Randomized controlled trials have demonstrated that lifestyle interventions focusing on diet and exercise can significantly prevent or delay the onset of type 2 diabetes. These trials showed that people with impaired glucose tolerance who received lifestyle advice like diet modification and increased physical activity had approximately a 50% lower risk of developing type 2 diabetes over follow-up periods ranging from 2-20 years compared to control groups. Maintaining a healthy lifestyle through diet and exercise can protect against most cases of type 2 diabetes.

The present investigation, Subjective Global Assessment (SGA) and its different variants are being widely used as a nutritional status or risk assessment tool in clinical and hospital practice for myriads of disease including life-threatening one such as cancer, chronic kidney diseases. SGA is based on measurement and observation of several parameters such as weight change, dietary intake change, gastrointestinal symptoms, functional capacity, co morbidities related to nutritional condition and physical examination. However, the tool is not devoid of limitation and is being constantly improved for the optimization of its use in various other diseases. Therefore, clinicians need an easy to use and interpret, low cost, reliable tool to assess nutritional status. The PG-SGA is a more sensitive tool than other versions of SGA and is successfully being used as a screening tool in diseases like cancer, tuberculosis, HIV and chronic kidney disease (CKD) etc. According to the theory of “reverse epidemiology”, a patient with better nutritional status is supposed to have increased scope of survival. Therefore, it is increasingly being used patients who are at the pre-dialysis stage or being treated with dialysis. The review will summarize the basics of the nutritional assessment tool, its indications, and limitation of use in clinical practice etc. Moreover, the review will summarize the recommendations for use of PG-SGA in CKD and a brief review of existing literature to understand the scope of use and future perspective of the application of this tool for using in CKD patient population. This study analyzed differences in health parameters between dietary groups using data from the Austrian Health Interview Survey. Over 1,300 participants were matched by age, sex, and socioeconomic status into four dietary groups: vegetarian, carnivorous with high fruit/vegetable intake, carnivorous with low meat intake, and carnivorous with high meat intake. The study found that vegetarians had a lower BMI but poorer overall health, with higher rates of cancer, allergies, and mental health issues. They also had greater healthcare needs and lower quality of life compared to the other dietary groups. The development of a new dietary adjunct with a novel natural antioxidant impact on diabetes mellitus with prevention of its long term deleterious effect on the male fertility in general has been increasingly expressed in recent time. Hence, we aim to evaluate the effects of aqueous extract of Sesame radiatum leaves on adult male Sprague Dawley rats’ testis using unbiased stereological, biochemical and hormonal studies. Thirty adult male rats were divided into three groups of 10 rats each. The treated groups; 1 and 2 received 28.0 and 14 mg/kg bwt of aqueous extract of sesame leaves via oral garvage, respectively, while the control group received equal volume of 0.9% (w/v) normal saline per day for 6 weeks. Serum follicle-stimulating hormone (FSH), testosterone and blood glucose were assayed. In addition five microns of uniformly random transverse sections of processed testicular tissues were equally analyzed using an un-biased stereological study. The result showed that the mean percentage volume fractions (Vf) of epithelial cells and lumen of the testis were 76% (P<0.05)><0.05),>0.05) higher than the control in a dose related manner. Serum testosterone and FSH were significantly higher and lower, respectively, in the high dose sesame when compared to control. Sesame leaves intake improved glucose profile and testicular parameters in a dose related manner via possible improved insulin activity on the cells with a stimulatory impact on sperm production. This also confirmed its folkloric claims.

