Diabetic Diet Education Guide
1. Why am I here today?
2. Do you follow a special diet at home?
3. What kind of diet?
4. What do you eat typically?
5. Meal pattern? B, L, D, snacks?
6. Have you heard of a diabetic diet? Why would you follow that?
7. What is a carbohydrate? Starches (beans, breads, pastas, tortillas, corn, cereals), Milk,
Fruit, Fruit Juice, Cakes/cookies, peas/corn. (have pt look at sheet with serving sizes)
8. Portion size: every cho has an assigned portion. This is the most important thing, can
eat carbohydrates just need to follow portion sizes.
9. Consistent meals throughout the day: B, L, D snacks.
10. Have you learned about CHO counting?
a. 1 serving of CHO=15grams, this means a serving is within the assigned
portion size
b. You can have______servings of CHO at each meal and ______snacks
c. That means there are______grams of CHO at every meal.
11. Look at this food label.
a. How many CHOs in a serving?
b. How much is a serving?
c. How many more CHOs can you have at this meal/snack?
12. What area of life isn’t working for you? Portion sizes, consistent eating, not knowing
what CHOs you’re eating? (If patient is a recurrent/non-compliant)
