation.
Payment:
When doing in-person treatments in my office, payment can be made either by cash, money order or check – payable at
the time of treatment. Sessions are $125 per hour. For long distance work via the telephone or Skype, payment is to be
made prior to service via money order, PayPal, or Bank wire. By signing the attached Acknowledgment and Release
form you agree that I may work with you in the above-described manner. Many of my clients experience increased well-
being and improvement in their condition. There are no refunds with HolisticDNA Energy Healing Sessions, as it requires
your efforts as well as my expertise. I am not aware of any risks or negative side effects associated with the HolisticDNA
Energy Healing / sessions.