The development of a new dietary adjunct with a novel natural antioxidant impact on diabetes mellitus with prevention of its long term deleterious effect on the male fertility in general has been increasingly expressed in recent time. Hence, we aim to evaluate the effects of aqueous extract of Sesame radiatum leaves on adult male Sprague Dawley rats’ testis using unbiased stereological, biochemical and hormonal studies. Thirty adult male rats were divided into three groups of 10 rats each. The treated groups; 1 and 2 received 28.0 and 14 mg/kg bwt of aqueous extract of sesame leaves via oral garvage, respectively, while the control group received equal volume of 0.9% (w/v) normal saline per day for 6 weeks. Serum follicle-stimulating hormone (FSH), testosterone and blood glucose were assayed. In addition five microns of uniformly random transverse sections of processed testicular tissues were equally analyzed using an un-biased stereological study. The result showed that the mean percentage volume fractions (Vf) of epithelial cells and lumen of the testis were 76% (P<0.05)><0.05),>0.05) higher than the control in a dose related manner. Serum testosterone and FSH were significantly higher and lower, respectively, in the high dose sesame when compared to control. Sesame leaves intake improved glucose profile and testicular parameters in a dose related manner via possible improved insulin activity on the cells with a stimulatory impact on sperm production. This also confirmed its folkloric claims.

Types of diabetes:
Type 1 diabetes only accounts for 5% of diabetes cases and is usually diagnosed in childhood or adolescent ages (Rosenthal & Burchum, 2017). In type 1, the body destroys its own pancreatic beta cells through an autoimmune process. For this reason, no insulin can be produced innately, hence why it is known as insulin-dependent diabetes (Rosenthal & Burchum, 2017). Juvenile diabetes used to be used interchangeably with the term Type 1 diabetes, although the incidence of children developing type 2 diabetes is on the rise as well as the correlated rates of childhood obesity (Valaiyapathi et al., 2020).
Type 2 diabetes, which will be the main focus of this discussion, is usually diagnosed after the age of 40, and while there is a large hereditary component, it is often brought on largely by modifiable risk factors such as obesity, poor diet and sedentary lifestyle (Rosenthal & Burchum, 2017). It is characterized by the development of insulin resistance within target tissues such as the liver and adipose tissue, as well as an impaired or delayed secretion of insulin (Rosenthal & Burchum, 2017). Diagnosis of diabetes can include a combination of different tests including a fasting plasma glucose (FPG) of greater than 126 mg/dL, a random glucose of greater than 200, an oral glucose tolerance test (OGTT) of greater than 11, and an A1c of greater than 6.5% (Quattrocchi et al., 2020). It is important to note that other conditions can affect the hgb A1c as well, such as sickle cell, anemia, blood transfusions, dialysis and pregnancy (Quattrocchi et al., 2020). Therefore, multiple tests should be performed and possibly repeated before making a definitive diagnosis.
Gestational diabetes is brought on by pregnancy and subsides rapidly after the birth of the child (Rosenthal & Burchum, 2017). It can be difficult to control due to elevated cortisol levels during pregnancy, other placental hormones that can antagonize the actions of insulin, and also due to the ability of glucose to freely get into the blood of the fetus (Rosenthal & Burchum, 2017). For this reason, blood glucose levels often need to be checked six to seven times per day and be correlated properly with meals / amount of carbohydrates to avoid harm to the fetus. Diet and insulin are utilized primarily to treat this type of diabetes (Rosenthal & Burchum, 2017).

