The global epidemic of type 2 diabetes (T2D) and its co-morbidities threatens to overwhelm public health services and urgent patient intervention is necessary. A review of mainly randomised controlled trials investigating the reduction of biochemical T2D risk markers through fasting or caloric restriction (CR) found that in T2D or where baseline fasting glucose or HbA1c were elevated, there were significant improvements in fasting glucose and HbA1c, while fasting insulin and insulin resistance may show improvement regardless of condition or baseline levels. There may, however, be ethnic differences, with a clear positive correlation found only in Caucasians. Intermittent CR (i.e. non-continuous periods of fasting) is at least as effective as isocaloric continuous CR, while CR of 400-800 kcal/day is possibly more effective than higher levels for reducing fasting glucose and HbA1c. Time restricted feeding also shows promise but there are few human studies. The findings suggest that the optimum regimen to reduce biochemical risk markers for T2D is an intermittent fasting programme employing a very low-calorie diet with the longest possible number of consecutive days of fasting. The addition of liquid meal replacements, low carbohydrate CR and supplementation of vitamin D, ω-3 PUFAs and L-carnitine may also be of benefit.

Gluten has become a controversial topic for those with Autism Spectrum Disorder (ASD) as some research has shown improvements in symptoms after eliminating gluten from the diet. Breastfeeding and time of gluten introduction did not prevent celiac disease, as all subjects eventually developed the disease. Vaginal delivery is supported as helping develop gut microbiome which could provide defense against celiac disease. Probiotic supplementation, specifically Bifidobacterium strains, provides some defense against celiac disease by reducing inflammatory cytokines, but more research is needed to determine effective probiotic strains and role of gut microbiota in celiac disease pathogenesis.

Abstract
Title. Lived experience of diabetes among older, rural people.
Aim. This paper is a report of a study conducted to elucidate experiences and
perceptions of self-management of diabetes as narrated by older people diagnosed
with insulin-dependent diabetes living in a rural area.
Background. Older people worldwide are disproportionately affected by diabetes
and are more likely to have co-morbidities and disabilities. Guidelines for
management, developed by the American Diabetes Association, are not targeted for
this population. A plethora of quantitative research has investigated self-manage-
ment issues, with little change to outcomes. This pleads for consideration of a new
diabetes education model, which includes consideration of experiences within
clients’ worldviews.
Method. Unstructured interviews starting with an open question were conducted
from a purposive sample in 2005. Interviews were transcribed and analysed
according to the tenets of existential phenomenology, a process which began with
bracketing the researcher’s biases.
Findings. Living with poorly controlled diabetes led participants to introspection
and existential questioning. Four connected themes were identified: ‘Your Body Will
Let You Know’; ‘I Thought I Was Fine, But I Wasn’t’; ‘The Only Way Out is to Die’;
and ‘You Just Go On’.
Conclusion. Currently designed from a medical perspective, diabetes education
should be based on a nursing model incorporating the client’s insights and experi-
ences. When managing diabetes is viewed from a client’s perspective, the focus
becomes solving problems that arise in self-regulation of one’s own regimen rather
than in complying with doctor’s orders. Nurses need to reframe the problem by
excluding the compliance/noncompliance model and developing a conceptual
perspective on self-management that is grounded in world and body.

Celiac Disease: An Epidemiological Reivew
Celiac disease is a lifelong autoimmune disorder triggered by ingesting gluten, which damages the small intestine. Several studies examined factors that influence the risk and development of celiac disease. Introducing gluten at 12 months rather than 6 months had no long-term effect on risk. Gradually introducing gluten between 4-6 months while breastfeeding may reduce risk. A randomized controlled trial found introducing small amounts of gluten at 16-24 weeks did not reduce risk by age 3. Genetics play a role, as the HLA-DQ2 and HLA-DQ8 genes increase susceptibility. Ongoing education is important for managing the lifelong gluten-free diet required to treat celiac disease.
Mediterranean Vs Carbohydrate Restricted Vs Healthy Who Diet
This study examined the relationship between adherence to Mediterranean, carbohydrate-restricted, and general healthy diets and mortality risk among elderly Swedish men over 10 years. The study found that greater adherence to a Mediterranean-like diet was associated with a 29% lower total mortality risk and a 37% lower cardiovascular mortality risk. Neural networks, genetic algorithm selection and bootstrap simulation revealed relationships between 5YS and N0-12 (rank=1), thrombocytes/CC (rank=2), segmented neutrophils/CC (3), eosinophils/CC (4), erythrocytes/CC (5), healthy cells/CC (6), lymphocytes/CC (7), stick neutrophils/CC (8), leucocytes/CC (9), monocytes/CC (10). Correct prediction of 5YS was 100% by neural networks computing (error=0.000; area under ROC curve=1.0).

There is little to no variety for real gluten-free products. For example, Domino’s Pizza has a “gluten-free” pizza but Domino’s has a disclaimer for people with celiac disease saying that they should not eat their “gluten-free” pizza. Shopping for real gluten-free items can be expensive, hard to find, and time consuming, but companies are trying to have more gluten-free options. Overall, this diet is relatively healthy with a good amount of vitamins.
