Risk is an inherent aspect that is found in each organization including the health care facilities. There is need to ensure that risk is managed in the health care facilities to facilitates them in achieving ethical and legal responsibilities that are stipulated by the organization and other health or safety organization. Summary of risk management plan
Employee Training and Development Programs in Risk Management is the type plan that I selected within a setting of a health care facility. In each organization there are some changes and development or growth that are incorporated which are directly facilitated by effective training and development programs to be effective. Employee Training and Development Programs is one of the fundamental programs that is mostly utilized by health organization to manage their risk effectively. This program is more focused to the employee and change within the organization. New employees are also subjected to training and development to properly inaugurate them to the respective roles in the health facility. The employee in general are the specific audience that the program aims to enlighten in the process of risk management (Anderson & Mapp, 1996). Having an electronic pressure ulcer risk and incidence form for each patient allows various healthcare professionals and members of the interdisciplinary team to have secure access to reliable and current information in real-time (Plaskitt, Heywood, and Arrowsmith, 2015).
Develop a plan for implementing the proposed solution
Per Wager et al (2009), it is crucial that a team is organized that serves “to plan, coordinate, budget, and manage all aspects of the new system implementation” (p. 244). A team will be assembled to gain much-needed support for the program. This implementation team is vital in engaging various stakeholders in providing support and commitment to the project. The team members include charge nurses, wound consultant nurses, quality improvement nurses, an MD champion and department managers from areas such as education, equipment and information technology (IT) and administration.
An immersion event will be launched to inform and engage all staff members in the project. The event will allow necessary information to be disseminated, share goals and desired outcomes as well as the rationale behind the project.
Shedenhelm et al (2010), states that providing education through a variety of methods allows training to be received well for recipients with diverse learning styles. Furthermore, ongoing training should be developed and advertised through educational newsletters and emails that provide education reminders and other important information. Shedenhelm et al (2010), also emphasizes provision of multiple opportunities through multiple sites locations at varied times increases turnout. Furthermore, each nursing units will be provided pressure ulcer training bundles that including competencies will be presented.

In 1996, after releasing America's Health in Transition: Protecting and Improving Quality, the IOM launched a concerted, ongoing effort focused on assessing and improving the nation's quality of care.
The first phase of this Quality Initiative documented the serious and pervasive nature of the nation's overall quality problem, concluding that "the burden of harm conveyed by the collective impact of all of our health care quality problems is staggering" (Chassen et al., 1998).

The degree to which health services for individuals and populations increase the likelihood of desired health outcomes and are consistent with current professional knowledge.
This phase built on an intensive review of the literature conducted by RAND to understand the scope of this issue (Schuster) and a framework was established that defined the nature of the problem as one of overuse, misuse and underuse of health care services (Chassen et al). More specifically, the report Ensuring Quality Cancer Care (1999) documented the wide gulf that exists between ideal cancer care and the reality many Americans with cancer experience.
During the second phase, spanning 1999-2001, the Committee on Quality of Health Care in America, laid out a vision for how the health care system and related policy environment must be radically transformed in order to close the chasm between what we know to be good quality care and what actually exists in practice. The reports released during this phase—To Err is Human: Building a Safer Health System(1999) and Crossing the Quality Chasm: A New Health System for the 21st Century(2001)—stress that reform around the margins is inadequate to address system ills.
The series of IOM quality reports have included a number of metrics that illustrate how wide the quality chasm is and how important it is to close this gulf, between what we know is good quality care and what the norm is in practice.
To Err is Human put the spotlight on how tens of thousands of Americans die each year from medical errors and effectively put the issue of patient safety and quality on the radar screen of public and private policymakers. Yes, defensive medicine is practiced in the United States. However, it is important to
understand: (1) what impact it has on healthcare expenditures (2) to what degree does it occur
(prevalence) and (3) if so, what can be done to prevent it?
As an emergency services administrator for a Level 1 trauma center, experience has led
me to understand the dynamic influencing physicians in their clinical decision making process.
Ideally, this process should be void of non-clinical bias or influence. However, this is not the
case in many circumstances. Physicians are considering risk and liability when ordering tests
and procedures. This risk management, or risk mis-management, phenomenon is called
defensive medicine. By definition, these occurrences are medical practices intended to
exonerate practitioners from liability with limited or without medical benefit to the patient
(Sethi et al, 2012). Physicians have been directed by health policy to provide value based care,
but defensive medicine practice works against this care model.
There have been studies conducted measuring physician attitudes towards tort reform
and defensive medicine practices. While studies show physicians, especially high risk medical
specialists, regularly practicing defensive medicine, the cost implications are unclear.
Furthermore, proposed reforms to the medical tort system must be investigated. Some have
proposed to completely do away with the medical tort litigation and insurance system,
replacing it with a system similar to workman’s compensation models. While it may be a reflex
mechanism to use cost as a metric to measure results of defensive medicine practices, patient
outcomes and quality of life implications must also be measured. The patient is the one who is
being subjected to additional and unwarranted procedures.

