Editor's Notes
Good morning everyone, my topic for today is……. COMPLICATIONS OF DRUG ABUSE
I would highlight 4 different aspects of complications due to substance abuse- namely ---1 DEVELOPMENT of Dependence & Behavioral PROBLEMS , Medical complications,
Social complications & Financial complications.
Substance Dependence is a result of hijack of brain reward pathways – involving mesolimbic - dopamine & serotonin neurons.
Drugs of abuse generally mimic naturally occurring neurotransmitters and act by affecting their receptors at synapse level.
Hijacking of brain reward system by drugs leads to OBSESSIVe PREOCCUPATION with drugs and neglect of everything else. As we could clearly see a addicted person keeps obsessing about drugs, drugs, his next dose, money,,, drugs…..and nothing else.
Substance use progresses from initial experimentation ……often with GATEWAY DRUGS …. Alcohol, tobacoo or Cannabis ……., gradually more frequent use , ….to regular intake.
With time,….
Prolonged drug abuse changes the brain in ways that reinforce drug taking and lead to addiction. These changes are difficult to undo and last a long time.
Short term complications of substance use include intoxication, confusion, delirium, blackout, amnesia and overdose seizures.
With long term use there is development of DEPENDENCE . Long term use leads to many PSYCHIATRIC & NEUROLOGICAL complications.
There is high degree of comorbidity of substance dependence & Mental illness ……Read bottom line
NCRB data of three conssecutive yrs shows more than 4000 substance abusers commited suicide every year .
In this graph showing relation of dependence & Physical harm ………Heroin also known as CHITTA at the right upper corner is the most addicting and harmful drug currently being used.
Briefly I will outline most characteristic complications of commonly used substances in our area.
HEROIN ( the real heroin among drugs of abuse) is extremely addictive. Tolerance develops rapidly and users shift from snorting to injecting intravenously to keep up with the requirement of dose escalation.
Most serious complications are due to hazardous IV injections------ blood borne infections &
Alcohol spares no body organ. Common complications include……..
Most of patients dependent on benzodiazepines suffer from withdrawal seizures. They also have memory problems . Benzodiazapine use in elderly can contribute to falls – & fractures .
And here comes the darling of cardio-respiratory people. Nicotine- either SMOKED or CHEWED is leading cause of CA of LUNG and OROPHARYNX. …… It is Also leading cause of Ischemic heart disease & Peripheral vascular disease.
CANNABIS is the Most commonly abused substance in the world. Responsible for various psychiatric disorders. Smoking cannabis is linked to development of COPD.
Cocaine rarely available & used in our area of Punjab is often used Party drug in Metro Cities.
Complications of substance use involve most specialties of medicine & are leading cause of morbidity & mortality.
The most ……
According to the UNODC's World drug report 2014 , there are approximately 1,80,000 drug related deaths each year, most of them due to preventable overdoses
Isolation, neglect of responibilities
I will throw light on………Maqboolpura ( near Amritsar),,,,,, has got the name……….. Widows’ colony ………………………….after 30 women were widowed here in three years Tribune 1999

Substance use is a constant drain on the finances of a family.
.
Addict pays for drugs by literally “begging, borrowing and stealing” …….with time self earned money becomes inadequate and family resources' get exhausted …. Turns to petty theft….and Gradually taking up other crimes.
Understanding the economics of drugs is important – an addict earns 6000 to 20,000 a month , earlier spending about 300 rs a day for his fix of opium/ BHUKHI ……… now he needs rs 1400 a day for Heroin …..so circumstances compel him to become a peddler.
Upto 40% of work accidents have been attributed to drug use.
According to National crime bureau, 60% of children in PUNJAB have tried some or the drug…… an ominous sign
According to UN , India is the largest consumer of Heroin in South Asia. with a business of 20 lakh crores per year.
