Essay 2As a registered nurse, you have the power to influence change in patient outcomes.
An important aspect of influencing change is identifying areas that need improvement. This
is done primarily through measurement of data. There are several different measures to
gather data within organizations as well as on a national scale. Some of these measurements
include core measures, standards, best practices, evidence-based practices, and the National
Database of Nursing Quality Indicators (NDNQI). These mechanisms have also been
discussed as a means for helping nurses to deliver quality care and improve patient safety.
Each measurement essentially focuses on providing care that is safe, effective, patient-
centered, timely, efficient, and equitable.Although there are several different
measurements, NDNQI data is used in the process of attaining Magnet Recognition. Magnet
Recognition is the highest honor a health care organization can receive for nursing
excellence and high-quality patient care. The nurse-specific measures presented in the
NDNQI help inform nursing staffs and their organizations of areas where nursing practices
can be improved and where nursing practice efforts are producing positive clinical
outcomes. Nurses must be directly involved in developing and implementing action plans
based on the data presented by the NDNQI.This week, you will explore the importance of
nurse empowerment in effecting change and how action plans are created based on the
results of the NDNQI as presented on a dashboard. You will also consider how nurses
advocate for patients’ rights, even when that means ing a patient whose personal choices

Improvement Plan Tool Kit
This improvement plan tool kit aims to enable nurses to implement and sustain safety
improvement measures in health care settings in a geropsychiatric unit. The tool kit has been
organized into four categories with three annotated sources each. The categories are as follows:
general organizational safety and quality best practices, environmental safety and quality risks,
staff-led preventive strategies, and best practices for reporting and improving environmental
safety issues.

Standardized Bedside Reporting
One of the goals of healthcare is to ensure that the patients get the best service possible while not compromising on the satisfaction and goodwill of the nurses and other healthcare professionals. A key aspect of ensuring quality healthcare is the consistent handling of patient information from nurse to nurse during shifts; information handled wrongly can jeopardize the patients’ health (Baker, 2010). It is important to implement procedures that ensure consistent and smooth handling of patient information from nurse to nurse to increase patient safety and improve nurse satisfaction. This paper will explore the merits of standardized bedside reporting as opposed to board reporting in ensuring a positive outcome and consistent quality healthcare.
Change model overview
A key aspect in determining whether bedside shift reporting has any merits over board reporting is the John Hopkins Nursing Evidence-Based Practice Process (JHNEBP). The John Hopkins Nursing Evidence-Based Practice Process is a framework for guiding the translation and synthesis of evidence into valid healthcare practice. JHNEBP has three cornerstones that include research, education, and practice; the framework ensures that research evidence is the basis of clinical decision-making. (Dearholt & Dang, 2012) The implementation of the John Hopkins Nursing Evidence-Based Practice Process has three key phases, the first phase is the identification of an important question, the second phase involves the systematic review of research evidence, and the third phase is translating the results into action. Nurses should use the JHNEBP process because it provides a clear way for healthcare professionals to translate research results into healthcare practice.
Practice Question
The team includes several key stakeholders who will benefit greatly from my research. Among the team members include myself as ER nurse, charge nurse, ERT ( Emergency room tech), nurse case manager, nurse supervisor, physician and hospital manager.
The evidence-based practice question that the team members will explore is "Does the use of a standardized bedside report versus board reporting help increase patient safety, nurse satisfaction, and positive outcome?" The evidence-based practice question assesses the ability of bedside shift reporting to improve healthcare provision. The practice area of the question is clinical. The practice issue came about because of assessing risk management concerns in ensuring good health practices. To answer the question, the team members gathered evidence from patient preferences, peer-reviewed journals, and clinical guidelines. The team members searched peer-reviewed journal databases to gather relevant information from previous research that could affect the results.
Understanding the merits of bedside shift reporting as opposed to board reporting is important as most healthcare organization use either strategy in collecting and passin.
Implementing Fixed Patient For Nurse Ratios
This document proposes implementing hourly rounding at a hospital to improve patient safety and satisfaction. A task force would be established to use research evidence to propose, implement, and evaluate the change. The Johns Hopkins Nursing Evidence-Based Practice model would be used as a guideline, consisting of three phases - identifying the issue, searching for evidence, and translating evidence into practice. Implementing hourly rounding would help meet patients' needs, reduce call lights and falls, and improve HCAHPS scores.