1214 respondents, 96% admitted to have practiced defensive medicine by ordering labs,

1. Summary Description
Risk management is one of the most appropriate tools in scientific governance and approaches necessity to perform comprehensive analyses with the intention to uncover causes of adverse events (Cagliano, A. C., Grimaldi. S., Rafele C. 2011). According to Kavaler & Spiegel (2003), health risk management includes the assessment of risks and the definition of mitigation strategies for these risks. The risk assessment includes both the identification and evaluation of the risk`s impact while risk mitigation minimizes the impact of the risks. Therefore, a healthcare organization should have qualified risk managers for the assessment, development, implementation, and monitoring of risk management plans. My risk management program will focus on new employees. This will seek to foster the utilization of risk assessment and risk management tools in new fields of application including policy making and provision of a platform between the insurance communities.
1. Risk management plan to the new employees
The process of risk management in a healthcare organization basically has five steps which include the context establishment, risks identification, risks analysis, and risk evaluation and management (Kavaler, F., & Spiegel, A. D. (2003). Since the healthcare industry is an essential sector that impact every living individual, the offered services in this industry is very significant to the world`s health market competition. Therefore, for a successful risk management program, a person should be aware and knowledgeable of the risk involved.
New employee will be required to go through the organization structure and understand leader within the organization, the different position function in the risk management program and how-to communication and escalation policies. They will gain knowledge of risk exposure and the available polices that are in place to reduce them. All these are prerequisite to new workers in the understanding of the healthcare organization`s risk management plan.
Secondly, the risk management program will be explained to the new workers regarding program analysis and what steps the organization has in place for identification, management, mitigation and transfer of risks.
This program of risk management will entail four categories. These include total avoidance of risks by the organization which can be implemented by putting policies that are geared towards preventing the chances of losses. Secondly risk mitigation which will aim at reducing the impact of the risks of the organization. This will be done by setting out a plan on how to deal with the risk in case it occurs. Emergency service providers and agencies alike participate in life-saving activities every day. Workers in these agencies provide assistance in daily emergency situations, transporting and treating patients in various situations. Their input in the medical process is incontrovertible as they ensure the welfare of the patient.
The healthcare sector is exposed to varying infections despite the availability of multiple disinfection procedures. Contamination remains a significant issue to the EMS responders as ambulances become a constant source of various pathogenic bacteria as a result of transporting patients from emergency scenes to the medical facilities. The transportation procedure creates a situation where the paramedics as well as the patient are exposed to the pathogens, a majority of which may be infectious (Reed, et al., 1993). Although there are various safeguards and disposable equipment which reduce the risk to the medical staff, the transportation process remains susceptible to contamination from blood and infectious substance. The result of these infections on the EMS in Saudi Arabia specifically is significant, as estimated 35 to 45 medical providers have been infected .
The ambulance service managed by the Red Crescent is the source of focus for this project for its lack of policy and guidelines on the pre-hospital process. Precautions such as disinfection and sterilization are not accompanied by corresponding infection control procedures. Currently in the Kingdom, there are over 5, 000 EMS staff using approximately 1,400 ambulances, and they are all at risk (Reed, et al., 1993). To ensure effective control of possible infections, a significantly holistic approach is required; one which is evidence-based and also cost-effective. Description of the Problem
The requirement for disaster medicine is more crucial than ever before with diseases such as MERS affecting areas deemed disaster-prone. Medical workers and emergency service providers at large require the evidence as a stepping stone to new decisions and measures involving medical care for health workers. EMS in Saudi Arabia has faced its fair share of challenges, with a number of its employees contracting the deadly virus MERS. EMS plans to use this information in creating effective influenza plans which will enhance its exis.

