This document proposes a group award system to evaluate Performance Improvement and Accountability Groups (PAGs) for a Local Health Integration Network (LHIN). It includes: 1) A radial assessment tool to measure PAG performance on factors like goals, strengths, weaknesses, and impact. 2) Awards would be given after key reviews to recognize high-achieving, self-motivated teams and fuel competition. 3) Benefits include enhancing teamwork, improving performance, and recognizing achievement. Rewards could include plaques, certificates, or training courses. 4) The tool is demonstrated through a sample calculation for a cardiology team's performance assessment.