How does your facility incorporate EBP in a clinical setting to promote patient outcomes? Do you have recommendations on how your facility can improve its use of EBP?
My facility incorporates evidence-based practice in the clinical setting in a resourceful manner in order to promote patient outcomes. The institution integrates clinical expertise, the best research evidence and patient values in the decision making process to foster the implementation of evidence based practice. In this endeavor, the facility also encourages patients to bring their personal preferences, unique concerns, values and expectations in order to ensure that the clinicians have a heighted understanding of their pertinent medical issues and provide patient-centered care that meets the distinct needs of the patients.
According to
Polit & Beck (2011),
through the practice of EBP, the healthcare facility can be in a better position to ascertain the effects of therapy, the prognosis of diseases, the utility of diagnostic tests as well as the etiology of disorders. It is worth mentioning that my facility follows various steps in implementing the EBP practices. These steps include assessing the patient, asking clinical questions derived from the patient’s case, acquiring the evidence through searching appropriate resources and then appraising the evidence for its applicability and validity. The other steps usually include integrating the evidence with patient preferences, clinical expertise and applying it to practice. The institution also encourages all practitioners to evaluate their performance with the patient as a yardstick for determining the effectiveness of the EBP process.
Although the facility has registered notable success in its EBP initiatives, various recommendations can help it to improve on these processes. One of such proposals is to aim at creating and sustaining strong nurse-client relationships

. This can enable the practitioners to have a better understanding of the patient’s unique values and preferences, all of which are fundamental components of EBP practice. The leaders of the facility should also serve as positive role models through advocating, embracing and communicating the benefits of EBP to other employees in the organization. Ultimately, this will help to create a culture that supports the adoption and implementation of evidence-based practice across the entire healthcare establishment

How does your facility incorporate EBP in a clinical setting to promote patient outcomes? Do you have recommendations on how your facility can improve its use of EBP?
My facility incorporates evidence-based practice in the clinical setting in a resourceful manner in order to promote patient outcomes. The institution integrates clinical expertise, the best research evidence and patient values in the decision making process to foster the implementation of evidence based practice. In this endeavor, the facility also encourages patients to bring their personal preferences, unique concerns, values and expectations in order to ensure that the clinicians have a heighted understanding of their pertinent medical issues and provide patient-centered care that meets the distinct needs of the patients.
According to
Polit & Beck (2011),
through the practice of EBP, the healthcare facility can be in a better position to ascertain the effects of therapy, the prognosis of diseases, the utility of diagnostic tests as well as the etiology of disorders. It is worth mentioning that my facility follows various steps in implementing the EBP practices. These steps include assessing the patient, asking clinical questions derived from the patient’s case, acquiring the evidence through searching appropriate resources and then appraising the evidence for its applicability and validity. The other steps usually include integrating the evidence with patient preferences, clinical expertise and applying it to practice. The institution also encourages all practitioners to evaluate their performance with the patient as a yardstick for determining the effectiveness of the EBP process.
Although the facility has registered notable success in its EBP initiatives, various recommendations can help it to improve on these processes. One of such proposals is to aim at creating and sustaining strong nurse-client relationships
(Majid, Foo, & Luyt, 2011)
. This can enable the practitioners to have a better understanding of the patient’s unique values and preferences, all of which are fundamental components of EBP practice. The leaders of the facility should also serve as positive role models through advocating, embracing and communicating the benefits of EBP to other employees in the organization. Ultimately, this will help to create a culture that supports the adoption and implementation of evidence-based practice across the entire healthcare establishment

Nearly fifteen years ago, the Institute of Medicine published the “To Err Is Human” report, which exposed the substantial impact of medical errors in the US healthcare system and called for a dramatic system change, including an improved understanding of those errors (McCarthy, Tuiskula, Driscoll, & Davis, 2017). Medical errors are considered to be failure to achieve the original goal or plan of action, and these errors may range from a patient falls to a mistake in the operating room. Not only do medical errors cause harm to the patient and jeopardize the patient’s trust, but they also cause a financial strain for the health system (“To Err is Human,” 1999). One of the contributing factors to medical errors is the lack of effective communication between doctors who are treating the same patient. This results in healthcare providers overprescribing medications for patients as well as increases the possibility of a patient having unnecessary tests or procedures performed. The report’s four-tiered approach includes:
· Focusing on creating a stronger foundation of education on patient safety
· Mandating a nationwide reporting system to encourage timely reporting of errors
· Increasing the standards of performance for healthcare providers
· Taking advantage of the security that safety systems offer (“To Err is Human,” 1999)
Creating a strong educational foundation for patient safety is most important. Healthcare personnel are much more likely to actively participate in reporting systems, encourage one another to perform at a higher level, and take advantage of safety systems when they are well educated on patient safety and the implications of medical errors. The reporting system seems to provide the least amount of impact on patient safety as they can result in losing patient trust in certain healthcare systems. The healthcare system as a whole has made progress in establishing a safe environment for patients when they are in need of care.