There are many different types of oral medications and different types of insulin that can be used to manage diabetes. Each class of oral medications works differently in the body to help lower blood sugar. A stepwise approach for managing diabetes, especially alongside different comorbidities such as heart disease and CKD should be implemented, as noted by the recommendations by the ADA, which I will link the updated 2023 articles for standards of care for pharm management under the references listed below. Type I Diabetes is hyperglycemia due to insulin deficiency. This is caused by autoimmune destruction of B cells that are within the islets of Langerhans (Chetan, Thrower,& Narendran, 2019). Some signs surrounding Type I DM is polydipsia, polyuria, and weight loss. This is more common in children, but can be diagnosed at any age throughout life. Type I diabetes used to be called juvenile diabetes,but this is now known to not be the correct way to view this disease.
Type 2 Diabetes is impaired insulin secretion and abnormal suppression of postprandial (Hurtado & Vella, 2019). Type 2 DM the pancreas resists insulin, or does not produce enough insulin on its own. Often Type I and Type 2 DM are diagnosed incorrectly or mistaken for each other as signs and symptoms are similar. Being overweight or reduced amount of exercise is though to be a contributing factor as well as genetics, but this is till not fully understood.
Gestational diabetes is characterized by spontaneous hyperglycemia during pregnancy. Some risk factors include family history of diabetes, obesity during pregnancy, and advanced maternal age (Plows, Stanley, Baker, 2019). B cell dysfunction which lacks the ability to release insulin when the body needs it is what causes gestational diabetes.
Consider one type of drug used to treat the type of diabetes you selected, including proper preparation and administration of this drug. Then, reflect on dietary considerations related to treatment.
Patients who have Type 2 DM have both insulin resistance and impaired insulin secretion (Rosenthal & Burchum, 2021). The liver, muscle and adipose tissue are insulin resistant and increased blood sugar causes B cell function to fail. Insulin secretion decreases and B cells must increase how hard they must work to work against insulin resistance within the tissues (Rosenthal & Burchum, 2021).
The medication I chose os Metformin from the class of Biguanide medications for treatment of Type 2 DM. Metformin uses 3 mechanisms to help lower blood sugar.
Inhibits glucose production in the liver
Reduces glucose absorption in the gut
Sensitizes insulin receptors in fat and skeletal muscle
Metformin is taken by mouth and is absorbed by the small intestines and excreted by the kidneys. Metformin is known to be a great choice for those who skip meals as it will not make blood sugar levels become dangerously low. Those with DM Type 2 should eat healthy carbs such as fruits and vegetables and not indulge in sugary drinks. Protein and polyunsaturated fats should be a staple in the everyday diet. Alcohol should be avoided, and the patient should try to eat several small “meals” per day tp keep blood sugar steady. If blood sugar becomes low having a sugary snack with a complex carbohydrate such as peanut butter and crackers should be on hand.

Investigators of a study of a large sample of American children aged 9-14 years reported that 7.1% of boys and 13.4% of girls displayed disordered eating behaviours.35 The pivotal effect on health has led to the inclusion of eating disorders among the priority mental illnesses for children and adolescents identified by WHO.36 Eating disorders have been reported worldwide both in developed regions and emerging economies such as Brazil and China.37,38 The lifetime prevalence of eating disorders in adults is about 0.6% for anorexia nervosa, 1% for bulimia nervosa, and 3% for binge eating disorder.19,20 Women are more affected than are men, and the sex differences in lifetime prevalence in adults could be less substantial than that quoted in standard texts: 0.9% for anorexia nervosa, 1.5% for bulimia nervosa, and 3.5% for binge eating disorder in women; and 0.3%, 0.5%, and 2.0%, respectively, in men.20 Many people with eating disorders, who were detected in community studies in the USA, do not seek treatment.20 Pathogenesis A comprehensive review published in 2004 summarised the risk factors for eating disorders,39 and a position paper from the Academy of Eating Disorders outlined the evidence supporting these diseases as biologically-based forms of severe mental illnesses.40 In this section we draw attention to some present areas of emphasis.

