Discussion 2 – End of Life Care Eduardo Roqueso Florida National University Advanced Family Practicum III Jacqueline Alonso DNP, Family ARNP The ability to provide primary palliative care is a skill set that nurse practitioners should acquire, develop, and refine. Primary palliative care skills include thorough and effective symptom assessment and management, initiating and guiding conversations around advance care planning, and completing medical directives that guide care with life-limiting/life-threatening illness (Wheeler, 2016). Although sometimes confused with hospice, it’s important to remember hospice is a specific type of palliative care designed to address the needs of those who have 6 months or less to live. In other words, hospice care is always palliative, but not all palliative care is hospice care. (IPC, 2018). As frontline clinicians, APRNs, along with their RN colleagues, spend the most time with patients at their bedside. All APRNs must have fundamental palliative care skills. This includes understanding illness trajectories from diagnosis to death, appropriate assessment and management of pain, particularly in the current opioid misuse epidemic b (Dahlin & Coyne, 2019). The most recognized area of leadership for APRNs is Clinical Care. To be successful, APRNs must understand the diagnosis and the natural course of illnesses and diseases along with the critical decision-making points to manage and treat these conditions from diagnosis to end-of-life care, based on best available evidence. APRNs are in a key position to deliver collaborative interdisciplinary palliative care for patients with serious illness and move palliative care upstream to caring for populations across the lifespan and across the health care continuum (Dahlin & Coyne, 2019). In palliative care, when presenting with a symptom complaint, disease progression must always be considered. Disease progression needs to be identified to help with the ongoing goals-of-care discussions. Often-cited symptoms include pain, breathlessness, fatigue, anorexia, nausea/vomiting, constipation, depression, dry mouth, and sleep disturbance (Wheeler, 2016) In a perfect palliative care world, advance directives for care at end of life would be documented, medical powers of attorney would be named, and these documents would be shared with all health care providers. The reality is that this does not happen, and many hospitalizations are for a crisis when decisions need to be made and there has not been an open and honest discussion of the illness trajectory. NPs enter into these discussions without an agenda for what are the “correct” decisions. The desired outcome is to have the patient understand the issues and make informed choices based on their goals and their definition of quality of life. Health promotion by definition is “educating people about healthy lifestyles, reduction of risk, developmental needs, activities of daily living (ADL’s) and preventive self-care.” (Whitney et al., 2018). In order to live a healthy lifestyle an individual must know what a “healthy lifestyle” consists of. The same goes for reducing risks, ADL’s, preventative self-care and developmental needs. It is important for the healthcare professional or in our case, the nurse, to educate an individual continuously for the duration of the patient's care with the nurse. They should be educated on what the medications are that they are being given, as well as why it is being administered, dosage, route etc. For example, patients always want to know why they are getting Lovenox or Protonix when they have not taken it before. They should also be educated on diet choices, at the hospital that I work at, sometimes the patients will be placed on certain diets like nothing by mouth (NPO), American Diabetes Association (ADA), renal, heart healthy, low potassium etc., and they state that they do not have diet restrictions at home, so they have to be educated on why they were placed on the diet and they should consider this type of diet once discharged.
The nursing process is used in developing health education because the nurse must assess the patient. This assessment is not only a head to toe assessment, but the nurse must also determine the patients needs, readiness to learn and the patient's ability and desire to actively engage in their plan of care and meeting goals. Then the nurse makes their nursing diagnoses, for example is their non-compliance evident, immobility, learning deficit etc. The nurse then develops a plan of care and action that can be made with the patient and begins to implement the plan of care. The nurse and patient evaluate how well they are meeting their care goals and continue to develop a plan that changes to the patients needs and better assists with meeting goals. Throughout the whole process the nurse is educating the patient on what the problem is preventing the patient from getting better and then the nurse educates the patient on how they can work towards solving that problem.

For the topic you choose:
Establish a clear thesis about your topic as part of the introductory paragraph (often the thesis is the last thing one determines after doing the basic research and outline; however it will be placed in the first paragraph of your paper).