Despite continuing evidence of problems in patient safety and gaps between the care that patients receive and the evidence about what they should receive, efforts to improve quality in healthcare show mostly inconsistent and patchy results.

The concept of a knowledge worker
The term “knowledge worker” was first coined by Peter Drucker. Ducker defined knowledge workers as high-level workers who apply theoretical and analytical knowledge, acquired through formal training, to develop products and services (CFI, 2019). He emphasized that due to the high level of productivity and creativity of knowledge worker, that they would be the most valuable assets in the 21st century organization. Professionals that can be referred to as knowledge worker includes engineers, pharmacists, architects, financial analysts, public accountants, physicians, scientists, design thinkers, and lawyers. Furthermore, knowledge workers have a high degree of expertise, experience, education and the primary purpose of their jobs involve the distribution, creation and application of knowledge.

A nurse leader is one who inspire others to work together to achieve a common goal, for instance enhanced patient care or satisfaction. All nurses are called to nurse leadership, however there are different levels of nurse leadership. Nurse leaders are expected to help the organization to fulfill the organization’s mission, vision, values, and strategies to achieve long-range plans. Aims and objectives. To quantify quantitative outcomes of a practice change to a
blended form of bedside nursing report.
Background. The literature identifies several benefits of bedside nursing shift
report. However, published studies have not adequately quantified outcomes
related to this process change, having either small or unreported sample sizes or
not testing for statistical significance.
Design. Quasi-experimental pre- and postimplementation design.
Methods. Seven medical-surgical units in a large university hospital implemented a
blend of recorded and bedside nursing report. Outcomes monitored included patient

When nurses understand what interventions are needed to achieve desired patient out-
comes, they can more easily defi ne their practice. Work Complexity Assessment (WCA)
is a process that helps nurses to identify interventions performed on a routine basis for
their specifi c patient population. This article describes the WCA process and links it to
the Nursing Interventions Classifi cation (NIC) and the Nursing Outcomes Classifi cation
(NOC). WCA, NIC, and NOC are all tools that help nurses understand the work they do
and the outcomes they achieve, and that thereby acknowledge and validate nursing’s
contribution to patient care.
A shortage of nurses in the United States has been documented since 1998 (Ulrich, Buerhaus, Donelan, Norman, & Ditt us, 2005). A nursing workforce
that is aging (Ulrich et al., 2005), an aging population with increasing needs for
health care services (American Association of Colleges of Nursing [AACN], 2008;
Ulrich et al., 2005), and the inability of colleges and universities to meet grow-
ing nursing enrollment needs (AACN) have been cited as major reasons for this
shortage. The shortage has taken on new signifi cance in light of the Institute of
Medicine reports on quality and patient safety (Institute of Medicine, n.d.) and
descriptions of the work environment (Ulrich et al., 2005).
In this time of scarce resources, it is important to address the complexity of
the work that nurses do, the outcomes of this work, and the interventions used
to achieve these outcomes. Patient satisfaction, the ultimate outcome of care (Do-
nabedian, 1966), is a crucial concern for nurses and, for some health care institu-
tions, is tied to reimbursement (Centers for Medicare and Medicaid, 2008).
The nursing profession has developed tools to describe nursing practice and
patient outcomes. When nurses understand what interventions are needed to
achieve desired patient outcomes, they can more easily defi ne their practice. Work
Complexity Assessment (WCA) is a process that helps nurses identify interventions
performed on a routine basis for their specifi c patient population, using the Nurs-

Nearly fifteen years ago, the Institute of Medicine published the “To Err Is Human” report, which exposed the substantial impact of medical errors in the US healthcare system and called for a dramatic system change, including an improved understanding of those errors (McCarthy, Tuiskula, Driscoll, & Davis, 2017). Medical errors are considered to be failure to achieve the original goal or plan of action, and these errors may range from a patient falls to a mistake in the operating room. Not only do medical errors cause harm to the patient and jeopardize the patient’s trust, but they also cause a financial strain for the health system (“To Err is Human,” 1999). One of the contributing factors to medical errors is the lack of effective communication between doctors who are treating the same patient. This results in healthcare providers overprescribing medications for patients as well as increases the possibility of a patient having unnecessary tests or procedures performed. The report’s four-tiered approach includes:
· Focusing on creating a stronger foundation of education on patient safety
· Mandating a nationwide reporting system to encourage timely reporting of errors
· Increasing the standards of performance for healthcare providers
· Taking advantage of the security that safety systems offer (“To Err is Human,” 1999)
Creating a strong educational foundation for patient safety is most important. Healthcare personnel are much more likely to actively participate in reporting systems, encourage one another to perform at a higher level, and take advantage of safety systems when they are well educated on patient safety and the implications of medical errors. The reporting system seems to provide the least amount of impact on patient safety as they can result in losing patient trust in certain healthcare systems. The healthcare system as a whole has made progress in establishing a safe environment for patients when they are in need of care.

