SOCW 6446: Social Work Practice With Children and Adolescents
Treatment Plan TemplatePART A
Instructions: Use this template to create a treatment plan. Provide your response to each area in the box below:
I. Identify a list of problems reported to you by the client and/or caregiver(s).
II. As you are able, identify a provisional primary psychiatric diagnosis you believe may be present and may need to be addressed. (Note: Refer to the DSM-5 for diagnostic criteria for specific problems listed.)
III. Identify the level of care needed to address the presenting problem(s). This could include:
a. Inpatient
b. Residential treatment
c. Partial hospitalization
d. Intensive outpatient counseling
e. Outpatient counseling
IV. Identified strengths: When identifying goals, include strengths that will help client achieve long-term goal(s) (e.g., supportive family). Client should help identify strengths. Initially, it may be difficult to help client identify more than one or two strengths, but as the course of treatment continues, more should become evident.
V. Identified problems/deficits: Includes factors in client’s life that may impede successful recovery.
VI. Explain one treatment intervention you might use in the case you selected and justify the use of the intervention. Next select a treatment modality— individual counseling, group counseling, family counseling, or a combination of these. Support your recommended intervention and modality with evidence from scholarly resources.
(Note: Consider researching evidence-based treatments or treatment outcomes that you can use to help guide your recommendations for treatment.)
VII. Identify and describe how you will tailor the treatment to the client’s unique individual and cultural background.
VIII. Explain how you would involve the parents/guardians in the treatment plan and why their involvement might be important.
PART B:
Based on the answers provided above, create a treatment plan by describing the counseling goals in the most measurable way possible (e.g., how will you and the client be able to recognize that the problem has been reduced or the goal has been partially or completely met?). Complete row 3 in the template below. Identify 1-3 long-term goals and the associated short-term goals, objectives, strategies, and expected outcomes.
Long-Term Goal(s):
Short-Term Goals
Objectives
Strategies
Expected Outcome
(With Time Frame)
Stated as broad desirable outcome that will be broken down into short-term goals and objectives; usually, one long-term goal will be adequate for first year.
Series of time-limited goals that will lead to achievement of long-term goal
Statements of what client will do to achieve short-term goal. Stated in measurable, behavioral terms
How objective will be carried out or accomplished
Objective, measurable desirable outcome with timeframe

Drug addiction among teenagers is a serious and growing problem in the Philippines. It is estimated that as many as 60,000 young people are dealing with drug addiction, with marijuana being the most commonly used drug. Within the last ten years, drug addiction among Filipino youth has risen dramatically. Both governmental and private organizations in the Philippines are taking measures to treat addicted youth, educate the public on the dangers of drugs, and help the country address this alarming issue.
The document discusses ways to help patients with bipolar disorder recover. It suggests that parents should be the primary support for patients by educating themselves on bipolar disorder, monitoring their child's moods and activities, seeking medical help, and providing a supportive home environment. Friends can also play a supportive role by being understanding, encouraging patients' interests, and advising parents if needed. On a societal level, online support blogs and social media sites could help connect people to share experiences and reduce stigma. The top three ideas selected were: 1) parents being the primary mentors and friends, 2) friends also playing a supportive role, and 3) online support resources to help connect people on a societal level.
This document discusses addiction from several perspectives. It defines addiction as a pathological relationship between a human and an addictive object like substances or behaviors. Addiction is described as having physical, psychological, and social components. The development of addiction is influenced by social and environmental factors. Treatment approaches discussed include cognitive-emotional-behavioral therapy and a five-phase therapeutic model. The document also notes the issue of drug addiction in Palestine and rehabilitation organizations working to address it.
The document discusses drug abuse issues in the Philippines. It notes that drug abuse is a serious problem that destroys lives and communities. According to government statistics, the most common drugs abused are shabu, marijuana, and inhalants. The document also discusses risk factors for drug abuse like poverty, peer pressure, and easy access to drugs. It notes President Duterte's campaign to eliminate drug supply and demand through harsh enforcement measures.
The document outlines a substance abuse prevention program called H.U.G.S. NOT DRUGS for adolescents. It describes the program goals of educating youth on substance use risks and teaching refusal skills. The program uses role-playing exercises and addresses common myths about drugs. It also provides facts on local substance use rates and consequences of underage alcohol and drug use according to the law.
This document discusses factors that contribute to drug abuse and ways to address the problem. It provides examples of individuals who abuse drugs, like Jhan and Tina, and explains what led them to drug use. Key contributing factors discussed include family problems, peer pressure, curiosity, and using drugs to cope with problems or feel enjoyment. The document recommends preventing drug abuse through education, counseling, parenting support, and promoting healthy activities as alternatives to drug use.
1) The document discusses ways to help patients with bipolar disorder, including the roles that parents, friends, and society can play.
2) It suggests that parents should be the patients' first mentors and friends, making themselves available to understand mood swings and counsel patients.
3) Friends can also play a vital role by being supportive, engaging with patients' hobbies, and keeping them calm and positive.
4) The document recommends that society can help by launching online support blogs and social networks where people with bipolar disorder can safely share experiences.
