In a state of the art minimally invasive endoscopic out - patient
surgical center specializing in treatment of the spine treating patient’s
from all over the United States and the world, perform pre-operative
history and physicals for surgical clearance. Order, interpret and
review with patient diagnostic testing including MRI’s, x-rays, CT’s,

Perform post operative assessments and clear patient to return
home. Educate patient regarding wound care, surgical complications,
activity restrictions and recovery process. Provide psychosocial and
emotional support to chronic pain patients. Directly supervise medical
assistants. Provide follow up phone calls to patients with post
operative issues or concerns that need to be evaluated and triage
appropriate plan of care. Collaborate with on-site surgeons and

physicians and mid level providers.
April 2007 - April 2010 Center for Radiation Oncology Brandon, FL
In a free standing radiation practice for adults, evaluated and
treated cancer patient’s of all type including breast, lung, prostate,
head and neck, and gynecologic. Unique is that this was a woman
owned practice and nearly fifty percent of the patient’s referred were
breast cancer. Main duties included radiation related symptom
management depending on diagnosis and area being treated during
the acute “on-treat” phase. Monitored high risk patients receiving
concomitant chemotherapy and radiation for complications. Provided
ongoing surveillance, diagnostic testing and physical exam to those
in post treatment follow up. Coordinate with staff nurses, medical
oncologist and other treating physicians involved with case, patient
families, and Hospice. Traveled between five offices in Hillsborough
County. Saw hospital consults at eight facilities and made
rounds as needed. Took call as needed. Radiation modalities
provided in the practice included 3-D conformal, IMRT, and
Accuboost. High Dose Rate Brachy therapies (HDR) included
MammoSite, prostate seed implants, endobronchial, and Leipzig.
Worked directly in tandem and collaborated with physician
partner – Dr. Kathryn Kepes. Intermittently worked with two
other physicians in the practice Dr. Virginia Goytia and Dr. Nagy
Elsayyad. Precepted students and mentored nursing staff. Areas of
interest included smoking cessation, female sexual dysfunction
and alternative therapies.

In an urgent care and limited continuing care setting for children and
adults, evaluated and treated work/non-work related acute illness

clinics for a large employer, performing initial evaluation and follow
up care on Workmen’s Compensation injuries, new hire physicals,

In long-term care facilities, provided primary health care to the
geriatric population using a unique managed care program.
Coordinated Medicare services and provided preventive and primary
care visits to identify and manage health care problems early. The
goal of this project was to reduce complications resulting in ED visits
and hospitalizations and improve quality of life and decrease
mortality and morbidity. Communicated and collaborated with the
resident's physician regarding changes prn and made rounds
monthly. Triaged after hour/weekend calls from the facilities and
communicated with the resident's family monthly and prn.

thyroid disease, asthma/COPD, depression/anxiety, CAD,

Lesbian population. Experiences included complicated HIV care and hospital
rounds with Dr. Hopkins and more generalized primary care with the two
nurse practitioners there including yearly exams with prostate or pap
screenings, skin biopsies and cryo therapy, as well as management of minor
acute illness. Unique to her office, all notes were done on computer with

April 2007 - April 2010 Center for Radiation Oncology Brandon, FL In a free standing radiation practice for adults, evaluated and treated cancer patient’s of all type including breast, lung, prostate, head and neck, and gynecologic. Unique is that this was a woman owned practice and nearly fifty percent of the patient’s referred were breast cancer. Main duties included radiation related symptom management depending on diagnosis and area being treated during the acute “on-treat” phase. Monitored high risk patients receiving concomitant chemotherapy and radiation for complications. Provided ongoing surveillance, diagnostic testing and physical exam to those in post treatment follow up. Coordinate with staff nurses, medical oncologist and other treating physicians involved with case, patient families, and Hospice. Traveled between five offices in Hillsborough County. Saw hospital consults at eight facilities and made rounds as needed. Took call as needed. Radiation modalities provided in the practice included 3-D conformal, IMRT, and Accuboost. High Dose Rate Brachy therapies (HDR) included MammoSite, prostate seed implants, endobronchial, and Leipzig. Worked directly in tandem and collaborated with physician partner – Dr. Kathryn Kepes. July 2003 – October 2005 Lakeside Occupational Med Centers Largo, FL As a solo provider, worked at two remote on-site occupational health clinics for a large employer, performing initial evaluation and follow up care on Workmen’s Compensation injuries, new hire physicals, and DOT exams. Also, provided urgent care for personal issues and counseling on health promotion and prevention. Coordinated Medicare services and provided preventive and primary care visits to identify and manage health care problems early. The goal of this project was to reduce complications resulting in ED visits and hospitalizations and improve quality of life and decrease mortality and morbidity. Communicated and collaborated with the resident's physician regarding changes prn and made rounds monthly.