This Seminar adds to the previous Lancet Seminar about eating disorders, published in 2003, with an emphasis on the biological contributions to illness onset and maintenance. The diagnostic criteria are in the process of review, and the probable four new categories are: anorexia nervosa, bulimia nervosa, binge eating disorder, and eating disorder not otherwise specified. These categories will also be broader than they were previously, which will affect the population prevalence; the present lifetime prevalence of all eating disorders is about 5%. Abstract
The study was conducted to analyze the management of diabetes within the Hispanic America ethnic group. The research was carried out in the different hospitals in the United States to determine the best management practices for the elderly Hispanic America suffering from diabetes. The research used interviews with the patients and the nurses who offer the services to the patients. The research focused on the population of 65years and above Hispanic America. The result indicates that the patients with good care from the family respond to treatment as the proper management results in glycemic control. The research also got that most of the diabetes has a biological origin.
Management of diabetes in Hispanic America
P)-Population: Adults aged 65 years and above from the ethnic group of Hispanic origin who are the leading majority with the diabetes cases across the country. The other adults aged over 65 years and above but not Hispanic America are excluded from the exercise.
I)-Intervention: The research analysis the best management of Type 2 diabetes which is the most dominant type of the Hispanic America. The research compares the effectiveness of the Bariatric surgery in patients with body mass index, the healthy eating habit, and weight control measures. The best approach will be taught in every hospital holding the patients with diabetes in every two weeks seminar.
C)-Comparison: The procedure will take approximately three months then the result will be compared in line the previous mortality related cases. The progress in the health status of the patients will be matched with prior data before the process started.
O)-Outcome: The healthy eating habit and the weight management proved better in improving the conditions required to sustain the patients. The patients with the caretakers who help them in Glycemic control management improves even faster compared to patients without helpers.
T) – Time: The procedure will be analyzed monthly after every two weeks collection of data in the different hospitals.

Clinical Problem
A report from the Center for Disease Control and Prevention in 2015 revealed that (9.4%) 30.3 million Americans are diabetic and 84.1 million have prediabetes. This is a total population of over 100 million is at risk of developing type 2 diabetes which is a growing health problem being the seventh leading cause of death in the U.S. An estimated 1.5 million new cases were among 18-year old bracket and the rates of diagnosed diabetes increased proportionally to age. Below 44 years accounted for 4%, below 64 years at 17 % and 25% for those above 65 years across both genders. One-third of adults in America has prediabetes but sadly, they are unaware despite reports released by The National Diabetes Statistics Report every year. These reports elaborate on prevalence and incidence, prediabetes, long-term complications, risk factors, mortality, and cost. Diabetes poses the risk of serious complications like death, blindness, stroke, kidney disorders, cardiac diseases and health problems that lead to amputation of legs. However, the risks can be mitigated through physical body activities, proper dieting and prescribed use of insulin and other related measures to control the blood sugar levels. Diabetes Prevention Program was funded by NIH to research a yearly evidence-based program to improve healthy weight loss through diet and physical activities. There also efforts to determine the effectiveness of public service campaigns in improving the real-life experience in the diagnosis and treatment of diabetes.
PICOT Question.
The population affected by diabetes cuts across all ages, gender, race, and ethnicity. The prevalence is significantly high from 18 years and it increases with age to about 25% above 65 years. In terms of gender, men are at higher risk accounting for 37% while women are at 30% across races and educational levels. On races, the rates were higher among Indians/Alaska natives at 15%, non-Hispanic blacks at 12.7% and Hispanics at 12%. Among Asians, the rates were lower at 8% and 7.4% for non-Hispanic whites.
Intervention indicator for diabetes shows that individuals who do not observe a healthy diet are more exposed to the disease. Some risk behaviors include lack of exercise and excessive intake of junk foods that lead to obesity and increased blood sugar levels. Diabetes prevalence varied according to education levels were those with less than high school education at 12.6% and 7.2% for those higher than high school education.
Comparison and use of a control group from the popularity of Complementary and Alternative Medicine and Traditional Chinese Medicine showed distinct knowledge of diabetes, blood sugar control, and self-care. The experimental group received education through interactive multimedia for three months while the control group received.