Yes, defensive medicine is practiced in the United States. However, it is important to
understand: (1) what impact it has on healthcare expenditures (2) to what degree does it occur
(prevalence) and (3) if so, what can be done to prevent it?
As an emergency services administrator for a Level 1 trauma center, experience has led
me to understand the dynamic influencing physicians in their clinical decision making process.
Ideally, this process should be void of non-clinical bias or influence. However, this is not the
case in many circumstances. Physicians are considering risk and liability when ordering tests
and procedures. This risk management, or risk mis-management, phenomenon is called
defensive medicine. By definition, these occurrences are medical practices intended to
exonerate practitioners from liability with limited or without medical benefit to the patient
(Sethi et al, 2012). Physicians have been directed by health policy to provide value based care,
but defensive medicine practice works against this care model.
There have been studies conducted measuring physician attitudes towards tort reform
and defensive medicine practices. While studies show physicians, especially high risk medical
specialists, regularly practicing defensive medicine, the cost implications are unclear.
Furthermore, proposed reforms to the medical tort system must be investigated. Some have
proposed to completely do away with the medical tort litigation and insurance system,
replacing it with a system similar to workman’s compensation models. While it may be a reflex
mechanism to use cost as a metric to measure results of defensive medicine practices, patient
outcomes and quality of life implications must also be measured. The patient is the one who is
being subjected to additional and unwarranted procedures.

1. Summary Description
Risk management is one of the most appropriate tools in scientific governance and approaches necessity to perform comprehensive analyses with the intention to uncover causes of adverse events (Cagliano, A. C., Grimaldi. S., Rafele C. 2011). According to Kavaler & Spiegel (2003), health risk management includes the assessment of risks and the definition of mitigation strategies for these risks. The risk assessment includes both the identification and evaluation of the risk`s impact while risk mitigation minimizes the impact of the risks. Therefore, a healthcare organization should have qualified risk managers for the assessment, development, implementation, and monitoring of risk management plans. My risk management program will focus on new employees. This will seek to foster the utilization of risk assessment and risk management tools in new fields of application including policy making and provision of a platform between the insurance communities.
1. Risk management plan to the new employees
The process of risk management in a healthcare organization basically has five steps which include the context establishment, risks identification, risks analysis, and risk evaluation and management (Kavaler, F., & Spiegel, A. D. (2003). Since the healthcare industry is an essential sector that impact every living individual, the offered services in this industry is very significant to the world`s health market competition. Therefore, for a successful risk management program, a person should be aware and knowledgeable of the risk involved.
New employee will be required to go through the organization structure and understand leader within the organization, the different position function in the risk management program and how-to communication and escalation policies. They will gain knowledge of risk exposure and the available polices that are in place to reduce them. All these are prerequisite to new workers in the understanding of the healthcare organization`s risk management plan.
Secondly, the risk management program will be explained to the new workers regarding program analysis and what steps the organization has in place for identification, management, mitigation and transfer of risks.
This program of risk management will entail four categories. These include total avoidance of risks by the organization which can be implemented by putting policies that are geared towards preventing the chances of losses. Secondly risk mitigation which will aim at reducing the impact of the risks of the organization. This will be done by setting out a plan on how to deal with the risk in case it occurs. Thirdly risk acceptance will aim at the effect that the organization will incur through insurance and policies for budgeting. Last.