Despite continuing evidence of problems in patient safety and gaps between the care that patients receive and the evidence about what they should receive, efforts to improve quality in healthcare show mostly inconsistent and patchy results.

The concept of a knowledge worker
The term “knowledge worker” was first coined by Peter Drucker. Ducker defined knowledge workers as high-level workers who apply theoretical and analytical knowledge, acquired through formal training, to develop products and services (CFI, 2019). He emphasized that due to the high level of productivity and creativity of knowledge worker, that they would be the most valuable assets in the 21st century organization. Professionals that can be referred to as knowledge worker includes engineers, pharmacists, architects, financial analysts, public accountants, physicians, scientists, design thinkers, and lawyers. Furthermore, knowledge workers have a high degree of expertise, experience, education and the primary purpose of their jobs involve the distribution, creation and application of knowledge.

Aims and objectives. To quantify quantitative outcomes of a practice change to a
blended form of bedside nursing report.
Background. The literature identifies several benefits of bedside nursing shift
report. However, published studies have not adequately quantified outcomes
related to this process change, having either small or unreported sample sizes or
not testing for statistical significance.
Design. Quasi-experimental pre- and postimplementation design.
Methods. Seven medical-surgical units in a large university hospital implemented a
blend of recorded and bedside nursing report. Outcomes monitored included patient
and nursing satisfaction, patient falls, nursing overtime and medication errors.

When nurses understand what interventions are needed to achieve desired patient out-
comes, they can more easily defi ne their practice. Work Complexity Assessment (WCA)
is a process that helps nurses to identify interventions performed on a routine basis for
their specifi c patient population. This article describes the WCA process and links it to
the Nursing Interventions Classifi cation (NIC) and the Nursing Outcomes Classifi cation
(NOC). WCA, NIC, and NOC are all tools that help nurses understand the work they do
and the outcomes they achieve, and that thereby acknowledge and validate nursing’s
contribution to patient care.
A shortage of nurses in the United States has been documented since 1998 (Ulrich, Buerhaus, Donelan, Norman, & Ditt us, 2005). A nursing workforce
that is aging (Ulrich et al., 2005), an aging population with increasing needs for
health care services (American Association of Colleges of Nursing [AACN], 2008;
Ulrich et al., 2005), and the inability of colleges and universities to meet grow-
ing nursing enrollment needs (AACN) have been cited as major reasons for this
shortage. The shortage has taken on new signifi cance in light of the Institute of
Medicine reports on quality and patient safety (Institute of Medicine, n.d.) and
descriptions of the work environment (Ulrich et al., 2005).
In this time of scarce resources, it is important to address the complexity of
the work that nurses do, the outcomes of this work, and the interventions used
to achieve these outcomes. Patient satisfaction, the ultimate outcome of care (Do-
nabedian, 1966), is a crucial concern for nurses and, for some health care institu-
tions, is tied to reimbursement (Centers for Medicare and Medicaid, 2008).
The nursing profession has developed tools to describe nursing practice and
patient outcomes. When nurses understand what interventions are needed to
achieve desired patient outcomes, they can more easily defi ne their practice. Work
Complexity Assessment (WCA) is a process that helps nurses identify interventions
performed on a routine basis for their specifi c patient population, using the Nurs-
ing Interventions Classifi cation (NIC) system described below. Nursing Outcomes
Classifi cation (NOC) is a system that measures nursing-sensitive patient outcomes.

An important aspect of influencing change is identifying areas that need improvement. This is done primarily through measurement of data. There are several different measures to gather data within organizations as well as on a national scale. Some of these measurements include core measures, standards, best practices, evidence-based practices, and the National Database of Nursing Quality Indicators (NDNQI). These mechanisms have also been discussed as a means for helping nurses to deliver quality care and improve patient safety. Each measurement essentially focuses on providing care that is safe, effective, patient- centered, timely, efficient, and equitable.Although there are several different measurements, NDNQI data is used in the process of attaining Magnet Recognition. Magnet Recognition is the highest honor a health care organization can receive for nursing excellence and high-quality patient care. The nurse-specific measures presented in the NDNQI help inform nursing staffs and their organizations of areas where nursing practices can be improved and where nursing practice efforts are producing positive clinical outcomes. Nurses must be directly involved in developing and implementing action plans based on the data presented by the NDNQI.This week, you will explore the importance of nurse empowerment in effecting change and how action plans are created based on the results of the NDNQI as presented on a dashboard.