This is a comparative essay. Comparison approached properly will require some critical thinking on your part. Use a point-by-point approach for the essay. During early adulthood, people begin make important life decisions in three areas: career, family, and health. Young adults establish patterns of nutrition and physical activity that can have either positive or negative effects on their physical development in later stages of adulthood. They also begin to make decisions about what career path to follow. At some point during early adulthood, individuals also choose either to marry or remain single, and decide whether or not to start a family. Consider the following example:
Jeff is 23 years old. He recently graduated from college with a business degree and took a managerial position with a midsize company. He likes his coworkers, but often finds himself bored with his work. Although he was active in college, recently he has gained about 10 pounds due to his sedentary job and lack of time to exercise. Jeff broke up with his college girlfriend after graduation, and since then he has dated a few different women, but hasn’t had another serious relationship. Jeff wonders what the remainder of his 20’s and his 30’s will bring.
Based on your reading, prepare a PowerPoint presentation that would help individuals like Jeff set goals related to career, health, and family. The presentations should discuss the relationship between lifestyle choices and health risks. In addition, it should address how factors like personality and attachment may influence both career- and family-related goals. Finally, address diversity in the presentation. Explain how your plan could be adapted to fit the needs of different socioeconomic or ethnic groups.
Your PowerPoint presentation should include an introduction, a conclusion, and detailed speaker’s notes. Include your references on one slide.
Develop a 6–8-slide presentation in PowerPoint format with speaker’s notes. Apply APA standards to citation of sources. Use the following file naming convention: LastnameFirstInitial_M2_A2.ppt.

Discussion 2 – End of Life Care Eduardo Roqueso Florida National University Advanced Family Practicum III Jacqueline Alonso DNP, Family ARNP The ability to provide primary palliative care is a skill set that nurse practitioners should acquire, develop, and refine. Primary palliative care skills include thorough and effective symptom assessment and management, initiating and guiding conversations around advance care planning, and completing medical directives that guide care with life-limiting/life- threatening illness (Wheeler, 2016). Although sometimes confused with hospice, it’s important to remember hospice is a specific type of palliative care designed to address the needs of those who have 6 months or less to live. In other words, hospice care is always palliative, but not all palliative care is hospice care. (IPC, 2018). As frontline clinicians, APRNs, along with their RN colleagues, spend the most time with patients at their bedside. All APRNs must have fundamental palliative care skills. This includes understanding illness trajectories from diagnosis to death, appropriate assessment and management of pain, particularly in the current opioid misuse epidemic b (Dahlin & Coyne, 2019). The most recognized area of leadership for APRNs is Clinical Care. To be successful, APRNs must understand the diagnosis and the natural course of illnesses and diseases along with the critical decision-making points to manage and treat
- 2. these conditions from diagnosis to end-of-life care, based on best available evidence. APRNs are in a key position to deliver collaborative interdisciplinary palliative care for patients with serious illness and move palliative care upstream to caring for populations across the lifespan and across the health care continuum (Dahlin & Coyne, 2019). In palliative care, when presenting with a symptom complaint, disease progression must always be considered. Disease progression needs to be identified to help with the ongoing goals-of-care discussions. Often-cited symptoms include pain, breathlessness, fatigue, anorexia, nausea/vomiting, constipation, depression, dry mouth, and sleep disturbance (Wheeler, 2016) In a perfect palliative care world, advance directives for care at end of life would be documented, medical powers of attorney would be named, and these documents would be shared with all health care providers. The reality is that this does not happen, and many hospitalizations are for a crisis when decisions need to be made and there has not been an open and honest discussion of the illness trajectory. NPs enter into these discussions without an agenda for what are the “correct” decisions. The desired outcome is to have the patient understand the issues and make informed choices based on their goals and their definition of quality of life. NPs need to be prepared to discuss disease trajectory, realistic interventions, and prognosis.