The result of these infections on the EMS in Saudi Arabia specifically is significant, as estimated 35 to 45 medical providers have been infected .
The ambulance service managed by the Red Crescent is the source of focus for this project for its lack of policy and guidelines on the pre-hospital process. Precautions such as disinfection and sterilization are not accompanied by corresponding infection control procedures. Currently in the Kingdom, there are over 5, 000 EMS staff using approximately 1,400 ambulances, and they are all at risk (Reed, et al., 1993). We will highlight the advantages of working with these technologies beyond the realm of teaching, learning and assessment by considering prompt engineering skills, industry impact, curriculum changes, and the need for staff upskilling. In contrast, not engaging strategically with Generative AI poses risks, including falling behind peers, missed opportunities and failing to ensure our graduates remain employable. However, hospitals also face risks which challenge the way in which they assist patients in recovering as expected (McAlister, Stewart, Ferrua & McMurray, 2004). Some risks have more impacts while others have fewer results in hospitals. However, there is the need to implement risk management plans that will take care of the different types of risks in the hospitals. - 2. the hospital The risk management plan addresses the issue of poor communication in a hospital and some of the impacts which it will have on patients and the management. The program identifies some of the risks and the consequences which affect patients as they receive medical attention. Ineffective communication makes it hard for doctors and nurses to participate well when providing medical care to the patients (Kasper et al. 2002). Patients are at the receiving end since they suffer from instances of wrong medication and delayed services. The hospital also is at risk of poor performance as teamwork may not work for the care providers. The example provides a clear picture of how one challenge might cause risks in healthcare facilities. The probability of the risks indicates the number of times when the risks might occur. The impact also identifies the extent to which it affects the patients. The priority, on the other hand, indicates the amount of attention that should be given to each of the risks.Question TwoThe implementation of the risk plan will require the administration to take some steps that will make the activity successful. The following steps will assist in identification of the best risk management plan in the healthcare facilities;Risk identificationThe administration should state the risk affecting operations through some failures taking place in the center. This will only take place where the hospital recognizes mistakes and failures arising from some of the working methods and styles applied by the care providers. Uncovering and understanding the risk will make it possible to describe its impact (Capomolla et al. 2002). On the other hand, it will make it easy to identify the impacts which it might have on the outcomes of various operations in the hospital.Risk analysisThe step will include determining the likelihood of occurrence of the risk and the consequences which it might have on the operations of the healthcare (McAlister, Stewart, Ferrua & McMurray, 2004). The step will help the administration in understanding the nature of the risk and the way in which it will affect the journey to achieving goals and objectives of the healthcare facility. The information acquired during this stage should be put in the Project Risk Register.Evaluation and ranking riskThe stage will involve ranking the risks based on the impact that each will have on organizational operations. The administration will have to identify the likelihood and the consequences of the risks (Kasper et al. 2002). The stage will also recognize some of the acceptable risks and those that need immediate attention.Treating the riskThe step will involve all activities related to planning for ways in which mitigating the threat will take place. The set of plans that will assist in eliminating the risk will assist in dealing with the challenges which affect the hospital (Capomolla et al. 2002). The preventive strategies and the contingency plan will be developed at this stage.Monitoring and reviewing the riskThe step will include taking the Project Risk Register and using it to track, monitor and evaluate the risks.Question ThreeAdministration of safe health is a requirement in all hospitals. The agencies liable for the activity include;The Department of Health and Human Services Office for Civil Rights- the body is responsible for enforcement of HIPAA. In this case, the body audits the healthcare providers as well as their business associates (McAlister, Stewart, Ferrua & McMurray, 2004). The body also handles fines for noncompliance with the regulations.The Centers for Medicare and Medicaid Services, (CMS)- The body ensures that there is compliance of resources and regulators for meaningful use of the Medicare and Medicaid programs in their facilities (Kasper et al. 2002).The Office of the National Coordinator for

Health IT- The health care resources and regulators should comply with the requirements of the healthcare programs which should benefit all people in the society.The Food and Drug Administration- The agency helps in managing the IT applied in the provision of healthcare services (McAlister, Stewart, Ferrua & McMurray, 2004). The FDA, therefore, evaluates the medical devices and classifies them according to the level of risks which they could expose their users.Question FourThe selected risk management plan complies with the requirements American Society of Healthcare Risk Management standards. For instance, it contains the various matrices that define the components of the risk management plan. The priorities and the impacts as well as the probabilities meet the standards of ASHRM and ensure patient safety (Capomolla et al. 2002). Also, it calls for the cooperation of the various stakeholders so that they can work together to ensure that the facility provides a healthy working environment for all. The compliance with the standards also makes it clear that the hospital has its management determined to provide quality products and services to the patient.Question FiveUsing the above plan, I would make changes related to the probability of the occurrence of the different risks based on the extent to which they affect patients in the hospitals. I would raise the chances to show the seriousness of the risks and how they have a negative impact on patients (Kasper et al. 2002). This will ensure that the management becomes concerned and pays the right amount of attention to avoid them from happening. I would also add the number of mitigation methods to give a variety which will fit the various hospitals where one method does not work.