Metabolic syndrome (MetS) is the constellation of risks namely abdominal obesity, insulin-resistance, type 2
diabetes, dyslipidemia and arterial hypertension. In the late 90’s the World Health Organisation (WHO) gave a
definition of the MetS to better understand mechanism and therefore tempt to find appropriate treatment. Starting
with that first definition (initial one) of the metabolic syndrome a range of alternative definitions was suggested. The
most widely accepted definition was formulated by EGIR (European Group for the Study of Insulin Resistance) and
NCEP (USA National Cholesterol Education Panel).
History of metabolic syndrome
In the early 1920s, the Canadian Surgeon Frederick Banting1 extracted a compound they named insulin from the
islets of Langerhans in the pancreas. When injected into diabetic dogs, the compound decreased the levels of sugar
in the dogs’ blood and urine. The discovery, coupled with the purification of insulin, helped transform childhood
diabetes from a fatal disease to a chronic illness.
By the 1930s, it had become common to classify people with diabetes according to how they reacted to an injection
of insulin: insulin-sensitive or type 1 diabetes and insulin-insensitive or type 2 diabetes. Type 2 diabetes accounts
for at least 90% of all the cases of diabetes and refers to adults over 40. This type of diabetes is characterised by
insulin resistance, a phenomenon by which an individual’s body issues have a diminished response to insulin,
causing the body to produce larger quantities of insulin to maintain normal blood glucose level.
In the 1980s, insulin resistance had become a major field of research, as physicians began to understand it not only
has preconditional of type 2 diabetes, but as also associated with pathological cluster such as obesity, heart disease,
stroke and environment that can be named MetS.
Biomarkers1bis for the diagnosis and prognosis of nutritional or metabolic disorders
Worldwide, over 70 million people suffer from eating disorders.
Obesity2 and diabetes are amongst the greatest public health issues for this century. Indeed, over the last
30 years, obesity has more than doubled, achieving 1.5 billion persons. It does affect without any distinction all
the population (children, adult, elderly people) and all socioeconomic status. Obesity is a complex condition which
poses a major risk for serious diet-related, non-communicable diseases, including type 2 diabetes.
In the past 20 years, the rates of obesity have tripled in developing countries that have been adopting a Western
lifestyle involving decreased physical activity and overconsumption of cheap, energy-dense food.
About 18 million people die every year from cardiovascular disease, for which diabetes and hypertension are major
predisposing factors.
Today, more than 1.1 billion adults worldwide are overweight, and 312 million of them are obese.
The increase in the prevalence of type 2 diabetes is closely linked to the upsurge in obesity. About 90% of type 2
diabetes is attributable to excess weight.
Consequently, diabetes is rapidly emerging as a global health care problem that threatens to reach pandemic levels
by 2030; the number of people with diabetes worldwide is projected to increase from 171 million in 2000 to
366 million by 2030.
Anorexia nervosa (AN) 3. Amongst eating disorders, Anorexia Nervosa (AN) is characterized by a chronic
undernutrition state and a body mass index below 17 4. It affects 1-4% of women in the United States and is the
leading cause of death among adolescents in Europe.

3. Globally, AN affects about two million people (Global
burden of disease study 2013). It is estimated to occur in
0.9% to 4.3% of women and 0.2% to 0.3% of men in
Western countries at some point in their life (Smink et al.
2012). 30 to 40% of patients do not recover optimally
even after 10 to 13 years of follow-up (Rigaud et al.,
2011).
The exponential increase of nutritional and metabolic
diseases induced by a disequilibrium between the energy
input and expenditure, i.e. too much, too less, or energy
expenditure (such as obesity or anorexia) generates care
expenses that become a real health societal problem.
Indeed, such an important cost can be explained by
chronicity, severity and usually a lack of appropriate
treatment. It would be reduced by an early diagnosis of
the disease using relevant biomarkers or biological
markers that might lead to begin the treatment at an
early stage of the disorder. Thus, it is necessary to find
pertinent biological markers, easy to measure, to
evaluate the early phase and the different stages as well
as the severity of the disease to propose to the patient
pertinent-personalized therapeutic strategies.
With the identification these past 20 years of new
regulators of energy homeostasis in the brain or
peripheral organs, potential new biomarkers for
metabolic or nutritional disorders are proposed.

