was analyzedandpossiblereasonsforlowscore were discussed andactionplanwasprepared.
One of the mainreasonfor lowscore waspertainingtoqualityissuesassociatedwithraw
material andequipmentreceivedat yard. Projectqualityissuesreportedduringthe pastsix
monthswere discussed.Gapanalysiswasdone androotcauseswere identified.Mainrootcause
behindprojectqualityissueswere lackof propercommunicationamongthe functions,
Contractor failure of identifyingthe deficienciesinreceivedmaterial/equipment andnotgiving
the importance toproactive notifications.Actionplanwaspreparedaccordingly.Projectweld
repairrate wasanalyzed andactionplanwas preparedtomaintainthe projectweldrepairrate
below3%on jointbasis. Furthermore,downtrendinPQIscore especiallyinthe procurement
phase wasanalyzedandfactor affectingthe PQIscore were discussed.Processimprovements
and qualityinitiativesimplementedduringprojectexecutionwerealsodiscussed.
