examination as appropriate. Collect and interpret diagnostic data as necessary. Perform
advanced diagnostic and therapeutic interventions. Determine medications and other therapies
in collaboration with a physician. Formulate plan of care in collaboration withchild, family, and a
multidisciplinary team. Evaluate patient/family outcomes and modify plan of care in consultation
with physicians and other health team members. Facilitate continuity of care and make referrals
as appropriate. Provideseducation to meet needs of staff, management, and community.
Contributes to education of students. Evaluate and implement research findings in clinical
practice. Demonstrates advanced communication skills. Maintain currency in field through
continuing education, literature and seminars. Implement this knowledge foruse within the
Medical Center. Ensure the exchange of appropriate patient care and clinical information when
patients are admitted, referred, transferred or discharged. Participatein the assessment of a
patient's needs, the organization's capability to provide care or treatment and the appropriate