Which biomarkers?
To date, there is no effective pharmacological treatment that can guarantee a complete
nutritional recovery in patients suffering from these diseases. In order to develop new
therapeutical strategies and determine new candidate strategies for treatment, there is an
urgent need to identify specific biological markers that could be involved in the maintenance
and/or worsening of the disease. These past years, the scientific community has identified
specific biomarkers that are dysregulated in the overnutrition or undernutrition states and can also reflect the type
(anorexia nervosa versus obesity) or subtype (bingeing-purging versus restrictive AN) of the disease (Germain et al.,
2010; Germain et al., 2016) as well as its severity or duration (Terra et al., 2013).
Past and current research focuses on investigating endocrine biomarkers produced by the adipose tissue or
gastrointestinal tract, such as leptin (Legroux-Gérot et al., 2009; Jamar et al. 2016), adiponectin (Terra et al.,
2013; Lopez-Jaramillo et al., 2014), ghrelin and obestatin (Gorwood et al., 2016; Germain et al., 2010; Kuppens et
al., 2015) or 26-RFa (Galusca et al., 2012) in AN and obesity.
To better understand the mechanisms involved in the process of malnutrition/renutrition, identification of biomarkers
predictive of successful refeeding in patients or animal models mimicking certain aspects of the disease is needed.
There is an urgent
need to identify
specific biological
markers
A special focus is made on hormones produced in the adipose tissue or gastrointestinal tract since they are secreted
in the blood and signal to the brain about the nutritional or metabolic status. The absence of very specific and
sensitive immunoassays is considered to be a limitation to the appropriate measurement of these biomarkers.
However, these past years, new technological developments and their effects in immunoassays alleviate such a
downside.
4. Beyond AN, applications for other diseases associated with undernutrition, such as cachexia or malnutrition
worldwide. Ghrelin and leptin, as well as other adipocytokines such as resistin or adiponectin, are also investigated
as potential biomarkers to diagnose cachexia and predict survival in cancer patients (Mondello et al., 2014, Kerem et
al., 2008).
Obesity – anorexia/cachexia: two faces of a same neurobiological circuit?
The actions of these biomarkers on homeostatic and non-homeostatic neuronal circuits regulating eating
behaviors begin to be well described and better understood (Morton et al., 2014, Fig. 1- page 5). Peripheral
hormones reach the brain through specific way of transport across the blood brain barrier (Schaeffer et al., 2013)
targeting different
brain areas involved in the homeostatic and non-homeostatic regulation of food intake, energy expenditure and thus
body weight.
Briefly, hypothalamus and brainstem structures (nucleus tractus solitarius) are direct sensors of the energy and
metabolic balance while the meso-cortico-limbic system (ventral tegmental area – nucleus accumbens) is devoted to
the motivational/rewarding aspects of the food. Obesity and Anorexia Nervosa display deregulation in the synthesis
and/or release of several peripheral factors that modulate central feeding centers. In particular, plasma ghrelin and
leptin concentrations show opposite variations leading to the hypothesis of a key role of these hormones in the
physiopathology of these diseases (from the genesis to the different outcomes inherent to such feeding disorders).
A better understanding of their evolution/kinetic during the course of the disease is essential to potentially find a
pertinent therapeutic solution by acting at different levels to reprogram a dysfunctional system. Such fundamental
studies, using genetically-engineered mice (total deletion or conditional deletion of genes in specific neuronal or
peripheral population) have permitted to highlight the role of gut hormones and/or central neuromediators (like
dopamine and serotonin, for example). The mechanisms of action leading to a chronicisation of the disease (obesity
or anorexia) remain currently poorly understood rendering urgent to perform valuable and reproducible
measurements of the evolution of pertinent biomarkers during the course of the disease to better understand the
mechanisms of action of these hormonal biomarkers in pathological states at both central and peripheral levels.

New biomarkers for future personalized therapeutic treatments
Our own experience using Bertin Pharma kits
These past years, our research has been focused on understanding the role of preproghrelin, a complex prohormone
encoding various peptides (acyl and desacyl ghrelin, obestatin) with structural and functional heterogeneity, in the
pathophysiology of eating disorders. Our aim was to evaluate the relative ratio of these peptides in undernutrition
conditions in both rodents and humans. The absence of selective and reliable assays to measure all three ghrelin-
derived in the same biological sample was a limitation to understand how these key gastrointestinal peptides were
regulated in pathological conditions.
First, we validated Bertin Pharma’s immunoassay kits by demonstrating the absence of immunoractivity for both
acyl and desacyl ghrelin in preproghrelin deficient mice (Hassouna et al. 2014). Since immunoassays kits have been
developed in multi-species, we are able to use them for explorations in different models: rat, mouse, human. Thus, we
were able to assay ghrelin in both obesity (Lacroix et al. 2015) and undernutrition states in both animal models
(Méquinion et al. 2015) and AN patients in collaboration with members of the interdisciplinary network on eating
disorders, GIR-AFDAS-TCA (www.anorexieboulimie-afdas.fr) and CMME5 (Paris, France). Exploration of these
biomarkers in preclinical and clinical studies is essential to better understand the pathophysiology of the disease. In
collaboration with Bertin Pharma, we have worked on optimizing the conditions of sampling, treatment and storage
of samples.
Such assays have several advantages:
> they are developed in multi-species, from rodent (rat and mouse) to human, which is ideal for preclinical
to clinical investigations,
> they are specific for different isoforms of the same peptide (such as acyl versus desacyl ghrelin) that can
have various biological effects (Delhanty et al. 2013),
> they are sensitive enough to detect small concentrations or variation ranges,
> they are reproducible and designed to recover peptides with post-translational modifications (such as
acylation) with specific treatment.
What treatment(s) for tomorrow?
Lifestyle modification is the mainstay of prevention and treatment for metabolic syndrome and
type 2 diabetes; however, it can be costly and labour-intensive. Pharmacotherapy is considered as
second line of therapy in adults, but its use in children is controversial. In that framework, the use
of specific biomarkers is fundamental to better apprehend the mechanisms that are involved in
eating disorders and regulation of body weight. The future will be to use targeted and
personalised therapies. Indeed, the generalisation of genetic, epigenetic, or pharmacogenetic tools will be of
importance to know the ins and outs of the disorders related to food intake according to age, sex and endocrine
disruptors.
The current scientific priorities include more specifically the study of (i) the interaction between genes and
environment with a special focus on the role of the gut microbiota and the impact of various environmental pollutants
and (ii) the dialog between the brain and the gut. Recent data obtained by the French consortium Micro-Obes (2013)6
have shown that individuals with a poor gut bacteria diversity are exposed to a higher risk to develop complications
linked with an obese phenotype. Changing the composition of the daily diet was sufficient to restore a normal gut
microbiota. Thus, the development of an easy test to specifically identify these “at risk” persons may help to propose
an adapted and personalized treatment.
In this view, identifying biomarkers is a necessary step for achieving better diagnostics and follow evolution of
diseases and efficiency of treatments. The development of new valuable preclinical models (in vivo and/or in vitro) to
further assess the mechanisms and the impact of new potential treatments is fundamental to obtain substantial
knowledge and answers in the field of type II diabetes, nutrition and metabolic disorders. Such data might be obtained
through dedicated clinical platforms to phenotype and follow up large national cohorts.

Ghrelin: the hunger hormone
Ghrelin gene raises to mRNA prepro-ghrelin of 117 amino acids. This precursor is processed into ghrelin, 28
amino acids (human). This endogenous peptide appears directly related to feeding behavior. Its potential

10. Resistin: a credible link between insulin resistance and type 2 diabetes
Resistin is a peptide hormone belonging to the class of cysteine-rich secreted proteins which is termed the RELM
family. It is described as ADSF (Adipose Tissue-Specific Secretory Factor) and FIZZ3 (Found in Inflammatory
Zone).
Studies have shown that in mice, resistin impairs glucose tolerance and insulin action. It also inhibits
adipogenesis in murine 3T3-L1 cells. Therefore, resistin has been proposed as an adipocyte secreted factor that is
